Conditions of registration: None Date of last inspection 13.09.04 Brief Description of the Service: Blackwood Road is a purpose built detached bungalow standing in its own grounds in the centre of a local housing estate. It has lawned gardens to 3 sides of the building and has its own driveway and garage.The accommodation consists of 7 single bedrooms and a range of communal facilities. The rear of the home is a patio area and landscaped features, which include a water fountain.All areas of the home offer good disabled access.Personal care is provided to people between the ages of 18 – 65 years who have a learning disability as well as some who may also have a physical disability.Easy access is available to the local shops and post office and a bus service which offers access to the city centre is nearby. This is an overview of what the inspector found during the inspection. This inspection took place over 6 hours and was carried out as part of the statutory twice yearly inspection process and also involved a member of the Commissions Customer Service Team who was present in order to gain an understanding of the services offered in a care home. All areas of the premises were viewed which included external areas and a range of records was examined. This included records of care as well as records that related to health and safety. Discussion took place with the staff on duty on the morning shift and also the manager. This discussion focused on the key worker roles and also individual service user plans. It was established that the people who live in this home prefer to be known as tenants; therefore this term is used throughout the report. Discussion took place with the tenants and time was spent having lunch with them and also observing staff practices. As most of the tenants do not always use speech as a way of communicating observations took into account how staff made use of body language and gestures as a way of communicating with the tenants. The judgements made are based upon the evidence available at the time of the inspection. What the service does well:
Observations confirmed that staff work well with tenants on an individual basis and involve them as much as possible in the day to day household tasks as a way of promoting and developing their individual skills. Staff offered support and encouragement to tenants in order for the tenants to make decisions and choices. Staff have a good rapport with tenants and humour is very much a part of the daily work with both tenants and staff sharing a laugh and joke about events that occurred throughout the day. Observations also confirmed that staff have the ability and skill to deal with a range of behaviours that challenge and in a way which has positive outcomes for the person who is presenting the challenging behaviour. The home is very thorough in their assessment process when considering new tenants moving into the home. This ensures that tenants who live in the home have a number of opportunities to meet any new tenant making the admission process a positive experience for everyone. Discussions with staff confirmed that they have a good understanding of individual tenants needs and were able to speak with confidence about the work they are carrying out.

The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3 , 4 Prospective tenant’s needs are always assessed prior to admission in order to determine that their needs can be met in the home. Each tenant is issued with a copy of his or her terms and conditions of residence, which clearly states what is to be provided in the home. This information assists tenant’s to make an informed choice about where they live. Trial visits to the home offers prospective tenants the opportunity to sample life in the home before deciding to move in. EVIDENCE: Amendments have been made to the Statement of Purpose following advice offered during the last inspection with all references to the NCSC being removed. There has been one recent admission to the home and this has taken place on a planned basis over the last 12 months enabling the prospective tenant to meet the people who live in the home, have meals with them and also stay overnight. Part of the admission involved comprehensive assessments being compiled in order to ascertain that the tenants needs could be met in the home. This assessment process involves input from other professionals including a psychiatrist and the home confirmed that staff could meet the individual’s needs. The family of a tenant were issued with a copy of the Statement of Purpose and Service User Guide prior to the admission and very early in the assessment process in order to inform them of the services provided in the home.

Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7 Care practices ensure that the tenants can live as independent a lifestyle as possible. Although a wide range of information is contained in the individual care plan and the assessment information is comprehensive, at times the specific information required to assist tenants and staff meet personal goals is not always available. EVIDENCE: Each tenant has an individual care plan, which is developed as part of the assessment process and identifies the areas of care that staff are involved in. Examination of case files for two service users confirmed that staff make good use of other relevant professionals to assist with the care plan process. In one instance a psychologist had been involved to support staff in dealing with a range of behaviours of one service user that are challenging. This has resulted in a planned and consistent approach being adhered to by staff, which in turn has had an impact on reducing some of the challenges from the tenant. One tenants care plan states that they are on a healthy eating option as a way of controlling weight but this is not fully recorded in the care plan in that it does not clearly state what the healthy eating options are. This is an area that requires further development.

This would also assist staff who are working in the home on a temporary basis. The manager has implemented a planning record audit checklist, which is used on a monthly basis. This identifies a summary of issues that need addressing and which assists with the in house quality assurance programme. Observations made throughout the inspection confirmed that staff support tenants to make decisions about dayto-day life in the home and this is also recorded in tenants meetings and individual care plans. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11, 12, 13, 17 Tenants are assisted to lead active and fulfilling lifestyles by having a regular community presence and by accessing a range of community facilities. This will assist in them leading a full and enjoyable life. A good range of meals is available to tenants, which meet their dietary needs. EVIDENCE: Discussion with tenants and also with staff confirmed that that they have opportunities for personal development with some tenants attending a local college. In addition to this tenants are encouraged by staff to become involved in the day to day activities within the home which includes housekeeping tasks and assisting with meals dependent upon their capability and this offers them the opportunity to develop their individual personal skills. Good use is made of the local community facilities with tenants using local shops and pubs as well as the local church. Good use is also made of the facilities in the city centre with tenants stating that they like to use the shops in the city when buying their clothes as well as going to the theatre.

