Conditions of registration: Date of last inspection N/A Brief Description of the Service: Cherry Tree House is a registered care home for up to three older people. The home is situated in a quiet residential area, with easy access to shops, medical centre and public transport links to larger local shopping areas. Accommodation at the home is provided over two floors, with individual bedrooms, laundry area, lounge and kitchen/ dining area. This is an overview of what the inspector found during the inspection. This announced inspection took place over one day. The home was registered with the CSCI earlier this year and to date no residents have moved in. Therefore the findings of this inspection are based on conversations with the owner, manager and viewing paperwork. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3 and 5. Standard 6 is not applicable to this service. The service has good written information that enables potential residents to make a choice about whether they would like to move to the home. The plan for residents moving to the home is thorough and individualised to ensure that the home can adequately meet their needs. EVIDENCE: The home has produced a Service Users Guide and Statement of Purpose that both contain information about the services provided by the home and all relevant policies. There is a clear policy stating the criteria for admission and the needs of residents that can be met by the home. The manager and owner stated that all potential residents will be jointly assessed by them and with the full involvement of the social worker and relatives where appropriate. The process of moving to the home will be based on the preferences and needs of the resident including day visits, overnight stays, leading up to fully moving in. There will then be a review after one month with the resident, their social worker and all relevant parties to establish if they wish to remain at the home,

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 9 and 11 The care planning processes should enable each resident to be fully involved in identifying their needs and planning their care with the home. The home has adequate policies in place for the safe administration of medication. EVIDENCE: The home has a care plan format in place in which to record the care required for each resident. The manager and owner discussed that once a resident moves to the home an initial care plan will be put in place and that this will be reviewed after the first month once the resident decides to stay. This will then be developed to incorporate all the care and support needed by the resident whilst they are at the home. The owner and manager stated that the resident will be fully involved in this process and that the care provided will be individualised to each person’s needs. The care plan must also include each resident’s wishes in relation to the possibility of their becoming terminally ill and what they would like to happen in the event of their death. A policy must also be developed by the home for these instances. The owner and manager both stated that should a residents needs change they would review the care required and the resident could stay

However, they did emphasise that this would be dependent on the needs of the resident and that external support would be obtained until the home could no longer meet their needs. There are a number of policies relating to the safe administration of medication in place at the home. The owner and manager stated that if residents coming to the home manage their own medication they would encourage them to continue with this, and that they would carry out a risk assessment to ensure that the resident can manage this without their support. Each bedroom also contains a lockable drawer where the medication can be stored safely. The Registered Persons must ensure that all staff working at the home receive training in the safe administration of medicines. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13 and 15 The home aims to promote the individual needs and interests of the residents, providing support where necessary. Residents will be encouraged to maintain contact with family and friends as they wish. EVIDENCE: The owner and manager stated that activities for residents will be tailored around their individual needs, interests and hobbies. They discussed a number of community resources and publications available that provide information on local events and clubs that residents will be supported to attend if they wish. They further described that a priority would be to orientate the resident to the local area so that they are familiar with the community facilities and accessing public transport. Through accompanying residents with this the owner and manager discussed that they would be able to assess any risks that could occur as a result of residents going out and about, where they would provide support as necessary. The home is seeking to promote a homely and relaxed approach, whereby the routines eg. times for getting up and meals, would be flexible and based upon the wishes of the residents. The owner and manager also stated that residents would be involved in shopping and food preparation if they wish.

The home also has the use of a telephone that residents will be able to take into their room and use in private if desired. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 The home has a satisfactory complaints procedure. Some improvements are needed to ensure that residents living at the home will be adequately protected. EVIDENCE: The home has a complaints procedure that contains all the relevant information, and a copy of this is included in both the Service Users Guide and Statement of Purpose. The home has developed in-house policies in relation to Protection of Vulnerable Adults (POVA). Further work is required in this area by the home obtaining a copy of the local authority POVA procedures, and also ensuring that all staff working at the home undertake POVA training. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22 and 23 The standard of the décor within the home is of a high standard and presents as an attractive, homely and comfortable environment for service users. A further improvement is needed to ensure the safety of residents using the upstairs rooms. EVIDENCE: Cherry Tree House is a lovely bright house, airy and spacious throughout. On entering the home there is a hallway that is tastefully decorated, warm and inviting. A tour of the building shows that the high standard of décor continues throughout, with individually decorated bedrooms, a well-furnished comfortable lounge and modern kitchen/dining area. There are no en-suite bedrooms but there are two bathrooms for the use of residents. The upstairs bathroom has a bath with a seat and handles to assist residents with getting in and out. The downstairs shower room also contains a seat for the use of residents, and there is a toilet on both floors.

Improvements are needed to ensure the safety of residents by the provision of window restrictors on all windows of the upstairs rooms. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 and 29 The staffing at the home is adequate, and must be reviewed once residents are proposed to move to the home. The home must ensure that appropriate checks are carried out on staff prior to employment. EVIDENCE: At present there is only the owner and manager employed at the home. They described that they plan to employ more staff as people move to the home and also depending upon the needs of the residents. The home does have a proposed rota that ensures that there will be a member of staff on duty, and a minimum of one member of staff in the home at all times. The owner and manager stated that if residents wanted to, they can assist with interviewing new staff, so that they are involved in choosing who helps with their care. When recruiting staff the home must ensure that appropriate checks are carried out on all staff prior to employment at the home, and that staff records contain all the information as required in Schedule 2 and Schedule 4 of the Care Homes Regulations 2001. The owner and manager stated that all new staff would receive an induction that will include learning about health and safety, what to do in an emergency, the home policies and procedures, record keeping, medication, etc. They further stated that throughout the first six weeks of their induction new staff

Following a successful induction the home will then aim to put new staff onto the NVQ Level 2 Care course to ensure that they have appropriate training for their role. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33 and 38 The owner and manager have a clear understanding of the service and areas for improvement. Improvements need to be made by the implementation of health and safety checks to maintain the safety of the residents. EVIDENCE: The owner has a number of years experience in managing and providing a care service to older people, having previously owned and managed a care home. The manager has previously worked in different care settings and stated that she is currently undertaking the NVQ Level 4 in Management to enhance her skills. The owner and manager both demonstrated a good awareness of the areas of development needed for the service and working within the legislation in relation to running a care home. Both of them convey a very warm and

The home has devised a quality assurance questionnaire for residents to complete after the initial first month, then every six months during their stay. The questionnaire asks for comments on the home environment, care received, food, facilities, etc. The owner and manager stated that the feedback will be used to improve the service and address any concerns that may arise. The owner and manager need to ensure that systems are in place to ensure safe working practices at the home. These include ensuring that all staff employed have undertaken training in first aid, lifting and handling, fire safety, food hygiene and infection control. Routine checks also need to be commenced on the fridge and freezer temperature, hot water temperature, electrical installation, heating systems, etc, to ensure the safety of those living at the home. A record and certificates (where appropriate) must be maintained of all health and safety checks carried out at the home.
