Conditions of registration: Date of last inspection 28 October 2004 Brief Description of the Service: Caldwell Grange is owned and managed by Warwickshire County Council. The home provides care and accommodation for 35 older people with a wide range of needs. The home is situated in a quiet cul-de-sac on a large housing estate in Attleborough, Nuneaton, with many of the service users coming from the immediate local area. There are local shops, a post office, pub hairdressers and churches located within half a mile of the home. Caldwell Grange provides accommodation on two floors. All the sitting areas are on the ground floor, where there is a large dining room, incorporating a bar, and four lounge areas including a conservatory. The self-contained day care accommodation is available to residents outside day care hours. The home is set in its own grounds with a well maintained and accessible garden area. This is an overview of what the inspector found during the inspection. This routine unannounced visit was made over two days on 27th and 28th June 2005 and lasted 7 hours in total. The inspection process involved a tour of the home, talking with the manager, examining records and care plans, observation of care practices along with discussions with residents, staff and two relatives who were visiting the home during the inspection visit. Window restrictors have been fitted throughout the home ensuring the safety of the residents. The care planning documentation has been updated and these continue to be regularly reviewed and evaluated ensuring that the care needs of the residents are kept up to date. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3. Standard 6 is not applicable for this home. Assessments are undertaken to ensure that the home can meet the person’s identified care needs. EVIDENCE: The admissions procedure included an assessment by the home completed before admission. Records showed that assessments had been completed and this provided the necessary information for the home to be able to provide the care needed. The community care plans provided by the social worker, as part of the individual needs assessment process, were seen within the service user plans reviewed during this inspection. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 and 10 Health needs of residents are met with evidence of liaison with health and social care professionals on a regular basis. Personal support is offered in such a way as to maintain residents’ privacy and dignity. EVIDENCE: Warwickshire County Council has recently introduced a comprehensive new care planning and quality management system for its homes. The records of two residents were reviewed during this inspection. Good risk assessments were observed with ongoing monthly reviews of these, which enable staff to monitor changing dependency levels. Residents received dental, chiropody care and saw the optician. All residents are registered with a local General Practitioners Surgery (GP). Residents spoken with felt that their health care needs were being met advising that they saw the dentist, the chiropodist, the optician and GP or District Nurse when necessary. Nutritional issues were identified and if necessary a nutritional plan is put in place. Holistic assessments had been undertaken for each resident living at the home. Throughout the inspection it was observed that staff knock on residents’ doors, offer choices and ensure that all personal care and consultations are conducted in private, this assists in maintaining the residents privacy and dignity. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 14 Residents were satisfied with their lifestyle in the home and they had been able to exercise choice and influence decisions affecting them. Contact had been maintained with relatives and friends of residents. Opportunities to access the local community had been made available. EVIDENCE: Visitors are welcome at the home and information is available in the home’s brochure of visiting arrangements. An Anglican minister visits monthly and holds a service in the home. Representatives of the Methodist Church, Roman Catholic Church and Salvation Army also visit regularly to meet with residents and offer communion as required. The programme for activities include monthly outings. The home has access to a mini-bus which has a tail lift and can accommodate 3 wheelchair users and a number of other adults. Eight residents and four carers were looking forward to going on a weekend outing to a seaside resort in July. Recent trips by the residents have been to a local garden centre, theatre trips to the local municipal theatre. There is a gardening group and a craft club. Other activities include Music and Movement, board games, reminiscence, quizzes and bingo. All residents spoken with felt there was sufficient activities offered by the home.

A number of residents regularly visit a local shopping area for coffee or a pub meal. Choices are available at mealtimes. This standard was not reviewed, however, lunch was served during the inspection visit and it was seen to be nutritious and tasty. Residents commented positively on meals provided by the home, one resident advising that the food is “very good” and confirmed that they are offered daily choices. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 23, and 24. The home provides a safe and pleasant environment, which is generally well maintained, for residents to live comfortably with suitable facilities to meet their needs. EVIDENCE: There are well-maintained enclosed gardens for service users to enjoy. The weather was warm during the visit and a number of residents were seen to sit outside enjoying the sunshine. The gardens are not enclosed and residents can leave the home as they wish. Risk assessments have been undertaken for those residents who like to walk outside of the home’s boundaries, to ensure their individual safety. A tour was undertaken of the home. All interior areas were found to be clean and generally well maintained. Some bedrooms require re-decoration with torn or faded wallpaper evident. One bedroom with a dark wallpaper would benefit from a lighter colour when it is re-decorated. A damp patch was seen in one bedroom and the décor in this bedroom would be improved by replacing both the wall and ceiling paper.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 30 The home employs sufficient trained and competent staff to meet the needs of its residents. EVIDENCE: The staff duty rotas for the period of a month were reviewed. These demonstrated that staffing numbers were being maintained within agreed levels. It was observed during both days of the inspection visit that the skill mix of staff was sufficient in meeting residents needs and there were suitable numbers of experienced and trained staff on duty. There are always five care staff on duty in the mornings and three to four staff on in the afternoon. The manager works supernumerary hours to those of the care staff. At night there are two waking night staff and an emergency on call system in place. In addition to this the home employs domestic, kitchen and laundry staff. All residents that were spoken to stated that the staff are the best thing about the home with comments such as “they are lovely”, and “nothing is too much trouble for them”. Records indicated that 14 of the 29 care staff employed at the home have, or are in the process of completing an NVQ level 2 and three staff members are working towards an NVQ Level 3. Training records were examined which demonstrated that staff have regular training on the diseases and illnesses associated with old age ensuring they are competent to do their jobs.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 and 32 The home has an experienced Manager and is effectively and well managed. EVIDENCE: The Registered Manager has 17 years experience in managing a care home for older people. There are clear lines of accountability within the home. The manager has regular supervision meetings with her line manager and has completed the Registered Managers Award. Observations made and discussions with both residents and staff indicated that the manager is available and approachable should they wish to discuss any issues. The County Council are in the process of addressing the need for the registered provider or delegated person to visit the home monthly and write a report upon the conduct of the care home.