Discussion with the manager confirmed that advice has been sought from a dietician to assist with a healthy eating diet for two of the tenants. Lunch was taken with tenants and this was a pleasant experience being relaxed and offering the opportunity for discussion during the meal. Staff offered support to those tenants who required help with eating and this was carried out in a professional and unhurried manner. Menus are available which confirm the variety of meals that are provided and which also reflect the personal choice of the tenants. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19 The tenants receive good support from staff to ensure that their personal, physical and emotional health needs are met. EVIDENCE: A range of information is available in tenants individual files and care plans, which confirms that their physical and health needs are met, and in a way, which they prefer. For one tenant their health needs are met using additional activities and equipment and this includes the use of a trampoline and also going to the swimming pool. Discussion with staff confirmed that the tenant has all of the necessary equipment as well as being able to involve their parent when going to the swimming pool. Observations made confirmed that personal and intimate care tasks are carried out in private and in discussion with tenants they confirmed that staff accompany them to doctor’s appointments when necessary. One tenant has a profound physical disability and is totally dependent upon staff support with all activities of daily living. Observations made confirmed that staff are fully aware of this person’s needs and carry out all aspects of care as detailed within the tenant’s care plan. Support is also provided by a specialist tissue viability nurse who assists with pressure care. Discussion with this tenants relative confirmed that they were very satisfied with the care being offered.

The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 Whilst some of the tenant’s communication skills are very limited, arrangements are in place through the complaints process to promote their safety and offer protection. EVIDENCE: The home have a complaints process in place which has been designed in an accessible format and includes pictures in order to make it more easily understood by service users. In addition to this tenants meetings are held on a regular basis and these are used to discuss any concerns or complaints that tenants may have. Evidence was available in written records to confirm that any complaints, are dealt with to the satisfaction of the tenants. Discussion held with staff confirmed that they also take into consideration any changes in tenants behaviour or mood to establish whether this is an indication that they are dissatisfied with something in the home. In discussion with the tenants they confirmed that they had no complaints and if they did they would inform the manager. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 26, 30 Tenants benefit from a well-maintained, homely and clean accommodation. This helps to promote a positive image for service users, and ensures they remain comfortable and safe. Tenants bedrooms are furnished to a good standard. This contributes to their comfort. Aids and adaptations have been provided to promote tenants independence and safety. EVIDENCE: Three bedrooms have recently been fitted with new carpets and plans are in place for a further three bedrooms to have new carpets fitted. All bedrooms are in good decorative order having recently been painted and the kitchen has also been decorated. A new washing machine and tumble drier have been purchased and also new furniture for the lounge. This has ensured that good accommodation standards are being maintained in the home. There were no noticeable hazards or safety issues at the time of the visit and tenants are encouraged to become involved in some of the daily household cleaning tasks with support being offered by staff. All areas of the home were clean and tidy.

One bedroom is fully equipped with appropriate equipment to assist a tenant with mobility and this equipment has also been provided in a bathroom to ensure comfort and safety when bathing. Tenants were happy to show off their bedrooms as part of the visit and this was a good opportunity to have discussion about life in the home. Tenants confirmed that they had been shopping with staff to choose items such as curtains and bedding and this had been an enjoyable experience, as they liked shopping. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32, 34 35 Robust recruitment & selection procedures and regular training opportunities ensure that tenants are appropriately supported and protected by a competent and qualified staff team. Staff work well together, are well organised and share information and views so that tenants care is consistent and well thought out. The manager routinely supervises staff so that the skills they use to support tenants continue to be developed. EVIDENCE: Examination of staffing rotas confirmed that sufficient staff were on duty to meet service users needs both inside and outside of the home and this was evident when throughout the inspection staff were involved in supporting service users to go out. There were four staff on long-term sick leave and another person who had gone on sick leave prior to the inspection. Discussion with the manager confirmed that agreed staffing levels were being maintained by the use of two staff from another establishment and also with two staff that are undergoing the authorities social care worker training scheme. This has ensured consistency of support to the tenants. Examination of files for new staff confirmed that thorough and robust recruitment procedures are in place. In discussion with the manager it was confirmed that a recent training course had been held with a psychologist for

There is a positive commitment towards staff undergoing NVQ training and the Senior Residential Officer has recently commenced NVQ Level 4. One person has achieved NVQ Level 3, and another staff member is working towards NVQ Level 3. Furthermore one person has just commenced NVQ Level 2 training. For the two new staff who are in post they are working towards achieving a certificate in working with people with a learning disability. JUDGEMENT – we looked at outcomes for standard(s) 37, 42 The manager is an experienced leader who has developed the service and improved the health, welfare and lifestyle opportunities for tenants. The health and safety of the tenants is promoted by a well-managed staff team, which guarantees that the health, safety and welfare of tenants is always promoted and protected. EVIDENCE: The manager is about to complete the NVQ Level 4 in care. Despite the difficulties of having key members of staff on long term sick leave the staff morale within the home is high and the manager is enthusiastic about the ongoing developments that the staff team are making within the home. Discussions with staff confirmed that the manager is approachable, supportive and includes staff in the decision-making processes within the home using staff meetings and staff supervisions as a forum for supporting them. The atmosphere throughout the visit was vibrant and a good rapport was present between the staff team and the manager and also the tenants. Examination of accident records and fire safety confirmed that they are being maintained satisfactorily.
