The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Residents were cared for by staff and, during the inspection demonstrated their commitment to the care of individuals. They recognised that each individual had different needs. Residents were spoken with and explanations were given prior to moving/assisting a resident. The home continued to move forward with the documentation and organisation of the paperwork. Staff training was being encouraged, with staff committing their time to NVQ level II in Care. The home was comfortable and homely; residents were observed to be comfortable in the two lounges. The management respond to any advice proposed and addressed any requirements and or recommendations.

What has improved since the last inspection?

A New microwave had been purchased. Mandatory training had been completed plus and extra course for literacy.

What the care home could do better:

The home was well maintained, this standard will be further enhanced with the re-decoration and new furnishing proposed by the provider. This report made one requirement and one recommendation. The home was registered some years ago to provide care for ten older people. The service users presently at the home would not easily access the small selection of shops in the area.Standing in its own grounds Cameo is a two storey home, car parking was located at the front of the home. There was a garden at the rear of the home. Within the home was a small office, two lounges one used as a combined dining room. Cameo had only one shared bedroom, there were no en-suite facilities provided. This announced inspection was completed on the 6 July 2005 with the provider and the deputy each of whom contributed to the inspection, as did the staff and residents. Documents and records were provided based on the National Minimum Standard requirements. Residents were enjoying breakfast at the time of the inspection, a choice was offered and some residents enjoyed a bacon sandwich others preferred a continental style breakfast. One resident told the inspector that he had to wait for his breakfast until after the District Nurse had been due to his medical condition. The Commission had received three comment cards from residents all of whom indicated they were happy with the care, staff and food. Four comment cards were received form relatives. Two made additional comments “if possible it would be nice if there were more activities other than the T.V. They realised that not all the resident would want to join in “ “we are very satisfied with the care and attention my mother receives in the Cameo. The staff are very generous with the their time and commitment to the care of my mother. The Community Psychiatric Nurse visited the home and offered his opinion of the home and staff “ he was very satisfied with the care of residents under his care.” “One person in particular who had recently returned from hospital where he had concerns about her health had improved so much since coming home.” One of the staff that had previously worked in larger home preferred the small environment because as a carer she had more time to spend with residents. One of the residents confirmed that she had had a bath the previous night and had her hair washed. Comments from residents were positive and each one spoken with were comfortable and satisfied. At the time of the inspection there was one permanent vacancy and one resident in hospital returning after the assessment by the hospital. The deputy had already completed her re-assessment of the lady. Residents accommodation is located on the ground and first floor, the sample of the bedrooms seen were personalised to suit individuals. The home maintained exception standards of hygiene. There were plans to decorate the lounges, purchase new blinds for the small kitchen area, and to purchase new dining room chairs. The Statement of Purpose remained available to any person visiting the home. Each person admitted to the home would have a full assessment of his or her needs prior to admission. Home cooking was prepared wherever possible. The menus displayed a tendency for the old fashioned style of food. Special diets were catered for when appropriate. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,4,5, Standard 6 is not applicable to this home. The management and documentation made available provided prospective residents with detailed information about the home. Assessment procedures were in place to ensure that the home can meet individual needs. EVIDENCE: The Statement of Purpose and Service Users Guide were available to any person visiting the home to make a placement. Each of the residents were provided with a contract of the terms and conditions for the home. A trial and introduction invitation period was part of the homes commitment to meeting the needs of individuals. There was no appointment system to view the home. This was evidenced with the new resident who confirmed that she had been seen before she came in. