Conditions of registration: 1. Ten service users in the receipt of personal care only can be admitted into the home. Date of last inspection 29 January 2005 Brief Description of the Service: Berehill is a home for service users in the category of older persons and older persons dementia. The home is located close to a small village. The building is a large older detached property with accommodation over three floors that is accessed by using a shaft or stair lift. This is an overview of what the inspector found during the inspection. The inspection took place over five hours and the manager was present for the majority of the process. The provider was also in attendance for some of the inspection. Care and staff files were looked at. Four members of staff, three residents and one family member were spoken to. The gathering of references should be from appropriate persons or checked after receiving them to confirm contents are accurate. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Resident’s needs are assessed prior to admission to the home. EVIDENCE: The four resident’s files that were looked at all contained a pre-admission assessment of needs. The form is divided up into different sections and allows for written comment from the assessor on the particular need assessed. The manager or the deputy manager, carry out all the assessments. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 8 and 10 The home is currently meeting the health care needs of the residents. The resident’s are respected and are given privacy. EVIDENCE: Each of the resident’s once admitted to the home if are registered with a local G.P. surgery. This gives access to the range of services available under the NHS. The dentist and optician visit the home at regular intervals. Changes in resident’s health care needs are documented in the daily notes and what actions have been taken. The resident’s and family spoken to felt they were treated with respect and given privacy. Staff were observed knocking on doors before entering rooms and speaking to then residents in a manner suitable to their needs. Medical treatments take place in residents bedrooms. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 The home will act upon any complaints it may receive. EVIDENCE: During the inspection the complaint log book was looked at and no complaints have been recorded for over two years. There is place a complaints policy and this is displayed in various parts of the home as well as the service user guide. The residents and family spoken to said if they had a complaint they would speak to the manager directly about their concerns. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 Residents live in a safe environment although the home is currently in need of redecoration in the communal areas. EVIDENCE: During the tour of the home it was noted that the environment is in need of redecoration. Some areas have already received attention but the majority of the home still requires work to make it a well-maintained place to live. There is a plan to carry out this work when building work on another site owned by the provider is complete. This is due to finish in the next few months. Although there is work to be done in the home it was very clean and had no unpleasant odours present on the day of the inspection. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 29 Residents are supported by a well maintained and skilled staff. Recruitment practices do not always safe guard residents. EVIDENCE: The duty rota for the previous four weeks was looked at and staffing levels appear to be meeting the needs of the residents. There is a registered nurse on duty at all times and is supported by a team of care assistants with different levels of skill and experience. Residents feel that staffing levels are a little low at times. This was discussed with the manager who stated levels remain constant even when fewer residents in the home. Files for three members of staff were looked at. One of the employees had a reference supplied by a senior carer rather than the manager. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 33 The resident have very little input into how the home is run. EVIDENCE: There is a resident’s survey that is completed on behalf of the resident by members of staff who ask the questions to the residents then write down or tick the reply. There are no residents meetings or forums but there are family/carer meetings held regularly.
