Service users under 60 years of age to be admitted for respite care only. 29th September 2005 Date of last inspection Brief Description of the Service: Birdsgrove Nursing Home is registered for 67 Old Age (OP) and 20 Dementia(DE) service users above the age of 60 years and is part of a group of care homes belonging to Craegmoor Healthcare ( Southern Counties Care Ltd). The home is situated on the borders of Bracknell town within a residential area on the main thoroughfare to the village of Warfield. The home is divided into three units determined by the development of the establishment over time. The Surrey Unit ( 25 ) originally a large converted domestic home, the Berkshire Unit ( 42 ) a purpose built 2 storey extension, and the Kent ( 20 ), built within the last decade for service users with dementia. The visit to the home was a planned unannounced inspection to review the quality and outcomes for the service users living there. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 The home has systems in place to ensure that service users privacy and dignity are upheld. EVIDENCE: The service users are provided with privacy for personal care, consultations with their GP’s and treatment within their own rooms. Staff are instructed of how they should treat service users at all times in the induction process and were seen to address service users appropriately. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 The standard of cleanliness and routine maintenance of the home has improved. EVIDENCE: The home has continued with its routine maintenance, renewal of carpets and some repainting since the last inspection visit. The majority of the home has had renewed flooring and bedrooms have been re-decorated as they have been made vacant. Some of the furniture in the communal spaces and service users rooms is worn and shabby in places. Relatives did comment to the inspector regard the poor quality of some of the furniture in the home. The home was fresh smelling and the majority of the home is kept clean. Relatives identified that the standard of cleanliness regarding furniture is not always maintained to a high level. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30 The numbers and skills of the staff employed meet the needs of the service users. The recruitment of staff is carried out appropriately. Staff are provided with the necessary training to carry out their roles. EVIDENCE: The home is divided into three areas and each area has a staff team of nurses, care staff and domestics that responsible for ensuring that the service users needs are met. Service users and relatives commented that on occasions there was a delay in staff responding to calls for assistance. The home has ensured that over 68 of the care staff have achieved NVQ level 2 and /or 3. They have another 10 staff that have commenced training for both levels. A senior nurse has commenced NVQ 4 to support the management team in developing their skills and knowledge. Three domestics have obtained NVQ level 1 and two have commenced NVQ level 2 in Hospitality. The recruitment process is carried out with shared responsibility with the companies Human Resources department. The recruitment files reviewed supported that sufficient information had been obtained in regard to verifying the identity of the individual and all the appropriate employment checks have been done.
Birdsgrove Nursing Home DS0000010975.V280418.R01.S.doc Version 5.1 Page 13 The training programme for staff over the last year has focussed on ensuring that the mandatory health and safety retraining is achieved and that nursing skills are maintained. The home has recently commenced using a Personal Performance Assessment tool for the member of staff and the supervisors to identify training needs. The current training that has commenced and is planned for the next year includes for all staff a focus on caring for service users with dementia. The registered nurses are currently in a programme to develop knowledge and skills for catheterisation and caring for those with continence problems. There is a rolling programme of in house training for nursing skills, care planning, infection control and wound care. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35 and 38 The management team are a skilled and experienced to run the home. The new quality assurance programme and processes will ensure that service users views of the service are sought. The home has suitable processes in place to protect service users finances and valuables. The home ensures that the health, safety and wellbeing of service users are protected. EVIDENCE: The manager has attended training to support her role within the home. This has included updates on supervision and appraisal skills, management and has further training planned.
Birdsgrove Nursing Home DS0000010975.V280418.R01.S.doc Version 5.1 Page 15 The company has introduced a new detailed quality assurance programme to monitor standards and outcomes for service users in the home. A current exercise has been carried out in regard to the dementia care service provision provided in the home. This process identified deficits and planned programme of development and audit has commenced from this. The information from this and other quality assurance processes have not yet been formalised into a document that can be provided to service users or the CSCI. The home does not manage service users monies and has the necessary processes to support service users should they wish to do this themselves. These include assessment tools and secure lockable spaces in their rooms. Staff are given information in their induction process and the staff handbook in regard to protecting service users from financial abuse. The records for safe working practices and maintenance that were not reviewed in the last inspection were inspected. Routine programme of checks are carried out for fire safety, electrical equipment, moving and handling aids and the lift. Water testing and temperature control are monitored regularly.
