This inspection was carried out on 1st August 2007.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 9 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The home has reviewed its statement of purpose cum service users guide, which now appears more explicit. A warm and relaxed atmosphere exists between staff and the people who use services and the staff team are sufficiently skilled to meet the majority of needs of the people living at the service. The assessment of needs are up to date and so too are the care plans. The introduction of the outcome star is good as it allows staff and individual people who use the service to have involvement together to determine the level of needs and support required to meet set objectives. The design enables people to see where they are currently and what they could aspire to achieve. One individual is preparing to move to more independent living accommodation and it was good to see the home move more towards rehabilitation and independent living. The people that I spoke with had not concerns about the service.

What has improved since the last inspection?

A review of the statement of purpose was undertaken and all of the people who use services had up to date assessments of needs and care plans. All have been allocated a social worker and or CPN. The home has a more relaxed environment since an individual not suited to the home moved out. Cosmetic exercises within the premises such as the fixing of cupboard doors in the office, kitchen and landings were addressed.

To include 9 service users with mental disorder, one of which is over the age of 65. 22/11/2006 Date of last inspection Brief Description of the Service: Camden Park House provides twenty-four hour support for up to ten people with complex mental healthcare needs. The home does not automatically provide personal care i.e. physical intimate care is not within the admission criteria; however the potential is there if required. The service is operated by Umbrella, which is a mental health charity, which provides a full range of care and support services in North London. The building is owned and maintained by Community Housing Association and service users have license agreements. The house is situated approximately 10-15 minutes from Camden Town Tube station and there is access is the 253, C10 and 29 bus routes. Within the local area are a variety of local shops including a supermarket, hairdressers, newsagent, florist, chemist as well as cafés and public houses. A post office is sited within walking distance. The home is spread over three floors. It comprises of two houses adjacent to each other, which have been adapted to provide registered accommodation for ten people. On the ground floor there is a main office, managers office, staff toilet, a single bedroom, a kitchen-cumdining area, a main lounge and a non-smoking lounge. There is also a single toilet. The laundry is located in the courtyard, which is formed by the two properties and has been developed into a communal outdoor space. The average fee charged to service users is £580 per week Camden Park House DS0000010339.V334258.R01.S.doc Version 5.2 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This was an unannounced inspection conducted for Camden Park House. Two visits occurred to accommodate the assigned manager who had a planned supervision on the day of the initial visit. The deputy manager for the home had completed her last sleep in service and was making the last preparations for the forthcoming holiday arrangements with a number of people who use the service. The second visit occurred on 9 August 07 followed by an additional visit to the provider’s head office to inspect staff records. Evidence gathered for the inspection necessitated the viewing of various records and files in respect to staff and people who use services. Five service users surveys were returned and one received from a care manager. I spoke with five of the people who use services and four staff. A tour of the building was conducted with the manager assigned and some information from the quality assurance assessment was used in the inspection process. An application has been made to increase the number of registered places at this service from 10 to 11. JUDGEMENT – we looked at outcomes for the following standard(s): 1 and 2 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The previous report required that the Statement of Purpose cum Service Users Guide be reviewed so that prospective people who use services are given up to date information. A clear and consistent referral and assessment process continues to be in place. EVIDENCE: The Statement of Purpose cum Service Users Guide has been reviewed, which enables prospective people who use services to have sufficient information to make informed choices about living at the home. Information included in the quality assurance assessment states that visits from prospective people who use services will be diarised. The service already permits potential people who use services to visit and to meet existing residents. The home plans to explore the possibility to allow existing people who use the service to have input in the selection process for prospective people who use services. The service is adapting a rehabilitative approach for the people living at Camden Park House. Clear and consistent policies and procedures continue to be in place and the assessments of needs were found to be in order following examination of the files held for people who use the service. Systems are in place, which demonstrates that choice and independence are the key objectives for service users living at the home. EVIDENCE: The stated purpose of the home is to support people who use services to gain sufficient life skills and confidence to hopefully move into semi independent or independent living accommodation. Care plans were up to date and the introduction and implementation of an outcome star designed and used in conjunction with the needs and choice of individual people who use the service to indicate the level of needs and support necessary to enhance their independent life and social skills and educational requirements. It was interesting to note the input from an individual, who clearly indicated that their abilities and level of needs significantly differs from the outcome Camden Park House DS0000010339.V334258.R01.S.doc Version 5.2 Page 11 levels outlined by staff. The indicators also reflect that where individual people believe that they are and what they want to aspire to. It will measure the achievements in a gradual process and is a good and easy system to work with. The outcome star takes into account risk management plans and allows appropriate support in respect to the provision of services and is person centred. To this end, the supporting needs of people using the service are appropriately planned according to individual needs and a care manager also confirms this. