Conditions of registration: This home may accommodate 39 older people who require nursing care. Date of last inspection 28th October 2004 Brief Description of the Service: Birchville Court is situated on the outskirts of Bushey Heath. The two storey period building with a modern ground floor extension to the rear is set back from the road in extensive grounds. Visitor’s parking is provided to the side of the main building. The home has been extensively modernised in the last eighteen months providing accommodation in 39 single rooms many of which have en-suite toilets and wash hand basins. A feature of the home is the overhead tracking hoists which have been provided in the bathrooms, shower room and toilets.There is a large lounge, sunroom, conservatory and dining room on the ground floor. The rooms in the older part of the house and the entrance hall are decorated in period style. This is an overview of what the inspector found during the inspection. This was a positive inspection. The home was clean and fresh on the day of the inspection. The inspector observed good interaction between staff and service users. The inspection took place on the 10th August 2005 and was carried out by one inspector. The inspection lasted five hours. Service users were appropriately dresses and were well presented. The home caters for very frail service users and provides ‘kirton’ for service users who would otherwise be unable to sit in the sitting room. Kirton chairs provide full support to enable a very frail person to sit comfortably and safely. Risk assessments have been completed to ensure the chairs are being used appropriately. Some very frail service users admitted to the home are in the last stages of their lives. The home is not registered to care for people with a terminal illness. It is recommended that the registration is reviewed to include a number of places to provide care for service users to have a terminal illness. What the service does well: What has improved since the last inspection?
The home has now started to conduct regular supervision with its staff. The Registered Manager and her staff continue to explore ways of enhancing the lives of its service users. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5 The home offers good information on the services it provides and the assessments carried out prior to admission are very good. EVIDENCE: The home has updated its Statement of Purpose to reflect the changes to the floor covering in the home. A very thorough, assessment of needs is carried out by the Registered Manager, prior to admission. If a potential service user’s needs cannot be met, a place is not offered. At the time of the inspection the home was waiting for a specialised piece of equipment to be supplied prior to admitting a service user. The home operates in a very open and transparent way families and representatives are welcome at all reasonable times. All service users are admitted for a trial period following which their needs will be assessed. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 The home provides very detailed, clear care plans that reflect the care carried out. The home meets service users health care needs in a manner that upholds their dignity. EVIDENCE: The care plans are excellent containing good detail written in a very clear easily accessible manner. They give details that include records of bruising, mental health status, falls prevention and body maps. All visits by health professionals are recorded. The home admits very frail service users and is keenly aware of the need for fully detailed care plans. The inspector read the care plan of a recently admitted service user. This contained all the appropriate and required information to ensure all identified needs can be met. The inspector observed service users being treated with respect and dignity and service users spoken with confirmed this. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13 The home provides a caring atmosphere where service users have their needs met. There are no restrictions on visitors to the home. EVIDENCE: A high number of service users are very frail. The home makes every effort to ensure they are not isolated in their rooms. Kirton chairs are provided to ensure they can be got up and spend some time in the sitting rooms. These chairs enclose the service users and ensure they are fully supported. Without these the service users would experience difficulty in spending time out of bed. The appropriate risk assessments have been carried out on all service uers who use kirton chairs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,26. With the exception of some of the ‘kirton’ chairs the home was clean and fresh. The grounds were well tended and safe to use by service users. The home meets the standards on size and the provision of bathing and lavatory facilities. EVIDENCE: On the day of the inspection the home was clean, fresh and odour free. The new floor coving in the dining room and sitting room is showing premature signs of wear and tear. The Registered Provider is aware of this and is pursuing the matter with the manufacturers. Some of the chairs in the dining room are also showing signs of wear and tear. When the inspector enquired, the Registered Manager informed her of the high needs of the service users mean they are in constant use. Additional chaiars must be provided to allow for deep cleaning. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard 26 Regulation 23(2)(c ) Requirement The Registered Manager must ensure that additional kirton chairs are provided to facilitate the chairs in use being deep cleaned.
