Conditions of registration: 1. Only persons over the age of 65 years may be admitted. Date of last inspection Brief Description of the Service: Camowen is a Care Home registered to provide care and accommodation to twenty service users in the category OP, Old age not falling within any other category. The registered premises is a converted property situated in the village of Tarring. Accommodation is provided on two floors in single occupancy rooms, nineteen of which provide en suite facilities. Communal space for service users is provided in two small lounge areas, a dining room and conservatory lounge. Ms. Amershi was registered as the provider in respect of Camowen in January 2005 and the registered manager is Mrs. L Little. This is an overview of what the inspector found during the inspection. This inspection is the first since Ms. Amershi was registered as the provider in respect of Camowen in January 2005; it was an announced inspection and was conducted between 9am and 4pm on Thursday 7th July 2005. Prior to the inspection one resident and eight relative/visitor comment cards were received and during the inspection the inspector met and spoke with eight residents individually and had lunch with two other residents. Findings and comments from surveys and residents have been included in this report. The registered provider accompanied the inspector on a tour of the premises and discussed changes that had been made since the registration in January. The inspector also met and spoke with four staff individually. What the service does well: What has improved since the last inspection?

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 & 5 Residents’ needs are assessed before they come to live at Camowen. Prospective residents and their representatives are invited to look around the home. EVIDENCE: Records showed that assessments had been provided by social services and/or the home had conducted their own assessment of need in relation to two newer residents. The inspector met and spoke with two newer residents who said that they and/or their family had visited the home before they came to live at Camowen. One resident also said that she had been made welcome and comfortable since arriving at the home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 & 10 Care records set out residents needs. Health needs are met with the support of health professionals. Residents self-administer when assessed as safe to do so. Medication is managed safely by the home. Residents’ privacy and dignity is respected. EVIDENCE: Care records are comprehensive and information about residents’ needs and abilities is kept up to date. Care records include risk assessments with clear actions being taken to minimise risks to residents’ health, safety and wellbeing, including falls, pressure areas and self-administration of medicines. A district nurse visited two residents on the day of the inspection. One resident told the inspector that a district nurse visits daily to monitor a particular aspect of her health, another said that she had recently seen her GP regarding a medication review.

The inspector observed staff knocking on doors to resident’s rooms and was told by residents that their choice to stay in their room and their privacy is respected. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13, 14 & 15 Residents maintain contact with family and friends. Residents are able to make choices in their daily life. A good choice and standard of food is provided. EVIDENCE: All of the relatives and visitors who completed a survey said they were welcomed into the home at anytime and that they could visit their friend or relative in private. Residents said that their visitors were welcomed into the home and one said that they use the small lounge during their visit. Residents said that their choices about how they spend their time are respected. One resident told the inspector of trips out of the home he takes independently and another said she goes out with relatives. One resident told the inspector that staff had encouraged and supported her choice to return to bed after breakfast and the support she needed was provided later in the morning. Feedback from residents about the choice and standard of food was good and the inspector noted positive comments about the vegetables and some different meals, which have recently been offered.

The timing of supper was an issue raised by two residents in a recent quality assurance exercise. The registered persons said that snacks are offered during the evening to ensure the time between supper and breakfast does not exceed 12 hours, as good practise would recommend. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 & 18 The complaints procedure is displayed and people know who to speak to about any dissatisfaction. Staff are aware of their responsibilities for reporting to protect residents from abuse. EVIDENCE: Two of the eight relatives who completed a comment card said that they were not aware of the home’s complaints procedure. The complaints procedure is posted in the main entrance hallway of the home but the registered persons may wish to consider positioning it more conspicuously. The resident survey indicated that the respondent does know who to speak to if they are unhappy with the care and feedback indicates that the registered manager and staff encourage good relationships within which concerns can be raised if needed. Staff who spoke with the inspector were aware of their responsibility to report any suspicion or allegation of abuse even if they did not recognise the term whistle blowing. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 24, 25 & 26 The internal and external environment is safe, well maintained and suitable for the needs of residents. Sufficient toilet and washing facilities are provided. Residents are facilitated to personalise their own rooms and a programme of redecoration and refurbishment is in operation. The home is safe and hygienic. EVIDENCE: The registered provider has sought reports from the Fire and Environmental Health authorities and an occupational therapist. The registered provider has also taken action to address the recommendations made in these reports to ensure the indoor and outdoor environment is safe and suitable to the needs of the residents. Camowen H60-H11 S62981 Camowen V227268 070705 Stage 4.doc Version 1.30 Page 15 The home has been subject to significant redecoration and refurbishment since January 2005 and the atmosphere is homely and welcoming. Plans to continue these works include replacement dining room chairs purchased from a specialist provider. A comment included on a relative/visitor comment card was “very beautiful environment-very positive that pets are in the home”. En suite toilet and wash hand basins are provided in nineteen of the twenty rooms. An assisted shower facility is currently being installed so the home meets the standard in relation to assisted bathing/showering facilities. Residents told the inspector that they had been able to bring items of their own belongings into the home. Locks have now been fitted to residents’ private accommodation to ensure privacy; one resident said she has a key to both her door and lockable drawer. Some armchairs in individual bedrooms need replacing and new furniture should continue to include a lockable facility. Most radiators have now been covered and window restrictors are in place to ensure the health and safety of residents. The home employs cleaning staff to maintain the cleanliness of the home and a new washing machine has been installed, which meets the required disinfection standards. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29 & 30 Staffing levels reflect the needs of the residents. The recruitment procedure could be improved to ensure the protection of residents. Staff are trained to do their work. EVIDENCE: All relatives and visitors who completed a comment cards said that in their opinion the home was always sufficiently staffed. The rotas provided show that the morning shift is covered by two staff from 8.30am – 4.30pm with an additional carer between 9am and 12noon. Two staff cover the afternoon shift and two staff cover the night duty, one of whom is waking and one of whom sleeps between midnight and 6am, the waking hours have recently been increased. The registered manager works full time in addition to these figures and the home’s deputy managers have days allocated for providing care and office tasks. Records of recruitment were seen in relation to two new care staff. These did not include a CRB and PoVA disclosure obtained by the home before the member of staff commenced employment. Gaps in employment should also be explored. Since the inspection, action has been taken to ensure that the home recruitment process is robust and protects residents. One of the newer members of staff had completed the TOPSS induction workbook. The home has a yearly plan of training updates in core training

The home is working toward achieving the target of 50 of staff trained to NVQ Level 2, currently achieving 44 and staff said they are being supported to enrol on NVQ training and that some specialist training in dementia is also planned. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32, 33, 36 & 38 Residents are asked for feedback and staff are supported by the management structure. The health, safety and welfare of service users and staff are promoted and protected. EVIDENCE: Residents were enthusiastic about changes that have occurred in the home recently. Staff said they felt involved in the process of change, listened to and supported, staff also said that they receive regular supervision. The home has a staff member identified to liaise with residents for quality assurance, including completing satisfaction questionnaires with residents regularly. The registered persons should collate the results of these and use them to prepare an action plan for developing the home in the best interests of the residents.

Accident records are linked to individual plans of care to address any issues and prevent accidents reoccurring where possible.