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10 ,11, The appropriate arrangements were in place to ensure that the health and personal needs were met. The systems used for the administration of medicines were satisfactory and training had been provided. EVIDENCE: Each of the residents had a care plan identifying their needs; the plans were available to all the staff. Reports on the daily routines were entered when necessary. Reports for any medical intervention were identified in a different colour for easy reference. Information was gathered from various sources and entered into the plan; and reviewed on a regular basis. Weights where able were taken and records evidenced. It was suggested that if a resident who was unable to be weighed and gave concerns then a fluid/food chart should be maintained. Arrangements were in place for continued care from other agencies this was evidenced during the inspection. Staff appropriately sat with residents and assisted them in a sensitive manner when dining. Each of the residents that required more support were spoken to before any action was taken. Staff enjoyed the relaxed style of the home, which allows them to spend more time with individuals. The inspector had no concerns following the discussion with staff and relative in respect of the care provided during the recent death of a resident. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,15, When entering the home there was a relaxed comfortable atmosphere, the staff interacted with the residents in a sensitive light manner. The residents had choice when dining, from the daily menus. EVIDENCE: The staff were the main people that stimulated residents with music, singing together, gentle exercises and simple large games stable for easy use by less dexterous people. Entertainment was arranged on a limited basis perhaps twice each year. The social event in the town had been withdrawn. It is hoped to resurrect the event in an alternative form. Visitors were welcomed during the inspection, and spent time in the home. One relative was spoken with and was satisfied with the care provided. The menu contained an old fashioned style of meals. Based on the likes and dislikes of the residents. The cook maintained the required temperatures evidenced from the records on a daily basis. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 18 The home had a complaints procedure displayed; the information provided the appropriate information to be used by any person with a concern. EVIDENCE: There had been one complaint raised against the home in 2005 this had been investigated, the report had been sent to the provider. The complaints procedure was displayed within the required documents and within the hall. The staff as part of the NVQ and in house training; verbal instructions ensured that the staff were made aware of how to recognise and report any abuse should this happens. It was suggested to the provider that she firms up the verbal training for recognising abuse with some written guidelines. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,24,25 26. Cameo House offered a comfortable environment within a homely setting. Externally at the front there would be no opportunity for residents to sit. Had the internal audit of the home been current hazard would have been noted. EVIDENCE: The home is located at the end of a cul-de-sac, well maintained with exceptionally high standards of hygiene. Accommodation was comfortable, residents bedrooms were personalised to suit individuals. At the rear of the home is a garden, the inspector I has never seen this area used by residents. Bathrooms are located on each floor the ground floor bathroom had a broken panel with a ragged edge, which could be hazardous to residents and the staffs legs. This report makes it a requirement to address the concerns. Each seat was loose and required attention this was pointed out to the provider. An audit of the homes environment would have identified this prior to the inspection. Staff had received infection control training. The provider had plans to lift the environment of the home by redecoration of the communal areas and main hallway. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 ,28,29,30 The home had in place proper procedures for the recruitment of staff. He levels of staff were deemed adequate for the number and dependency of the residents, and appropriate staff training was demonstrated by the way in which the care of the residents was addressed. EVIDENCE: The provider would be available over the course of a day. The deputy was on duty and had the responsibility and autonomy in the provider’s absence. On duty the day of the inspection were two staff attending to residents supported by the cook. Each morning there would be three staff on, reducing to two for the afternoon. One waking night staff with the provider on the campus. There were no staff vacancies at the time of this inspection. Recruitment was via the local outlets. The provider had complied with the Standards and only employed after all the required checks had been received. This was evidenced from the records provided. Mandatory training was current; three of the staff were undertaking Level II NVQ in Care. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,35,36,38, The provider ensured that the residents were safe and secure. The home had failed to carry out an audit, which would have highlighted any defects within or external to the home. Stakeholders’ comments provided constructive information in respect of the care provided. The home had good systems in place to protect the residents finances, and was well organised in maintaining systems and the environment. EVIDENCE: The home was operated to benefit the residents in a relaxed style that was followed by the staff. The deputy leads by example and was part of the working team. Staff confirmed that they had supervision of their working practices, and training needs. The home had received very positive comments from stakeholders to the home. Records evidenced that the home maintained the required records for tests and training in the event of fire. The home had the required servicing of equipment completed; this was evidenced from the records provided. Two of the funds held on behalf of the residents were checked and found to be accurate.