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15. 16 and 17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Systems are in place that offers a range of opportunities to people who use services. Improvement has been made in respect to the meal arrangements since the previous inspection. EVIDENCE: People who use services continue to enjoy an assortment of activities with support from their key workers. The staff team ensures that people who use the service are supported to enhance their personal development through engaging in everyday living skills and interests such as cooking in the home, shopping and to participate in household chores. Some individuals asked for support with their laundry. One person undertakes voluntary work at the provider’s headquarters; this individual very proudly showed me their artwork, which are proudly framed and hung up in the lounge. Camden Park House DS0000010339.V334258.R01.S.doc Version 5.2 Page 13 Staff were in the process of preparing for the forthcoming holiday with a number of people who use the service. An individual that I spoke with demonstrated disappointment in not being able to make the trip to Ilfracombe, due to a broken ankle that was in plaster. Additional support was brought in to provide assistance to the person who uses the service to encourage them to rest the ankle so that it would heal. The increased independence of individual living at the home is to be praised. This person is waiting for an offer of accommodation to enable him to more into more appropriate accommodation. They are bidding each week via the Council’s Housing Scheme and they were not keen to go on the trip to Ilfracombe. Someone else, with whom I spoke, looked forward to going on the planned holiday and confirmed their plan to give up smoking whilst there. Computer training tutor is brought in to show those who wish to learn or develop their computer skills to do so. Staff on duty commented that people who use services also participate in gardening and a magazine group. People who use services are supported to maintain family contacts and an individual spoke briefly about visiting their relatives. Observation and discussion with two individuals confirmed that these individuals are able to prepare and cook meals for themselves and meal arrangement has improved since the previous inspection, in that all of the people who use the service are now afforded the opportunity to state their preferences. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 and 20 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The personal and healthcare support of people who use services at this home are generally met though some improvement is required in respect to the support needs of an individual. Medication management is generally sound. EVIDENCE: As stated in the previous report support staff at this home does not provide personal care to people using the service. Personal care such as bathing is provided via social services; it would benefit the person who uses the service if personal care was provided by support staff, as opposed to having no control over the consistency in the approach to care delivery and ensures dignity for the person using the service. A care manager informed me since the previous inspection that staff had not supported and individual who use services to change his bedclothes for three months. This individual must be appropriately supported to change his bedclothes regularly, especially as this individual suffers from a skin condition. A care manager states that the home could improve the service by introducing regular rota for bed sheet changing to ensure it happens for those clients less inclined to do it themselves. Camden Park House DS0000010339.V334258.R01.S.doc Version 5.2 Page 15 An individual diagnosed with a hearing impairment should be facilitated to hear the television and the introduction of a loop system would benefit the people who use services in this regard. This home generally encourages health promotion and there is evidence that appropriate support in respect to specialist services is provided as necessary. People who use services are encouraged by staff to self medicate as appropriate. One person continues to manage his or her own medication. Staff states that they continue to experience difficulty in knowing the best way to support and individual whose first language is not English. This individual continues to exhibit aggression to staff and physical assault on an individual. In addition the needs of another person is not appropriately managed in this service and it is not clear whether alternative accommodation is being sought for this person to resettle. Also, because of the age of this individual staff are of the opinion that a move might also do more harm than good for this vulnerable individual. Medications prescribed for an individual were out of date and found stored above a cupboard in the office in a plastic box. Unused prescribed medication must be returned to the pharmacy in line with the home’s protocol. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Systems are in place to ensure that service users are listened to and encouraged to give their views. Staff are trained in respect to the protection of vulnerable adults though the existing policy needs review. EVIDENCE: People who use services continue to have their views listened to and they are afforded the opportunity to do so, in house meetings, key work sessions and privately. Those with whom I spoke confirmed their knowledge in respect to the complaints policy and procedure at this home. There were four complaints made since the last inspection all made against the people that expressed challenging behaviours. A complaint was made against an individual residing at the home who physically assaulted another individual by pouring hot liquid on them without provocation it was alleged. As stated earlier, another individual unsuited to the home was transferred to hospital for reassessment and would not be returning to the home. People who use services were reluctant to use the lounge when another individual was present. One of the people using the service had hot liquid thrown over them by another in an unprovoked situation and following which the police and the Commission were notified. A returned service user survey states, “I am not happy with the charges on my finances incurred by the Court of Protection.” Staff should involve the Camden Park House DS0000010339.V334258.R01.S.doc Version 5.2 Page 17 allocated social worker in this regard to ascertain how to go about supporting this individual. An appointee is being sought for an individual who has difficulty in managing their finances; this practice would prove to be very challenging for staff as the individual concerned looks forward to receiving a personal Giro and displays challenging behaviour when a Giro is late or not issued. The previous report required that the home obtains the host authority’s adult protection protocol and to align the policy and procedure to their own. I was informed that the adult protection protocol is under review, so this remains outstanding. