What the care home does well Care plans are well organized and show that service users and their representatives are involved in the development of them. Medication is managed well. Complaints are responded to and people are protected. The home is well maintained, clean and homely. The acting manager has improved the standards in the home through taking corrective actions following audits and through communicating with service users and their representatives. These are some of the comments we received from service users and their relatives. "Good relations between staff and clients" "Look after my Dad. The girls are great." "Lots of wonderful home cooked meals. Lots of activities for people to choose to join. Friendly, caring staff around who try to chat." Date of last inspection Brief description of the care home Beaconsfield Court is a registered care home with nursing, providing personal care and accommodation for up to 32 older persons, some of whom may require continuous nursing care. The home is owned by Southern Cross Homes Properties Ltd and operates within the private sector. The home is located near to the centre of the Care Homes for Older People Page 2 of 9 Brief description of the care home market town of Barnard Castle, close to local amenities. The home is a three-storey building, situated in its own private, well maintained grounds. Personal accommodation and communal space is located on all floors. Service areas are located on the ground floor. There are 30 single bedrooms and one double bedroom. Twenty three bedrooms have en suite facilities. All bedrooms are suitable for wheelchair users. These are all fairly spacious rooms. The weekly fees vary between £441.60 and £544.50 (residential care without continuous nursing) and between £462.81 and £755.70 for nursing care. The fee covers all accommodation, meals and personal care (plus nursing care for those residents in receipt of continuous nursing care at the home). Hairdressing, toiletries, newspapers, plus services from private opticians, dentists and chiropodists are not included in the fee. However, the actual amount people pay will depend upon their individual circumstances. Care Homes for Older People Page 3 of 9 What we found: This inspection was carried out on 10 May 2010. The visit lasted for four hours. In October 2007, the home was awarded an excellent quality rating. The purpose of this inspection was to see if the home was continuing to maintain that standard. When we visited the home we looked at records and documents held there, we talked to service users and staff who work at the home and we spent time talking to the acting manager of the home. Before we visited the home we asked the acting manager to complete a self assessment document. This document is called the Annual Quality Assurance Assessment document. The acting manager completed and returned the document to us on time. We also sent out surveys to people who live and work at the home. We received eight surveys from people who live at the home and two from people who work at the home. None of the surveys returned gave us cause for concern. We looked at care plans and medication records, complaints and adult protection referrals, the newly refurbished building, staff training and supervision records, the management arrangements for the home, quality assurance systems and maintenance records for the home. This is what we found. We looked at three care plans. Although some signatures were missing from one care plan, they all showed evidence of the service user or their representative having been involved in the development and agreement of those plans. The care plans were well organized and easy to follow. The care plan that did not have signatures in place was known to the acting manager. The pages that required a signature had been highlighted to remind staff that signatures were required when the person concerned was willing and able to sign. We were also shown a plan that identified when each care plan needed to be reviewed. This plan was up to date and we saw evidence of those reviews having been recorded and involving service users and their representatives. We also looked at how medication in the home was managed and found that regular audits take place and actions had been taken to improve the system used. We completed a sample stock check and found no discrepancies between the records held and the medication in stock. We did notice some examples of good practice in place. For example medication care plans, PRN (as and when needed) medication guidance and separate administration records to aid better stock control of PRN medication. We checked the complaints register and found it was up to date. The home has also shown that it is knowledgeable of and complies with the local authority adult protection procedure having recently made a referral. The manager of the home has also ensured that the Care Quality Commission was informed of the referral. We were told that training in safeguarding vulnerable adults had been organized for staff and managers. This training uses the local authority guidance and training materials. The home was clean and well furnished. The people we spoke to liked the refurbished communal areas and told us that they had been kept informed of progress and invited to help choose colour schemes. Whilst walking around the building we were able to see that a number of different activities were available for people to do. Staff were available on each floor, however, at some points during the day only one member of staff was available on each floor as the other member of staff was taking a break. We also observed some areas of staff practice that could improve for example, we observed a Care Homes for Older People Page 4 of 9 member of staff discussing dignity issues in a communal area, we also witnessed a member of staff removing a drink from someone without asking them if they minded. There was also a plate of sandwiches left beside someone wrapped in cling film before lunch was due to be served. These observations were passed on to the acting manager. The acting manager brought us up to date with the proposed plans for how the home would be managed. We were shown examples of internal quality assurance documents and audits that show that failures have been identified and dealt with. The acting manager told us that service user surveys were due to be sent out by the end of May. We were shown a staff supervision planner which had not been brought up to date. The planner only showed dates for January, however, the manager was able to show us evidence of supervision having been completed during April. We asked three staff when they last had supervision and what it was about, two were able to say approximately when they last had a formal supervision meeting and what they discussed. No one knew when their next supervision session was due. We asked to see a sample of maintenance certificates held in the home. The acting manager was not able to show us an up to date electrical wiring certificate which should have been available at the home.
