Conditions of registration: 1. The number of service users to be accommodated must not exceed sixty three (63). 2. The service users will be sixty five (65) years of age or older on admission. Date of last inspection 9 November 2004 Brief Description of the Service: Bernhard Baron Cottage Homes provide residential care and support for up to sixty-three older people. The premises comprise of one large single storey building, providing good quality service user accommodation, in single en-suite rooms. The main building also houses the spacious dining room, kitchen, lounges and offices. In addition there are twenty five self-contained cottages for more independent living. All accommodation is connected to a 24 hour call system.The buildings are set in large, well maintained gardens and car parking facilities are available. The home is well situated and is easily accessible by road. It is close to amenities, including the main line railway station at Polegate and is within five miles of the seafront and Eastbourne town centre. Bernhard Baron Cottage Homes is owned by the Quakers. It is a registered charity and is managed by a board of appointed Trustees. This is an overview of what the inspector found during the inspection. This unannounced inspection took place over six hours in May 2005. It found that all of the twenty National Minimum Standards that were assessed had been met and the overall quality of care provided was good. Service users and relatives spoken to during the inspection expressed satisfaction with the home, the staff and the service provided. A tour of the premises took place and documentation, including service user and staff files was inspected. One of the service users’ relatives, three of the staff on duty and seven of the sixty three residents were spoken to. What the service does well:
Bernhard Baron Cottage Homes is an established, well managed and well maintained service that continues to provide high quality care and accommodation for older people. Effective systems are in place for the admission and ongoing care of service users. Individual care plans developed from comprehensive pre-admission assessments ensure that an individual’s needs are met in a structured and consistent manner. Service users and, where appropriate, their relatives are involved in developing and reviewing individual care plans and the managers and staff work hard to create an open and inclusive atmosphere within the home. An extensive range of social, recreational and leisure activities is available in the home, which clearly reflects individual and collective interests and preferences A thorough staff recruitment procedure ensures the protection of service users. Staff receive effective induction and foundation training, regular supervision and are valued and supported by the management team. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 & 6 The admission procedure is comprehensive and thorough, ensuring that service users are admitted only on the basis of a full needs assessment undertaken by people competent to do so. EVIDENCE: Following a referral to the home, the manager carries out a comprehensive pre-admission assessment including any personal care needs, mobility issues, social and cultural needs and family involvement. As an indication of the reputation of Bernard Baron Cottage Homes, it was noted that there continues to be a ‘healthy’ waiting list of people interested in moving into the home. Relevant documentation, including a full needs assessment, relating to the most recent admission to the Home was inspected and found to be comprehensive, up to date and well maintained.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 & 10 Service users’ care plans are developed from a comprehensive assessment of an individual’s needs and enable staff to meet such needs in a structured and consistent manner. Policies and procedures for the control and administration of medication are effective with clear and comprehensive systems being in place to ensure service users’ medication needs are met. EVIDENCE: High quality care plans have been developed for each service user and are clearly and directly linked to the individual’s assessed needs. Service users’ individual plans that were inspected were found to be accurate, generally well maintained, and up to date. It was evident that residents are directly involved in the assessment process and their individual care planning and it was noted that needs assessments are agreed and signed by the service user Positive comments from service users included: ‘The staff are all very kind and know what help I need’.

However it was noted that some risk assessments had not been completed and showed no evidence of staff action or intervention. All service users are registered with local GPs and have access to other health care professionals, including District Nurses and physiotherapists, as required, via the surgeries. Nutritional screening takes place as part of the pre-admission assessment process. Satisfactory records relating to the receipt, administration, dispensing and storage of service users’ medication are in place and were found to be up to date and well maintained. A local pharmacist provides specific staff training in the control and handling of medicines, although at present only the senior care staff have responsibility for this. Service users’ medication is reviewed by the GP on a regular basis. Following personal risk assessments, service users in the cottages currently take responsibility for the ordering, storage and administering of their own medication and have signed to this effect. Since the previous inspection, such risk assessments are now regularly reviewed and recorded. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 & 15 Social activities and meals are both well managed, creative and provide daily variety and interest for people living in the home. Links with the community are good and support and enrich service users’ social opportunities. EVIDENCE: Independence continues to be promoted and encouraged within the home and wherever possible, service users are enabled and supported to make choices and take decisions affecting their life and daily routines. ‘There’s so much going on here. I’m so happy and the staff are wonderful’. There is a weekly activities programme and a monthly entertainment chart displayed in the dining room. It was noted that an extensive range of social, recreational and leisure activities is available in the home, which clearly reflects individual and collective interests and preferences, including singing, handicraft, music and movement, bowls, opera evenings, board games and film shows.

