What the care home does well

This is a care home where older people are well looked after. It has a competent staff team who understand the needs of the elderly people living there. The residents who were spoken to said they liked living in the care home. Staff is very caring and considerate and the atmosphere was very homely. One resident said staff is very friendly and helpful. A range of activities is provided for residents to take part in. The inspector watched a group of three residents who were planting bulbs into pots ready for Christmas.

What has improved since the last inspection?

Care plans are now place when a resident is admitted and include information for staff to follow to ensure all their needs are met. They are being reviewed as often as needed to make sure the information is up to date to ensure staff provide the appropriate care to each resident. Care plans covering short-term needs such as chest infections and wound care are also in place so that staff knows what treatment is required.

What the care home could do better:

The inspector has no doubt that, where necessary, complaints are taken seriously and action will be taken to resolve them. However, the registered person is advised that clear records should be kept even when residents or their families raise small concerns about the care provided. This will mean that, should it be necessary, the registered person can provide documentary evidence to demonstrate the action taken to address any such concerns or complaints made.

It provides personal and nursing care. Country Lodge is a detached two storey property which provides accommodation in three double and nineteen single bedrooms located on the ground and first floors. A vertical passenger lift provides access to all floors. A dining room and lounge are located on the ground floor. There is also a small sitting area on the first floor. The property is located on the outskirts of Worthing. The service is privately owned by Mr and Mrs John Wright, whilst the registered manager, who is responsible for the day to day running of the care home, is Mrs Debbie Spokes. The inspection was announced and started at 9am. It took place over five and a half hours. The inspector spoke separately to three of the twenty-one residents who a currently living at the care home. The inspector also spoke and to four of the staff who were on duty. Before the inspection the inspector received additional information from the registered provider with regard to the administration of the care home. The inspector also received comment cards from three residents and four relatives. Some records were also examined. What the service does well: What has improved since the last inspection? The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 9 Where possible residents are involved with the drawing up of their care plans. Otherwise their family or close friends have become involved in this process. Care plans have been developed out of assessments. These care plans provided information to staff to ensure residents’ health, personal and social care needs have been met. Residents who wish to can be responsible for their own medication. The home has drawn up appropriate policies and procedures to ensure the residents are protected. Other key standards were assessed as fully met during the last inspection. EVIDENCE: The inspector read the care plans of four residents who have been admitted since the last inspection. They were informative and included appropriate information for staff to follow. Where necessary, short-term care plans have also been drawn up. They included information that staff should follow in order to meet short-term needs of residents such as chest infections and wounds. The home has developed an appropriate policy and procedure for residents who wish to self medicate. Residents spoken to confirmed that they could look after their own medication if they wish. However, residents told the inspector they have chosen to ask the staff to look after their medication. Residents said that they were not physically capable of handling medication. They also told the inspector they did not want the worry of remembering to take them at the right times. One resident said that the staff does talk to her about her medication. She can ask staff if she thinks she should have something, which she has not been given. “They will also tell me when something has changed”. The inspector saw that the records of the receipt, administration and disposal of medication have been kept up to date. The registered provider has a contract with a clinical waste disposal company who have a licence to dispose of medication. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 The registered person has ensured that complaints are listened to, taken seriously and, when necessary acted upon. The registered person has also taken appropriate action to ensure residents are protected from abuse. EVIDENCE: The inspector looked at the written complaint procedure, which included details of each stage of the process with timescales. It also included information with regard to whom complaints should be directed. The address and telephone number of the Commission was also made available to prospective complainants. This is prominently displayed in the front hallway of the care home. Residents told the inspector that, should it be necessary to make a complaint, they knew who they should speak to. They also told the inspector that they were confident they would be listened to and their concerns taken seriously. One resident told the inspector that, recently it was necessary to speak to the manager about an incident. The resident also told the inspector that this had been dealt with and resolved to their satisfaction. The inspector informed the registered person a comment card from one relative indicated that they were not aware of the complaint procedure. Following discussion the inspector advised the registered provider of the purpose of such a record and what may be included. Residents told the inspector that staff was kind and considerate; residents felt they were treated with respect at all times. “ When you feel mopey, staff will cheer you up!” The inspector spoke to three staff that was on duty. They said that they had received training in understanding and identifying abuse. The training also included what staff should do if they discover an incident where a resident has been abused. Records of training were also examined and confirmed that staff have received training in the following areas: what is abuse; recognising the signs of abuse; the types of abuse; reporting abuse; the law regarding abuse. Records seen showed that staff has completed a short written summary of the training they have received and what they thought about it. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 29 The home’s recruitment procedures are robust and ensure residents are protected from potential abuse. Other key standards were assessed as fully met during the last inspection. EVIDENCE: The inspector looked at the recruitment records of three staff that have been appointed since the last inspection. Records included current criminal record checks at an enhanced level and information which confirmed the identify of the prospective member of staff. Information the registered person supplied to the Commission before the inspection clearly indicated that staff do not start work in the care home until such checks are in place. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 35 The registered person has put in place appropriate systems to ensure residents’ financial interests are safeguarded. Other key standards were assessed as fully met during the last inspection. EVIDENCE: Information the registered person supplied to the Commission before the inspection clearly indicated that the registered person does not act as appointee for any resident. A family member or a solicitor usually undertakes this. Residents spoken to confirmed that a relative deals with their financial affairs. The registered person is able to look after money or valuables deposited by residents for safekeeping. Residents also confirmed that Mr Wright does look after small amounts of money for them and does make purchases on their behalf.
