The inspector found no outstanding requirements from the previous inspection report, but made 2 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The home has a pleasant and calm atmosphere. Residents spoken with said that they are well looked after and that staff are kind. "Staff are good and look after me." "I have no complaints" were comments made. The manager promotes Equality and Diversity through staff recruitment, training and staff meetings. Residents were enjoying their meal and those residents who needed help were assisted in a dignified manner. Staff communicated with the residents in a pleasant, unrushed and respectful manner. The home provides a range of activities that interest and provide leisure to the residents. The manager takes residents` complaints seriously and acts upon them without delay. Residents said they knew how to make a complaint and felt confident that it would be dealt with. Staffing levels in respect of care are adequate for the number and dependency of the residents. Staff spoken with said that Chestfield House is a good place to work. They feel supported and are provided with many training opportunities. The great majority of staff have an NVQ qualification or are in the process of achieving this. Staff are provided with safeguarding vulnerable adults training. Mandatory training includes infection control, moving and handling, food hygiene, fire safety and First Aid. Further training is provided in palliative and dementia care. The manager is a moving and handling trainer. A recently employed member of staff said that she was enjoying her induction training. The registered manager keeps the CSCI informed of any reportable event. This ensures a good dialogue between the home and the inspector. The Group manager reported that the providers have invested considerably in the home to improve the environment. This includes fire doors, new carpets and profile beds for more dependent residents.

What the care home could do better:

The registered person must ensure that all parts of the home are kept clean and well maintained. This is a requirement of the report. All staff must be provided with Infection control training. This is a requirement of the report. The recording of complaint investigations could be improved by noting the action taken to prevent similar situations from reoccurring. It is recommended that a record be kept confirming that the complainant is satisfied with the outcome of the investigation. Monitoring systems should be in place covering all areas of the home to ensure that the home is clean, hygienically maintained and in good decorative order. A development plan should be in place as part of the quality assurance system to ensure that the home is run in residents` best interests. The home would benefit from a staffing review in respect of domestic staff on duty. A formal cleaning schedule should be introduced. The home should ensure that continence aids are discreetly stored and that residentsare provided with adequate storage space for their belongings.

The home can easily be reached via the M2 motorway, and the Thanet Way. It is part of the Unique Help Group of nursing homes and the group have another 4 nursing homes in the area. The house has been extensively renovated and provides accommodation for up to 31 Care Homes for Older People Page 2 of 9 residents. Accommodation is situated on two floors and comprises 23 single and 4 shared bedrooms, 14 of which have en-suite facilities. There is a passenger lift, which provides easy access to all rooms on the first floor. There are communal areas on both floors and gardens at the rear of the property. Car parking is available for several vehicles. The current fees for the service at the time of the visit range from £318.12 to £650.00 (a shared room used for single occupancy) per week. Throughout the inspection, assistance was provided by the registered manager in a helpful and professional manner. The Group manager was at the home to carry out a Regulation 26 visit and was present at the feedback. The purpose of the random inspection was to examine those standards relating to cleanliness, infection control, maintenance and dignity. These issues were recently reported to the CSCI as a complaint. The registered manager was not aware that the complaint had been made. An accompanied tour of the home was made and some of residents bedrooms, ensuite bathrooms, toilets, bathrooms and lounges visited. The tour identified several areas where the standard of cleanliness was not adequate. It further identified that a room was chilly, an ill fitting window, a rusty commode, a cluttered sluice, a broken foot operated bin and a poor view for some residents due to discarded items outside the home. A domestic member of staff was on duty from 8-14.00. The laundry assistant was off sick that day. The home currently does not operate a formal cleaning schedule and no recorded audits are carried out in respect of cleanliness. During the tour six residents were spoken with. They expressed their satisfaction with the service. A lunch time meal was observed. The registered nurse, care staff, chef, kitchen assistant and domestic on duty were spoken with. Records were examined in relation to staff training, staff rota, complaints and quality assurance. An Annual Service review (ASR) was carried out in May 2008. Since then the CSCI has received one complaint. The registered manager was made aware of this and carried out an investigation. Occupation at the time of the inspection comprised 26 residents. The registered manager has been working at the home for many years. She is supported by a deputy manager, Group manager and senior staff. In respect of quality assurance a satisfaction survey was carried out in the beginning of the year. Regulation 26 visits are regularly carried out by the Group manager.
