Conditions of registration: Date of last inspection 16th May 2005 Brief Description of the Service: Cornwell House is a care establishment providing personal care and accommodation for twenty older people. The Abbeyfield Ferring Society, a voluntary organisation, owns it. Mrs Lorraine Hall is the registered manager. The two storey detached building is located in a residential area close to local shops and the sea front. There is a level enclosed garden to the rear of the property and further gardens to the front, all of which are accessible to service users. All of the establishment’s bedrooms are for single occupancy and have en-suite facilities. There is a passenger lift. Cornwell House DS0000014472.V253749.R01.S.doc Version 5.0 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This inspection was announced and took place from 9-30am to 2-45pm. Cornwell House has performed well over the past years and at the last inspection no requirements were made. During the course of this inspection the inspector spoke to residents and staff. Records and Care Plans were looked at and a tour of the home was conducted. The Commission had sent out comment cards and a large number were returned to the inspector prior to the inspection. Comments from these cards will be included in the report. One resident told the inspector that, “I couldn’t be better cared for”. Another said, “We are all very special to the staff.” A relative wrote, “This is a wonderful care home with a very friendly, caring staff. Nothing is ever too much trouble - always time for a chat tempered with a lovely sense of humour – a fantastic care home!” All records were found to be in order and up to date. The Care Plans were extremely well written and contained all of the information needed to look after the residents. They were reviewed regularly. Staff training was ongoing and it was noted that the majority of staff were either National Vocational Qualification (NVQ) trained or were on training. The training records of all staff were well documented and up to date. Staff were being supervised by Care Team Leaders and detailed notes had been kept of these sessions. The building was warm, clean and welcoming. On touring the home the inspector noted that two radiators in bathrooms were unguarded. A requirement has been made to ensure that the residents are prevented from burning themselves on these appliances. At the last inspection an issue was raised by the Fire Officer regarding the sitting of waste bins on a fire escape route. This has been dealt with by the home. However, the inspector was concerned to see that part of this escape route was over uneven ground. A requirement has been made to attend to this. There was the manager, three care staff with two domestics, a cook and the administrator all on duty. At night there were two staff awake on duty. The rotas showed this to be the case. All Health and Safety issues were up to date. What the service does well:

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4, 5, 6 Residents and their families have the information they need to make an informed choice about where they live and are suitably assessed before placing. EVIDENCE: Each resident is given a contract when he/she enters the home. Most of the residents had family or friends who were able to act upon their behalf. The manager assesses all residents before they join the home either in their own home or in hospital. Some of the residents have transferred from the Society’s own Sheltered Housing Home elsewhere in the village. They are usually well known and this helps make the transition a little easier. All assessments were seen and found to be very detailed. The inspector was present when a prospective short stay resident phoned to enquire about the availability of a room. The manager explained what her responsibilities were and arranged to visit the prospective resident the next day at her home. The home does not take anyone in for intermediate care.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10, 11 The resident’s health, personal and social care needs are well met through training and supervision of the staff. EVIDENCE: Care Plans were extremely detailed and very well written and presented. The inspector was able to see them easily as they were kept in the residents’ rooms. A daily diary was also kept for each individual and the inspector read what staff had written about each resident. District Nurses visit the home when requested and records are kept of all such visits. The home is currently enjoying the services of a Community Matron who has taken on four residents in an effort to improve their health needs. The manager told the inspector that this was a very positive initiative and plans were in hand to extend this service to other residents. Medication was stored safely and records were well kept in regard to the administering and disposing of them. Staff were well trained with many having been on a local course. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 Residents say that they are happy with their lifestyle, are able to keep in contact with their family and friends and able to have control over their daily tasks. Good home cooked food is provided. EVIDENCE: Three residents told the inspector that they were happy to stay in their own rooms and not get too involved in activities “unless it was something special”. The home does have the services of a Volunteer who runs a weekly shop for them. Here they are able to buy some personal toiletries etc. Everyone seems to enjoy the fortnightly Music for Health session and the weekly skittles session. Holy Communion is offered by a local vicar every six weeks whilst people from other denominations visit on a regular basis. Residents are invited to a quarterly meeting with the manager. Two residents are able to go out on their own whist another attends a local Church Group with assistance from a carer. The home ensures that two residents attend a local club once a week by escorting them and bringing them back if no other transport is available. Cornwell House DS0000014472.V253749.R01.S.doc Version 5.0 Page 12 The majority of residents have their own private phone in their room so that they can contact their family and friends when they want to. Two residents have Sky TV. Family visits were important and the inspector was told that all visitors could help themselves to refreshments that are laid out in the dining room. The inspector noted a large selection of goodies for people to eat and drink. The inspector spoke to two relatives who were visiting. One visitor wrote to the inspector saying, “Always a warm friendly atmosphere here.” Residents sat down to a meal of Salmon and Broccoli Quiche or Steamed Fish or Omelette with new potatoes, courgettes and peas. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17, 18 Any complaint is taken seriously and staff and residents know how to complain. Staff are well aware of the Adult Protection Procedures and a Whistle Blowing Policy is in evidence. EVIDENCE: All staff have attended the Adult Protection Training and the procedures are in the office. There have been no complaints made about the home to the CSCI. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25, 26 The home is well maintained, safe and clean with no nasty smells. EVIDENCE: The home is well kept and is clean and tidy. The inspector observed the cleaners doing their rounds and every room was left clean and tidy. The home has installed a shower for those residents who would like one. The bathrooms have been recently refurbished allowing residents to be transferred to the hoist more easily. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 There is usually sufficient staff employed to ensure that residents’ basic needs are met. The home has a sound recruitment policy and all checks are made. There has been considerable input into staff training. EVIDENCE: All staff have undergone Criminal Records Bureau (CRB) checks and copies that these were satisfactory were kept in the safe. Prospective staff are invited to complete an application form and attend an interview. Staff training had been undertaken in all areas and the evidence showed that all staff were offered training relevant to their work. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 34, 35, 36, 37, 38 It is apparent that the needs of the residents are paramount and that things happen for their benefit. The home has no dealings with any personal finances preferring to leave that to families or other professionals. The home is considered a safe environment. EVIDENCE: Staff told the inspector that they received clear instruction from the manager and that she was very approachable. The home was constantly updating their practices to ensure that residents received the best care that they could. The home does not have any dealings with any money belonging to residents.
