This inspection was carried out on 18th October 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

This is a caring and stately environment for those people who live there. The building is well decorated and furnished creating a bright and airy atmosphere. The staff were seen to be caring and well motivated with the service users all speaking very highly of them. They were also particularly complimentary about the food.

What has improved since the last inspection?

Since the last inspection staff training has progressed and the number of care staff appropriately qualified now exceeds the requirements of the National Minimum Standards. Support staff are also being asked to train to NVQ level 2. On the day of the inspection a new carpet was being fitted in the Great Hall.

What the care home could do better:

No requirements have been made as a result of this inspection and the proprietors continue to review the service with a view to improving it.

The home may accommodate a maximum of 48 service users, of whom up to 48 may be Older Persons, 21 may be persons with a Physical Disability, and 1 may be suffering from Dementia. The home must comply with the Staffing Notice as issued by the Shropshire Area Health Authority dated 18th August 1994. 2nd February 2005 2. Date of last inspection Brief Description of the Service: Cheswardine Hall was built about 1880 in the style of a large country house, set in beautiful private grounds with a small lake. The Home provides residential and nursing care for a maximum of 48 older people, although on the day of the Inspection there were 29 Service Users in residence. It is owned and managed by Stephen and Anne Poole, both of whom take an active part in the daily operation of the Home. The Home’s furniture, décor, fixtures and fittings reflect the character and opulence of the house. This inspection was unannounced and took place over three hours during the late morning and early afternoon. Information was gained from documentation made available by the home, by discussions with the proprietors, the staff and the service users as well as by reading documents kept within the home and by looking around the building. Inspection work undertaken by CSCI is proportionate in relation to how a home has performed in the past. As Cheswardine Hall has a history of providing a good service for people this inspection was brief and focused only on a small number of “key” areas of work. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 The home has an appropriate admissions assessment procedure. EVIDENCE: Looking at the records and talking to the staff and the residents showed that the home has a preadmission assessment process that ascertains the needs of the individual and that this process is followed. The home does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 and 9 The personal and healthcare needs of the service users are met. Medication is stored and managed appropriately. EVIDENCE: Reading the service users records showed that the assessments enabled the home to create comprehensive care plans. These contained guidance for the staff on the range of assistance that each service user needs for both personal and health care. Talking to a number of the service users established that they were being cared for in the way outlined within the agreed plans. The records also showed that the care plans are reviewed at least monthly. The home also has a system for storing and recording medication that complies with the guidelines issued by the Royal Pharmaceutical Society. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): x None of these standards were fully assessed on this occasion. EVIDENCE: Although none of the standards in this section were fully assessed on this occasion the meal served during this visit looked very appealing and the residents all spoke very highly of it. A number of the residents were also seen receiving a lot of assistance with the meal and the staff helping them were seen doing so in a very sensitive manner. JUDGEMENT – we looked at outcomes for the following standard(s): x None of these standards were fully assessed on this occasion. EVIDENCE: The proprietor stated that there had been no complaints made since the last inspection nor had there been any referrals made in relation to abuse. These standards will be more fully assessed at the next inspection. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 The standard of the environment is good providing service users with an attractive place to live. EVIDENCE: Cheswardine Hall is in the village of Cheswardine, North Shropshire. Set within its own landscaped gardens, on the outskirts of the Village, the home is accessible by car and taxi. The interior of the home is opulent and very well maintained providing a general ambiance that is stylish, tranquil, warm and welcoming. As part of the home’s on-going maintenance programme bedrooms are redecorated and up-graded when vacant. Many of the double rooms are being used as singles for the comfort of residents. Communal areas comprise a library, music room, TV lounge, dining room and a large lounge, which is known as ‘the great hall’. The home also has a laundry that is well equipped and has staff that are especially trained in its use as part of their duties. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30. The home ensures that enough staff are available to carry out the service user plans. The home has good staff recruitment and training practices EVIDENCE: Staff records contained appropriately completed application forms, two references were seen on all files and CRB disclosures had been obtained prior to staff starting work. Records showed that they had received an induction that met the standard required by Skills for Care. The records also showed that eleven of the nineteen care and nursing staff had achieved NVQ2 in care or above and that housekeeping and estate staff are also working towards NVQ2 in Housekeeping as part of the homes programme of providing appropriate staffing to support the residents in the home. The staff duty rota showed that appropriate numbers of staff at each skill level were available on each shift. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 The home is managed by its experienced and well qualified proprietors. EVIDENCE: Mrs Poole is a qualified RGN and has completed her Registered Managers Award. Mr and Mrs Poole jointly conduct the administrative duties necessary for the running of the home, business management, the up-keep of the home’s interior and exterior building and decoration requirements, and for ensuring the maintenance of the extensive gardens.
