Conditions of registration: 1. This home may accommodate 39 older people who require nursing care. 10th August 2005 Date of last inspection Brief Description of the Service: Birchville Court is situated on the outskirts of Bushey Heath. The two storey period building with a modern ground floor extension to the rear is set back from the road in extensive grounds. Visitors parking is provided to the side of the main building. The home has been extensively modernised in the last eighteen months providing accommodation in 39 single rooms many of which have en-suite toilets and wash hand basins. A feature of the home is the overhead tracking hoists which have been provided in the bathrooms, shower room and toilets.There is a large lounge, sunroom, conservatory and dining room on the ground floor. This is an overview of what the inspector found during the inspection. This was a positive inspection. The inspector observed good interaction between staff and service users. Service users were appropriately dresses and were well presented. The home caters for very frail service users it provides ‘kirton’ chairs for service users who would otherwise be unable to sit in the sitting room. Kirton chairs provide full support to enable a very frail person to sit comfortably and safely. Risk assessments have been completed to ensure the chairs are being used appropriately. The home admits very frail service users some of whom are in the last stages of their lives and full support to the families of service users. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 & 11. Medication is administered and recorded appropriately. The home manages the passing of life with respect and dignity. EVIDENCE: The home has a room dedicated to the storage of medication. The temperature is controlled and medication including controlled drugs and stored and recorded appropriately. The home endeavours to offer a home for life to service users and the passing of life is respected and support and assistance is offered to the families of the service user. This includes offering meals and the staff being available to the families at difficult times. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 & 15. Service users have control over their lives where possible. The food served is of good quality and appealing. EVIDENCE: The home endeavours to offer choice and control to the very frail service users where possible. The Registered Manager has purchased kirton chairs to enable service users to be part of the home and to spend time in the communal areas. The kirton chairs offer more support and comfort than conventional chairs and can be easily moved around the home. The use of these chairs means that frail service users have more choice about where they spend their day. The food served on the day of the inspection was tasty and looked appealing. Complaints are listened to and responded to in a timely manner. Service users are protected from abuse. EVIDENCE: The home has a complaints policy that is followed. The Registered Manager operates an open door policy and uses this to defuse any problems by reacting immediately. The home follows Hertfordshire’s Protection of Vulnerable Adults. In the main the environment was clean and nicely decorated. The home has all the equipment necessary to enhance their lives and assist mobility. Rooms were personalised. The kitchen was in need of a deep clean. EVIDENCE: On the day of the inspection the home, with the exception of the kitchen, was clean and fresh. Bedrooms were decorated and furnished to reflect the personality of the service users. The home has sufficient equipment to assist service users with mobility. The home has an appropriate number of baths to meet the needs of the service users and the demands of the Care Standard Act 2000. The home is staffed by an appropriate number of well trained and supervised staff. EVIDENCE: The home follows a robust recruitment procedure. The staffing files inspected contained two references, proof of identity and clearance from the Criminal Records Bureau. All staff receive induction and mandatory training including moving and handling. Where observed, staff assisted service users with mobility in an appropriate manner and the interaction between staff and service users was positive. Service users spoken with informed the inspector that the staff were very gentle, kind and caring. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 31 – 33 & 38. The home is well managed and is run in the best interests of the service users. EVIDENCE: The Registered Manager is dedicated to the care of the service users in the home. She operates an open door policy to staff, service users and to their representatives. She provides strong leadership and management that protects the welfare of the service users. She is proactive in ensuring staff are aware of their duties and is fully aware of the extent of her responsibilities within the home. The home is run in the best interest of the service users and the manager is available to the service users and their families and representatives. The home ensures all equipment is serviced regularly fire fighting equipment is serviced annually and the home has a plan in place in the event of a fire. Fire drills are held regularly.
