Conditions of registration: 1. No person falling within the category MD can be admitted where there are already 9 persons of category MD already in the Home. 2. The Home can accommodate 1 Service User within the category MD who also has needs within the category of PD 3. No person under 30 years and over 65 years may be admitted to the Home. 4. The Service Users admitted to the home who fall within the category MD must have an aquired Brain Injury 5. The Home can continue to care for 1 existing named Service User over the age of 65 years. Date of last inspection 7th March 2005 Brief Description of the Service: 144 Boughton Road is a Care Home, owned by the Richardson Partnership for Care, providing personal care for up to nine service users who have a Mental Disorder following an Aquired Brain injury. The Home is situated in Kingsthorpe, a suburb of Northampton, and is close to a local shopping centre and amenties. The Home is easily accessible by public transport. The Home is set back from the main road and includes a car park at the front of the house. All service users have single room accommodation, and communal space includes a lounge, dining room and conservatory. There is a large garden to the rear of the house, where staff offices are also situated. The Home has a mini bus that is used to transport service users to regular activities. This is an overview of what the inspector found during the inspection. This Inspection took three and a half hours and was undertaken at 11.00 a.m., and was the first statutory unannounced Inspection this year. The focus of Inspections undertaken by the Commission for Social Care Inspection is upon outcomes for service users. The primary method used during this Inspection was that of case tracking, which involved selecting two service users currently living at the home, and tracking the care they are receiving, through looking at their records, and observation of and discussion with staff and service users at the home. Other related documents were also looked at. The Inspector spoke to four staff members, a senior worker and the Registered Manager and also toured the premises. This was a positive Inspection. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,4, Very good information is available for service users entering the home, and their individual needs are assessed on a regular basis. EVIDENCE: • A very good, comprehensive, illustrated and bound book ‘Welcome to 144 Boughton Road’ is available to prospective and new service users. Staff confirmed that service users have opportunities to visit the home for meals and overnight stays, in a planned and considered manner, before coming to live there. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,9 Service Users are enabled to make choices, take acceptable risks, and to have their needs met. EVIDENCE: • The two files looked at contained very good care planning information and records, covering all aspects of that service user’s daily life. Care plans were well detailed and regularly reviewed, signed and dated. There was evidence of service user involvement in care planning. Staff confirmed that service users are consulted on a three weekly basis about their care plans and life at the home, and are actively involved in decision-making. This is commendable. The two service users tracked had specific cultural and ethnic needs, and very good work was being undertaken by the staff team to support these. One service user is facilitated to read newspapers from the country of his origin, on the Internet, since they are unavailable in this country. Records indicated that he is ‘thrilled’ when this is done for him.

The Inspector saw an example of this. This is commendable. Risk assessments, discussions with staff, and daily records indicate that service users are encouraged to develop independence. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15,16,17 Service users are enabled to enjoy a normal lifestyle and to access a good range of leisure and social activities. EVIDENCE: • The Inspector, throughout the Inspection noted very good evidence that the service users regularly enjoyed outings and activities. Service users talked about evenings out, including a recent ‘Wild West’ night, and there were photographs on the walls to support this. On the day of the Inspection a Mexican night was planned, with Mexican food for evening tea. Case files clearly indicated service user interests and activity preferences. Each service user has a daily activity programme tailored to individual needs, much of this being provided in house, and community based. Dedicated transport is available to facilitate this.
C51 C08 S12714 Boughton Green Road V244584 160805 Stage 4.doc Version 1.40 Page 12 • Boughton Green Road Some service users attend for specialist day services e.g. Headway, one day a week. • On the day of Inspection, one service user had been shopping for new clothes with a staff member, and the staff member described how she facilitated this outing, travelling on public transport and encouraging the service user to choose and make decisions about his new clothes. Service users are individually consulted about daily meals, and the Inspector observed some of this process during the Inspection. Lunch was provided in a relaxed manner, and service users had a choice about whether to eat in the dining room or pleasant conservatory, both having large wooden tables. Specialist dietary requirements are provided for some service users. Residents house meetings are also utilised for consultation about food and meals. Service users told the Inspector that the food was very good. Care planning documents evidenced that contact with friends and family is supported. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19 Good attention is given to the personal and health care needs of the residents EVIDENCE: • The home’s assessment process, individual profiles, and reviews, provide good information about personal support needs and routines. The Inspector saw evidence of a strong key working system. During the Inspection service users were observed to receive a good level of individual support from staff. There is flexibility about individual daily routines. One service user had just got up for a shower when the Inspector arrived. Service user care plans contained clear details about specific healthcare needs. Routine health care appointments were evidenced on file. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,30 Boughton Road provides a homely, clean and comfortable environment for the service users living there. EVIDENCE: • Service users were noted to be comfortable and relaxed in their home. Communal areas were nicely furnished, clean and pleasant, with good natural light. The conservatory provides a good view of the large rear garden. All service users have single bedrooms. The Manager confirmed that an extension is being planned, and that all service users will be provided with en suite bedrooms. One service user showed the Inspector his bedroom. This was clean, comfortable, and personalised. The Inspector was concerned that the window restrictor for this first floor bedroom had broken. The senior member of staff confirmed that this had been recorded yesterday in the maintenance book for urgent repair. The Inspector advised the Manager that a risk assessment should be in place if the repair was not immediate. This was drawn up and made available before the Inspection was completed, and this included the necessity for

The Manager also showed the Inspector a recent audit of the home that indicated window risk assessments were to be compiled in respect of all service users. JUDGEMENT – we looked at outcomes for standard(s) 35 Service users are provided with care by an appropriately trained staff team. EVIDENCE: • The senior member of staff on duty during the Inspection was qualified to NVQ level 3, undertaking NVQ level 4, in addition to being a qualified NVQ assessor. She spoke about the needs of the specific client group that the home provides care for, in a knowledgeable and confident manner. Another support worker, undertaking NVQ level 3 confirmed that training was given a high priority at the home. The Inspector spoke to a domestic worker and was impressed that she had received a planned induction. Other staff also confirmed that training and development sessions are held on a regular rolling programme basis for all staff. Some of this is provided in house, plus other Health and Social Care professionals also contribute. The Inspector looked at training schedules for June to December 2005. These indicated a good range of essential basic training (e.g. Health and Safety), plus additional training appropriate to specific service user needs. The Registered Manager demonstrated a commitment to staff training, and to ensuring that the staff group are NVQ qualified.
