Overall number not to exceed 22. 18th May 2005 Date of last inspection Brief Description of the Service: Broadlands Park is a care home providing personal care and accommodation for 22 older people of whom up to 4 may also have dementia. It is a privately owned and run residential care home. The home is located in the rural Norfolk village of Upton. The service is long established and is based in a period house, with accommodation on the ground and first floor. The home has large gardens to the front and rear, although only those at the front of the building are used by residents. The home has single and double bedrooms all of which are ensuite. This is an overview of what the inspector found during the inspection. This inspection of the service was unannounced and took place over the late morning and lunch time period. The inspector spent time with the management, toured the home talked to staff and service users and observed staff working. The atmosphere within the home was calm and relaxed with the majority of service users sitting in the lounges watching television or talking to visitors What the service does well: What has improved since the last inspection? Care must be taken to ensure information relating to care is always made available in such a way as to maintain confidentiality at all times. The

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2,3,5 Prospective service users are given the opportunity to visit the home and the services assessment process allows for good admissions to be made. Service users know what they will pay for the service. EVIDENCE: During the inspection the Inspector tracked the records relating to two recent admissions to the service and talked to the homes manager about the admission process. From these it was possible to see appropriate assessment had taken place and that these assessments allowed the home to confidently make judgements about meeting need. It was clear that it is normal practice for prospective service users and their representatives to visit and assess the home and copies of the homes contact were held in the service users file. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8,10 With the exception of visiting dental services the residents health care needs are fully met. Care must be taken to ensure that the delivery of personal care is done in a way which fully supports service users right to privacy and confidentiality. EVIDENCE: Examination of service user plans and discussion with the management indicated an appropriate regime for the meeting of service users health care needs is in place. The one exception to this is access to a dentist who will carry out visits to the home. This was discussed with the manager and whilst the situation is far from ideal the service have done everything that they can to rectify this and are encouraged to continue in their efforts. During the inspectors tour of the building it was noted that instructions to care staff on the delivery of personal care were displayed on the wardrobe doors in some service users rooms. In some cases these rooms were shared and in all cases private and personal information could be seen by anyone going into the room. Whilst the information was designed to ensure all staff followed an agreed care pattern, this is not good practice and not respectful of service users right to confidentiality. Such information must be available in such a way

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 The service helps residents to exercise choice and control over their lives. Service users are supported in maintaining the contact that they wish with family and friends. Appropriate activity and stimulation is provided. EVIDENCE: Discussion with staff and the management indicated a good level of awareness of the importance of helping service users exercise choice in their daily lives. The home has an open visiting policy maintains a good level of communication with service users families. The manager confirmed that if a service user’s spouse has difficulty in visiting then she will collect them in order to maintain the relationship. The inspector was able to track the approach the provision of entertainment through a detailed log. This showed regular structured activity with visiting entertainers and regular input from staff on duty. In the case of the latter this had included quizzes, reading aloud, singing, nail care and music and movement. Holy communion and a monthly service are held. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 The service has an appropriate approach to adult protection. EVIDENCE: The majority of staff at the home have received adult protection training in support of the homes policy and procedure in this area. The service manager confirmed that further training was planned and that this would include all staff recruited since the last round of training. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,26 Whilst longer term solutions to the problems posed by the building are still to be finalised, service users live in a generally comfortable, clean and homely environment. EVIDENCE: Whilst this inspection did not focus on the environment the inspector did tour the public areas of the building and some bedrooms. The building was clean and tidy with no noticeable odour. Since the last inspection the provider has complied with a requirement to improve access to the garden and a new ramp has been installed at the front door. The requirement to submit timed proposals for the buildings future have not been fully met in the given timescale, nonetheless the Commission is aware of the efforts being made by the providers in this respect and as such the requirement is repeated with an extended timescale. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 In order to offer full protection to service users the provider must follow a robust recruitment process at all times. EVIDENCE: The inspector tracked the employment records of the two most recent appointments. Although the process as designed is satisfactory it was noted that both applicants had references outstanding , but had still commenced work. The inspector was assured by the manager that they would both be working under supervision until their criminal record bureau checks were received and references were returned. Whilst the Commission will accept staff working in a supervised manner once they have had a POVA First check, this is on the understanding that all other parts of the recruitment process are robust and this includes the need to obtain references before working. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33 Broadlands Park offers a caring resident centred service. EVIDENCE: These standards were not inspected in any depth on this occasion, however the inspector judges that service users needs and wishes are kept to the fore.
