Conditions of registration: 1. 4 Dementia (DE) - Minimum age 57 years on admission Date of last inspection 6th July 2005 Brief Description of the Service: Cameo is located on the periphery of the town of Tamworth positioned at the end of a cul-de-sac. The home was registered some years ago to provide care for ten older people. The service users presently at the home would not easily access the small selection of shops in the area. Standing in its own grounds Cameo is a two-storey home; car parking was located at the front of the home. There was a garden at the rear of the home. Within the home was a small office, two lounges one used as a combined dining room. Cameo had only one shared bedroom, there were no en-suite facilities provided. This is an overview of what the inspector found during the inspection. This unannounced inspection was completed on the 19th December 2005. Assistance for the inspector was provided by the care manager, staff and where able residents. Records, documents and reports were provided upon request. The home was maintained to exception standards, this homely environment was comfortable and warm for the resident group. At the time of this inspection the home was operating to its full capacity. The homes rolling programme for decorating identified that the hall stairs and landings had been recently decorated and coloured in a pale cream. Accommodation is located on two floors accessed via the stairs or shaft lift. New dining room chairs had been purchased since the previous inspection there were plans to lay new carpet in the entrance and hall. No person is admitted to the home without a full assessment of their needs. Arrangements were in place for any of the residents to receive a variety of professional care, this was evidenced during the inspection when three District Nurses called to attend to certain residents. The staff on duty demonstrated their knowledge of the residents needs and addressed them appropriately. A comment made by one social worker following a review was “ the home is doing a fantastic job” Residents confirmed that they were satisfied with the home the care and staff. The cook prepared the meal of the day, she was aware of the likes and dislikes of each person. What the service does well:
Cameo is a warm establishment with a homely atmosphere, residents were provided with an exceptional standard of hygiene. The staff observed during the inspection were competent to undertake their roles as carers. They were committed to assisting the residents to continue with their life style. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,3,4,5 Standard six was not relevant to this home. The information readily available would enable a prospective resident to make a choice of the facilities provided. Pre-assessments of an individuals needs were part of the homes policy. EVIDENCE: The Statement of Purpose remained available; there had been no reason to amend the documents relating to the service provided. Assessments continued, this was evidenced in the plan of care for one of the recently admitted residents. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10. Arrangements were in place for the continued health care of the residents. There was a requirement that each of the residents had a completed plan of their needs. The medication system was satisfactory. Staff members were competent in their role of carers. EVIDENCE: Three care plans were seen during the visit including two new residents. There was evidence of an assessment taking place prior to admission. One of the new residents did not have a plan of her needs completed by the management; there was however a plan of needs provided by the hospital. Other plans did identify individual needs and support required, the risk assessment and the action to minimise any risk. This was evidenced during the inspection when three separate nurses arrived to attend to certain residents. The medication system remained satisfactory with the records being current. Medication was stored appropriately within a locked trolley. The staff on duty were warm in their approach to the residents who responded to the support. One resident was much more alert since the change in her medication, she is less vocal. The staff worked as a team to promote a homely atmosphere where the priority was the resident. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 15 The home had a somewhat limited social activity programme; staff were the main stimulus for in house activities. Menus were based on the preference of the residents. EVIDENCE: The staff on duty at any one time would try to encourage residents in simple exercises and games, this is not an easy task; but the staff recognise that not every person wants to be involved. External entertainment is limited, carol singers recently came to the home, and films are also supplied. And the provider provides music, and a library is available for any resident to use. Menus were based on the likes and dislikes of residents. The inspector evidenced this when one resident chose cold meat to the corned beef hash of the day. Menus were based on an old fashioned style of food, balanced and plentiful. Home cooking was prepared wherever possible. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 The complaints process was displayed in the relevant document form; and available to any person. Residents were protected from abuse by the training provided. EVIDENCE: The Commission had received no complaints; the provider confirmed that she had received no complaints about the home or service it provided. The complaints process was made available to any person, staff and residents. The residents were protected via the awareness and training for any form of abuse that may occur. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 20 21 23 24 25 26 The home offered a safe and comfortable environment where residents could continue with their life style. EVIDENCE: Located on the periphery of the town of Tamworth Cameo provided high standards of hygiene throughout the home. The recently completed decorating has lifted the hall, stairs and landing. There were plans to re-carpet the entrance hall and corridor. New dining room chairs had been purchased. Residents were encouraged to personalise their bedrooms. The deputy care manager was developing the audit system for the home; this will ensure that on a monthly basis areas were checked. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,29,30 The staffing levels were satisfactory for the number and dependency of the present resident group. Staff members were competent in their role as carers for vulnerable older people. EVIDENCE: At any one time with the exception of nights there would be a minimum of three staff on duty in the morning, reducing to two for the afternoon shift; supported by the catering staff. At the time of this inspection there were no vacancies for staff. Evidencing staff records one person had only one written reference; the provider had taken a verbal reference when the pre printed form sent had not been returned. The inspector was told that often occurs when other homes or business did not complete the pre printed form. It is essential that two written references were provided, this may entail the person providing an alternative persons name. The number of staff with NVQ in Care had reduced by one; the deputy care manager had Level III NVQ in Care. The provider will not be registering for the Registered Managers Award Level IV NVQ. There was a need to pursue training for the remaining staff in respect of Moving & Handling. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,36,37,38 The home was operated to the best interests of the residents. The provider was in control of the running of the home. The home continued to develop in its records, receptive to any advice provided. There was a requirement for the provider to inform the Commission of the death of a resident. The home had a relaxed ethos where the resident’s care was the priority. EVIDENCE: The provider and her deputy had an uncomplicated relaxed style of caring, this was cascaded to the staff team. Suggestions were made to the lengthy supervision document, that perhaps the whole document could be used in stages throughout the year.

Records evidenced in respect of fire were in the main satisfactory the details for fire drills for the night staff were less informative, it was recommended that perhaps the time and date could also be recorded as for the day staff. The Registered Provider(s) must comply with the given timescales. No. 1.