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 27, 29 and 30 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Efforts are made to ensure that the environment standards are kept in good order. EVIDENCE: Previous requirements related to the non closure of fire doors, broken furniture being left on stair landing, leaking toilet and to carryout the matters outlined in the previous fire. The broken furniture stored on the stairway landing previously had been removed but additional items were stored. These were removed by the second visit to the home. Similarly, the door to a room for a person using the service was not properly closing and again staff addressed this by the second visit. Two toilets had no seats fitted to them as observed at the initial visit and remained outstanding to be addressed by the second visit. Various large cracks to some of the internal walls were observed in the property, which staff stated are under investigation by the landlords. Camden Park House DS0000010339.V334258.R01.S.doc Version 5.2 Page 19 Also observed was a ladder stored in the dining room and an empty picture frame. The glass in a picture frame housing the registration certificate for the home remained broken at the time of the second visit; Furniture in the lounge area were old and their materials covering them were in poor condition. The acting manager agreed that the furniture required upgrading to further enhance the home’s appearance. An application has been made to increase the number of people who use the service. The purpose of the room, which has en suite facility, is to prepare individuals for more independent living. The room proposed for registration is suitable in terms of size but the design and lay out of the room means that the bed could only be located under the window by a radiator. The radiator will need to be encased to safeguard and protect people using the service from harm. The home must invest in a loop system to facilitate an individual with hearing impairment to hear the television in the communal lounge. The home was clean and hygienic. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 33, 34 and 35 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The home’s recruitment procedural practice in respect to agency staff supplied to the home needs to be reappraised to ensure that service users are fully protected. EVIDENCE: There is evidently mutual respect between staff and people who use services. Camden Park House has a skilled team of staff with over 50 having the appropriate NVQ qualification and able to meet the needs of people using the service. The previous report required robust vetting of agency workers. At the previous inspection the home’s manager requested information in respect to the suitability of the agency workers to be faxed to the home. It was accepted that staff in the home needs to be able to verify the agency workers supplied to the home. At this inspection, there was no information available regarding the workers suitability to work in the home. The agency workers also confirmed that the agency had not supplied them with photo identifications, which would enable staff to check their identities. Camden Park House DS0000010339.V334258.R01.S.doc Version 5.2 Page 21 At the initial and follow up visit one staff was observed on duty which could potentially put people who use services and staff at risk, particularly when individuals are exhibiting challenging behaviours that impact on others. The previous requirement for sufficient number of staff on duty is repeated. The quality assurance assessment states that the home plans to fill the vacant posts within 12 months and provide clinical supervision to support staff in their roles. A visit to the provider’s head office to check the information held for locum staff highlighted that there was no evidence to demonstrate how locum staff are supported and no up to date the training records. The previous requirement is repeated in respect to the provider ensuring robust recruitment protocols are in place for agency and locum workers to ascertain the protection of people who uses the service. The home is reliant upon the use of agency and locum staff, as two vacancies existed at the time of the second visit. Strong efforts must be made to ensure the permanency of staff to the vacant posts to ensure continuity and consistency of service delivery, particularly since the home is moving towards preparing people to move on to more independent living in the community. The training records for permanent staff are available; a care manager associated with the home commented that the service could improve if staff were trained to provide personal care. Staff continues to work hard to ensure that the needs of an individual continues to be met and it was suggested in the last report that specialist supervision is provided to staff working at this home. It was pleasing to note in the quality assurance assessment document that the provider is considering the provision of clinical supervision. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39, 40 and 42 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Some requirements remained outstanding to be addressed, which impacts on the smooth running of the home. Service users have opportunities to comment on the service and on their individual care. The health and safety of service users continued to be compromised by potential hazardous situations relative to the environment. EVIDENCE: Efforts are made in respect to managing the home well. The home’s manager is on secondment and a deputy manager from another of the provider’s registered services is assigned as temporary manager for the home. The assigned manager is knowledgeable and capable to manage the home; the outstanding requirements have not all met compliance, which impacts on the management of the home. The home has a variety of policies and procedures, which ensures the rights and best interests of the people who uses the service. The previous report required that some Umbrella wide policies and procedures in draft format are formalized and implemented. This recommendation remained outstanding together with four requirements. Health and safety standards in the home are generally satisfactory; the service contractors outlined in a fairly recent report that the fire alarm panel system is old and needs upgrading. Additionally, the risk assessment undertaken by the home was partially compliant according to the London Fire Civil Defence Authority (LFCDA) and they commented that specific measures must be considered in respect to giving early fire detection warning to an individual user of the service.
