Conditions of registration: 1. The home may accommodate a maximum of 48 service users, of whom up to 48 may be Older Persons, 21 may be persons with a Physical Disability, and 1 may be suffering from Dementia. The home must comply with the Staffing Notice as issued by the Shropshire Area Health Authority dated 18th August 1994. 18th October 2005 2. Date of last inspection Brief Description of the Service: Cheswardine Hall was built about 1880 in the style of a large country house, set in beautiful private grounds with a small lake. The Home provides residential and nursing care for a maximum of 48 older people, although on the day of the Inspection there were 29 Service Users in residence. It is owned and managed by Stephen and Anne Poole, both of whom take an active part in the daily operation of the Home. The Home’s furniture, décor, fixtures and fittings reflect the character and opulence of the house. This is an overview of what the inspector found during the inspection. This inspection was unannounced and took place over two and a half hours during the morning. Information was gained from documentation made available by the home, by discussions with the proprietors and a number of the residents and by looking around the building. Inspection work undertaken by CSCI is proportionate in relation to how a home has performed in the past. As Cheswardine Hall has a history of providing a good service for people this inspection was brief and focused only on a small number of “key” areas of work. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 The residents are treated with respect and their right to privacy is upheld. EVIDENCE: During the visit a number of the residents spoke in a complimentary manner regarding how they are treated by the staff. Staff were seen talking to and interacting with the residents in a respectful way; an example being knocking on doors before entering rooms. The home has a range of written policies that cover a variety of activities relating to personal care. These stress the importance of maintaining residents’ privacy and dignity. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 The residents are able to take part in a variety of activities should they wish to do so. They are able to maintain contact with their families where practical and they themselves are also able to come and go with help provided as necessary. Meals available to the residents are of good quality. EVIDENCE: At the start of the visit a library van was just leaving the home after one of its visits. Talking to some of the residents confirmed that a variety of activities regularly take place within the home such as historical slide shows, sing-alongs, line dancing demonstrations and ‘chairobics’. The home now has a licenced bar for use by the residents and the proprietors outlined activities that they are considering introducing into this new facility. There are visitors other than families that regularly go to the home such as the hairdresser, local clergymen and, as already mentioned, the mobile library. During this and the last visit the residents were also very complimentary about the meals at the home. The menus showed that they are served a good variety of meals and were seen to be prepared to a good standard. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 The service users are protected from abuse. EVIDENCE: The home was seen to have a complaints procedure in place that contains all of the elements required by the Care Homes Regulations 2001. The proprietor confirmed that they had received no complaints since the last inspection. The proprietor also confirmed that there had been no reason to make any referrals under the local adult protection policies during the last year. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33, 35 and 38 The home is well run and the safety of the residents would be enhanced by the development of risk assessments for the storage and use of dangerous substances. EVIDENCE: The proprietors both spend a lot of time at the home administering it on a day to day basis. It was clear from watching Mrs Poole in particular that she has a detailed knowledge of the needs of the individual residents. She also discussed issues that had been raised at the residents’ meetings that she had organised. The proprietors also assist some of the residents by storing cash securely for them. They keep records that show when and for what the money is paid or received. However, the risk assessments for the storage and use of dangerous substances have not yet been developed.
