Conditions of registration: 1. 2. This home may accommodate 60 older people who require convalescent or general nursing care. This home may accommodate 10 people (aged 45 or above) with physical disability who require nursing care. Date of last inspection Brief Description of the Service: The Chorleywood Beaumont Care Home is registered to admit up to 60 elderly service users who require nursing care. Ten service users over 45 years of age with physical disabilities may be accommodated within the overall capacity of the home. The Chorleywood Beaumont is a restored period house with modern additions. There are Close Care apartments on the ground floor that are not part of the registered home. This service is inspected under the Domiciliary Care Agency Standards. All nursing clients have bedrooms on the first floor, with a range of day rooms and facilities on both floors. There are attractive gardens to the rear of the home and parking facilities to the front and side of the building. The home is situated in an attractive semi-rural location between Rickmansworth and Chorleywood, close to the junction of the M25 motorway. The home is part of the Westminster Health Care Ltd group, which is owned by Barchester Healthcare Homes Ltd. The current fee levels which are based on an individual assessment of need and the type of accommodation booked range from £750 - £1155 per week. This is an overview of what the inspector found during the inspection. This inspection and the inspection was carried out by one inspector over two days. The Domiciliary Care provided by the home to the residents of the close care apartments within the home was also inspected during this time and is detailed in a separate report. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. 6. This judgement has been made using available evidence including a visit to this service. Thorough assessments are carried out prior to a service user being admitted to the home. EVIDENCE: Four service users assessments were inspected and they contained full information on the service users needs and wishes. The pre-admission assessment was documented and kept in the service user’s file. The home will only admit a service user whose needs can be met. A copy of the contract was given to each service user on admission. The manager confirmed that each service user is given a Service User Guide on admission and that information is provided to all prospective service users. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The health and personal care of the service users is met in a manner that promotes respect and the dignity and privacy of the service user. Medication was in order. EVIDENCE: Four care plans were inspected they contained good detail of the service users needs and on how the care should be administered. A tissue viability nurse call to the home one-day per week to ensure the well being and good health of the service users. Wound assessments are carried out regularly and the care plans give detail on how to treat service users wounds. Turning charts and nutrition charts are kept in service user’s rooms. The administration, storage and recording of medicines was found to be in order. The privacy and dignity of the service users was maintained.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. 12,13,14,15. This judgement has been made using available evidence including a visit to this service. The daily life and social activity of the home meets the needs of the service users. The food was very good and served in pleasing surroundings. EVIDENCE: The home has a lively social life. On the day of the inspection service users were out in the morning and a trip was planned to St Albans to an ice show in the afternoon. There was a full programme of activities varying from watching cricket across the road on the green to visits out to the cinema and trips out for afternoon tea. There are photographs of events displayed in the home. The home has close contacts with the local community and there was a steady stream of visitors in the home throughout the inspection. Visitors were spoken with and informed the inspector that they feel welcome in the home. Rooms inspected were personalised and contained items of furniture and decoration that service users had brought into the home. The inspector joined the service users for lunch. This was served in the dining rooms, where attention to details was visible in the preparation of the tables. The food was of good quality and was very tasty. There was a menu choice on the day roast chicken or roast

Service users lunched at a time that suited them and could choose freely from the menu. Drinks are served in the bar prior to lunch. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. 16,18. This judgement has been made using available evidence including a visit to this service. Staff were not aware of Social Service’s Safeguarding Adults procedures. This judgement would have been adequate had the Registered Manager not undertaken to resolve this matter immediately. Complaints were dealt with using the home’s complaints procedure. Generally service users were protected from abuse. EVIDENCE: All complaints written and verbal were treated seriously. They were responded to promptly in accordance with the home’s complaints policy. Staff while very caring were unsure of Hertfordshire Social Service’s Safeguarding Adults Policy. In the event of an incident of abuse this policy must be followed. Staff spoken with showed that abuse would not be tolerated, but need clear guidance on how to deal with an episode of Safeguarding Adults. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. 19,26. This judgement has been made using available evidence including a visit to this service. This judgement would have been adequate had the work to re-decorate areas of the home not started. The environment was safe and well maintained. The home was clean and hygienic. EVIDENCE: The home has started to work on the environment. The dining rooms and the lounges have been re-decorated and plans are in place to ensure the corridors – which are showing signs of wear and tear – are decorated. There is an ongoing programme of all the individual bedrooms rooms being decorated, this is expected to be completed by August 2007. On the day of the inspection the home was clean and odour free. The gardens were very well tended and assessable to all service users. Service user spoken with stated that they use the grounds every day.

This judgement has been made using available evidence including a visit to this service. The home is staffed by well trained caring staff. EVIDENCE: Staff spoken with were aware of service user’s needs. The outcomes for service users was that they were cared for by well trained staff. Recruitment was in order with all files inspected (six) contained all the appropriate security and identity checks. Training records were available and staff on the floor displayed knowledge of the service users needs. There was sufficient staff on duty to meet the needs of the service users. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. This judgement has been made using available evidence including a visit to this service. The home is well managed. The health and safety of the service users and staff is promoted. EVIDENCE: Staff spoken with were aware of service user’s needs. The outcomes for service users was that they were cared for by well trained staff.