Service users are directly involved in running a regular ‘in-house’ shop and library service. Planned outings are also arranged. Religious observance and services are also facilitated. Service users are provided with a varied, wholesome and nutritious diet. At lunchtime a choice of meals is available and special diets are catered for. A weekly menu is displayed in the main dining room, reflecting service users’ preferences and including seasonal variations. The main house has set meal times, with lunch and teatime meals being served in a comfortable and spacious dining room. Service users residing in the cottages prepare their own breakfasts and teatime meal and collect provisions daily from the main kitchen. Service users spoken to expressed a high degree of satisfaction with the quality and presentation of meals. However a recent complaint was received following an unsuccessful attempt by the chef to introduce duck to the menu: ‘I’m a tough old bird myself but I am quite sure not as tough as the one today’. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 & 18 The open and inclusive atmosphere within the home enables service users, staff and visitors to feel able to express any concerns, confident that they will be listened to. Service users are safeguarded from abuse through robust policies, procedures and relevant staff training. EVIDENCE: A suggestion box is available that offers service users the opportunity to express their views and opinions. A service users’ meeting is held every three months. There is also a ‘Residents’ Forum’, which meets regularly, with managers, to discuss issues of concern or interest. Service users and members of staff spoken to confirmed that they would have no hesitation speaking to one of the managers or making a complaint if necessary and each person was confident that they would be listened to. The home has an updated policy and procedure for the protection of vulnerable adults, including a whistle blowing policy. This forms one of the ‘key policies and procedures’ that staff are now expected to sign to confirm that they have read, discussed and understand.

The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 & 26 The service is accessible, safe and clean and is clearly suitable for it’s stated purpose. Service users benefit from pleasant accommodation that is comfortable, well maintained and decorated to a satisfactory standard. EVIDENCE: Bernhard Baron Cottage Homes has been purpose built to a high specification and clearly meets service users’ individual and collective needs in a safe and comfortable environment. The home employs a full time gardener and the grounds are attractive, well maintained and accessible. Appropriate use of CCTV cameras is in operation. The main building and cottages, both internally and externally, are maintained to a high standard.

As previously documented, independence and individuality is promoted within the home and this is evident from the personalising of service users’ individual rooms, which clearly reflects individual tastes and interests. Several housekeepers are employed in the Home and infection control policies and procedures are in place. Levels of cleanliness were found to be high throughout. ‘I’m so pleased with my room and, as you can see, it’s kept so clean’. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 & 30 There are sufficient trained and competent staff on duty at all times to meet the assessed, low dependency needs of the service users. Thorough recruitment procedures help to ensure the safety and protection of service users. EVIDENCE: The stable and dedicated staff team is clearly able to meet the assessed, individual and collective needs of service users within the home. At night there are two waking night staff on duty and two additional on-call staff, one of whom is a manager. Staff files that were examined were found to be well maintained, containing all necessary information, including two written references, proof of identity and satisfactory Criminal Record Bureau (CRB) and Protection of Vulnerable Adults (POVA) checks. All new staff are provided with and sign a written contract, including a statement of terms and conditions. No volunteers are employed in the home.

The home operates a mentoring system for new employees and from discussions with staff and managers, it is both successful and beneficial. A three year rolling programme of mandatory staff training is in place and all individual training is recorded. Staff spoken to confirmed and acknowledged the support and training they receive. ‘The induction is good and there’s loads of other training that we do’. ‘The managers are great and everyone is so helpful’. ‘It’s the best move I ever made – coming here’. JUDGEMENT – we looked at outcomes for standard(s) 33, 35 & 38 Staff are aware of and adhere to up to date policies and procedures relating to health and safety, ensuring the health, safety and welfare of service users and staff. The home regularly reviews aspects of its performance, through an effective programme of self-monitoring and consultation, which includes seeking the views of service users and their relatives. EVIDENCE: Bernhard Baron Cottage Homes H59-H10 S21451 Bernhard Baron Cottage Homes V218036 110505 Stage4.doc Version 1.20 Page 20 The health, safety and welfare of service users and staff is of paramount importance within the home and staff training is provided in many aspects of safe working practices, including moving and handling; food hygiene; fire safety and first aid. All staff training is recorded. COSHH assessments and guidelines are in place. Temperature regulators are fitted to all hot water outlets, accessible to service users. All accidents, incidents and injuries are recorded and reported, as required. For the purposes of security, there is a CCTV system in place and digital external door locks. The home operates effective quality monitoring systems, including regular residents’ meetings and satisfaction questionnaires for both service users and relatives. Since the previous inspection, relatives’ questionnaires are now sent out – with a very positive response. ‘My mother is very happy at BBCH and I am most grateful to the staff, who are so helpful’. ‘We are delighted with the high standard of care our aunt is receiving at BBCH – Many thanks’. ‘BBCH is in a class of its own’. ‘Thank you for this questionnaire too – admirable’. The innovative service users’ forum, comprising four elected residents and management representatives continues to meet regularly. However there has been some discussion recently regarding the collective name of the group and its changing role and responsibilities. Monthly monitoring visits to the home by a member of the board of Trustees continue and their subsequent report provides useful feedback for the managers.
