Conditions of registration: None Date of last inspection 25/01/05 Brief Description of the Service: Charlotte House is a care home providing nursing care to sixty elderly mentally ill people over the age of sixty. It is also registered to provide care to ten people over the age of fifty. The home has fifty two single rooms and four double rooms, these are located on the ground and first floors. All rooms have en suite facilities. There are four units two on each floor. The home is located next to West Middlesex University Hospital and is close to Hounslow High Street. There is a large secure garden to the rear of the building that can be accessed from the ground floor. This is an overview of what the inspector found during the inspection. This inspection took place over four hours in the afternoon as part of the annual inspection process. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 The home carries out satisfactory assessments of service users prior to admission to ensure the home is able to meet their needs. EVIDENCE: The home carries out a pre-admission assessment and a more detailed assessment once a service user is admitted to the home. The information contained in the assessment documentation inspected covered all areas of need for each service user. The home had also obtained any Care Management assessments that had been completed. The homes assessment and the Care Management assessments had been used to develop care plans for the service users. All assessments had been carried out by a qualified member of staff. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 and 10 Assessed needs had been incorporated into individual plans of care although more attention needs to be paid to the social and leisure needs of service users. Health care needs had been assessed and were being met. The systems for administration of medication were satisfactory. Service users were being treated respectfully. EVIDENCE: A comprehensive, detailed care planning system is in place in the home and the majority of the care plans viewed were comprehensive documents that covered all areas of need. However, it was noted that very little attention had been paid to the social and leisure needs of service users and in some cases nothing at all was recorded. It was also noted that not all care plans were being regularly reviewed. Daily records are maintained for each service user and each service user has a named nurse and key worker. Health care needs were outlined in the records examined and were incorporated into the care plans. Any health care appointments attended or treatment received had been recorded clearly. Appropriate assessments are carried out to ascertain service users needs including assessments on nutrition, continence, moving and handling and risk of pressure sores.

Medication was appropriately stored and the medication administration record sheets were up to date and in order. There were no controlled drugs being used. The home has satisfactory medication policies and procedures in place. Staff were observed treating service users respectfully and the service users spoken to all said that the staff were ‘kind’ and ‘helpful’. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 15 The social and leisure needs of service users are met and service users are encouraged to maintain contact with family and friends. The dietary needs of service users are well catered for. EVIDENCE: The Activities Co-ordinators in the home arrange a wide range of activities to meet the individual and group needs of service users. The activities programme is displayed in service users bedrooms and includes activities such as gentle exercise, memory cards, nostalgia, games and music. Group trips out are sometimes arranged and entertainers also visit the home. As discussed earlier it is important individual social and leisure needs are recorded in the care plans for all service users. Visitors are welcomed at the home, service users can see their visitors in private either in their rooms or in the small lounge areas on both floors. There are also areas in the garden where service users can meet with their visitors depending on the weather. Relatives were observed visiting service users during the inspection. The lunchtime meal being served on the day of the inspection was Chicken Korma or Ravioli and Butterscotch Sponge with Custard for pudding. This was not the meal recorded on the menu for that day. The Cook explained that this was because the home had started using a new supplier who had not provided

Specialist diets are catered for as required. The service users spoken to said that they enjoyed the food in the home. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 The home has a satisfactory complaints system with information available to service users, staff and visitors to the home. The home has adequate systems in place for the protection of service users from abuse. EVIDENCE: The home has an appropriate complaints procedure in place that is displayed in the home. Information regarding making a complaint is also provided to all service users and/or their representatives. The complaint record was examined and was satisfactory. The home has satisfactory procedures in place for the protection of service users and clear guidelines for staff regarding the action they should take if they witness or suspect abuse is taking place or if an allegation is made. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 24 and 26 The home was generally clean, safe and comfortable for the people living there. EVIDENCE: The home is very well maintained both internally and externally. There is an ongoing programme of redecoration throughout the home that is undertaken by the handyman. The home maintains a record of maintenance undertaken, however, it was noted during the inspection that action needed to be taken to complete some minor repairs in service users bedrooms. One bedroom had a broken drawer and in another room the bottom of the radiator needed repairing. There is ample communal space in the home for service users. Each unit has a main lounge area and a small room that service users can see visitors in. Each floor has a dining room. There is a pleasant garden area that is well used during the summer months.

It was evident that service users had been encouraged to personalise their rooms. Screens were available in the double rooms and service users have a lockable drawer in their bedrooms for items they want to lock away. The home was clean and tidy on the day of the inspection. The laundry was satisfactory and appropriate infection control policies and procedures are in place and appropriately displayed throughout the home. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 and 30 The numbers and skill mix of the staff team was adequate and the recruitment process was satisfactory. Staff training provided was adequate EVIDENCE: The home has a relatively stable staff team. The acting manager said that the home had five staff vacancies, four for carers and one vacancy for a qualified nurse and that there were bank staff available to cover any staff absences to avoid the use of agency staff. The staffing rotas viewed showed satisfactory numbers of staff on duty and the numbers on duty during the inspection were satisfactory to meet the needs of service users. The staff records viewed showed that robust recruitment procedures were in place and all the necessary recruitment checks had taken place. This has improved since the last inspection. The home has a training programme in place and records are maintained so that attendance at training sessions can be monitored. Each member of staff has an individual training record that is kept in the staff office on each floor. These records were not always up to date. Staff had attended recent training sessions on fire safety, dementia and the protection of vulnerable adults. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 and 38 Satisfactory management arrangements were in place for the home. The home has appropriate systems in place to ensure the health, safety and welfare of service users and staff are promoted and protected, however these are not always adhered to. EVIDENCE: Since the last inspection the Registered Manager has left and one of the senior nurses who has been at the home for several years is now Acting Manager. The Acting Manager said that she had previously deputised in the absence of the Registered Manager and felt well supported in her new role. Recruitment for a permanent manager was taking place. The health and safety and maintenance and servicing records for the home were viewed. Generally these were in order, however, some information was not up to date. The Landlord’s Gas Safety Certificate was two months out of date. When this was highlighted to the administrative worker for the home an appointment was made for a gas safety inspection to take place the day after

The records relating to fire safety were also examined. These were not satisfactory. There was no current fire risk assessment in place for the home and the records regarding fire drills could not be found. These records have since been forwarded to the CSCI but they are not very clear and the fire drill evaluation record had not been completed for all fire drills. The fire alarm call points and emergency lighting were both being checked on a regular basis to ensure they are in good working order. The accident records for the home were satisfactory.
