Conditions of registration: Date of last inspection 11th January 2006 Brief Description of the Service: Castle View is a Care Home registered with the Commission for Social Care Inspection to provide a residential service for up to 4 older people. It is situated in a rural area on the Shrewsbury to Newport road close to the village of Uffington. The home is owned and managed by Mr & Mrs MacDonald. All accommodation is on the ground floor with access to well maintained gardens. Car parking facilities are available. The home provides a peaceful country setting for those residents choosing a quiet and leisurely lifestyle. The home makes their services known to prospective service users in The Statement of Purpose and Service User Guide. The inspection report is mentioned in this document and is given out on request. Fees are reviewed annually and are £280. The only additional charges to service users are for toiletries, hairdressing and newspapers. This is clearly laid out in the terms and conditions. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has a satisfactory and functional admissions procedure providing an effective needs assessment and suitability evaluation for both privately funded residents and those placed by the local authority. EVIDENCE: The home has an up to date Statement of Purpose and a Service Users Guide, which includes all the required information for prospective residents. Documentation examined indicated that individuals have a full and comprehensive assessment of their needs prior to admission, which is undertaken including the family and health care professionals and is reviewed and amended as requirements change. Castle View DS0000020636.V297095.R01.S.doc Version 5.2 Page 8 Significant time and effort is spent making each admission to the home personal and effective and family members and the prospective service user may visit the home as often as they wish until a decision is made. Observations, inspection of records and discussions indicated that the home meets the individual needs of the elderly people living at the home in a professional and sensitive manner. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The health and personal needs of service users are well met with evidence of regular review and of multi disciplinary working taking place on a regular basis. EVIDENCE: It was evident during the inspection from looking at records, inspecting the facilities and chatting the proprietors and residents that individual health, personal and social care needs were being well met. All service users have a plan of care, these are reviewed and updated at regular intervals. The proprietors promote the service users psychological health to ensure that they are happy and content and they promote their rights as individuals. At the time of this inspection the recording, storage and administration of medication appeared satisfactory.

Those residents spoken to were extremely complimentary regarding the quality of their lives at the home and visiting health professionals continue to praise the management and care standards there. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): All Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The routines of daily living at Castle View are very flexible and each resident finds the lifestyle experienced in the home meets their individual needs. Individual activities take place, there is an open visiting policy and the menu offers a good choice of well balanced and wholesome meals. EVIDENCE: Individual needs, likes and dislikes are clearly identified. There was much evidence to indicate that individual rights are fully promoted and residents are certainly enabled to exercise choice and control over their own lives as far as they are able, personalise their own rooms and use them as private places. Activities, within the home and outside, take place to suit individual needs and tastes and these are fully discussed and agreed with the residents. Visitors are always made welcome, are included in events and are given all the necessary information on aspects of the home and the welfare of the residents. Visitors spoken to have always been complimentary regarding the quality of life for the residents at the home.

The proprietors have undertaken relevant training in Basic Food Hygiene. At the time of the most recent Environmental Health Officers inspection matters were satisfactory. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 17. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has a satisfactory complaints system and there is evidence that residents feel that their views are listened to and acted upon. The arrangements for the protection of residents from abuse are documented. EVIDENCE: The home has a complaints procedure, which is referred to for information in the Service User Guide. There is a system of recording complaints and it was noted that there were no entries. There have been no complaints directed to the Commission for Social Care Inspection since the last inspection. The home has copy of the Shropshire County Council Multi Agency Adult Protection Procedure and the proprietors are fully aware of residents legal rights. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 24 and 26. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home continues to provide a good quality, comfortable and safe environment for those in residence. EVIDENCE: The location and layout of the home are suitable for elderly residents. The communal room is well equipped, warm and homely. Bedrooms are personalised and suit individual needs and the gardens and grounds are very attractive, well maintained and accessible to residents and their visitors. At the time of the most recent Fire Officer and Environmental Health Officer’s inspections matters were reported to be satisfactory and any recommendations made have been complied with.

The standard of hygiene and cleanliness at the time of this inspection was excellent and laundry facilities appeared satisfactory. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): x These standards are not applicable as no care staff are employed. EVIDENCE: There are no care staff other than the registered providers at Castle View. The home employs one part time domestic assistant (cleaner) for eight hours per week and satisfactory checks have been undertaken. The home’s position on recruitment remains unchanged in that there is a procedure in place, which will only be used, should the domestic assistant leave. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 and 38. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The Proprietors approach creates an open and positive atmosphere from which the residents benefit. The home regularly reviews all aspects of its performance through a programme of self review, questionnaires and consultations and meets the requirements of the Fire Officer and Environmental Health Officer, promoting the health, safety and welfare of the residents. EVIDENCE: The proprietors have successfully managed the home for many years. Mr MacDonald is a Registered Nurse and has had experience in management. Records required by legislation to be kept in the home are continually being

The Fire Officer and the Environmental Health Officer reported matters to be satisfactory at the time of their last inspections and recommendations made have been complied with. Insurance cover is up to date. Both Proprietors are trained in first aid and an accident book is available. Records of checks to electrical systems and equipment and Fire Precaution are available. Risk assessments and a health and safety policy are in place and toxic substances are stored in a locked cupboard. No potential hazards were identified.
