CSCI found this care home to be providing an Adequate service.

The inspector found no outstanding requirements from the previous inspection report, but made 1 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The home employs staff that are kind and caring. The care documentation is detailed and life histories have been compiled for all of the people using the service. The environment has improved since the last inspection and makes the home a much more pleasant place to live in. The wet room has vastly improved the bathing facilities for people using the service. The home is warm and comfortable with no offensive odours.People`s privacy and dignity are respected. Training is provided for staff to improve their knowledge and skills. Electronic records are maintained and kept up to date. More Registered Mental Nurses have been employed and there is one on duty at all times, which ensures someone skilled in the care of people with dementia or a mental illness is available for people using the service and for staff to consult. A member of staff is responsible for activities and this area has improved since the last inspection. Training has been provided for the manager and the activities co-ordinator. The meals provided are nutritious and there are some choices.

What the care home could do better:

The home`s risk assessment for the use of restraint must be reviewed to specify the risks to individuals and the action to be taken by staff. Care plans for medication and specific conditions like diabetes need to be in place to inform and direct staff on the care to be provided. This was discussed with the manager. The remaining recommendations should be given serious consideration.

Date of last inspection Brief description of the care home Cornwallis Care Services Ltd, own two nursing and one residential care home in Cornwall. Cowbridge Nursing Home is registered to provide accommodation and care to 30 residents who may experience mental health problems or dementia. Cowbridge Nursing Home is a detached property situated on the outskirts of Lostwithiel. The original house has a modern day extension. The grounds are extensive with views over the surrounding countryside. There is an area of garden and a patio that are enclosed and accessible to people using the service. Accommodation is provided on two floors with a very small passenger lift for access. There are rooms for both single and double accommodation - only one room has en-suite facilities. Assisted bathing and shower Care Homes for Older People Page 2 of 9 facilities are provided. All rooms have call bells. There are three sitting rooms and a dining room. Refurbishment is currently taking place. Information about the home is available in the form of a statement of purpose and residents? guide, which can be supplied to enquirers on request. A copy of most recent inspection report is available in the home. Fees range from 478.67 pounds to 600 pounds per week; this information was supplied to the Commission on 28/03/08. Additional charges are made in respect of private healthcare provision, hairdressing and personal items such as newspapers, confectionary and toiletries. Care Homes for Older People Page 3 of 9 What we found: We met with the manager to review the requirements and recommendations set at the last key inspection on 28 March 2008. An improvement plan has been received in respect of the requirements. An expert by experience accompanied the inspector on this inspection to talk with residents and staff about life in the home. This is a record of our findings. Requirement 1 We were shown information provided to people using the service regarding their fees and the nursing contribution was shown separately. This requirement is now met. Requirement 2 The risk assessment document for the use of restraint has not yet been developed fully, the manager assured us that this would be done within one month of this inspection. The restraint policy has been updated. Requirement 3 We were told that the new controlled drugs cupboard had been ordered. Since this inspection we have been informed that it has arrived. This requirement is now met. Requirement 4 The manager stated that there is currently no covert administration of medicines. This requirement is now met. Requirement 5 Medicine charts were examined and there were records of all medicines received included on the charts. This requirement is now met. Requirement 6 The training matrix supplied to us shows all care staff have received abuse training. Three ancillary staff are booked to attend by the end of November 2008. This requirement is now met. Requirement 7 The wet room has been completed and has vastly improved the bathing facilities for people using the service. This requirement is now met. Requirement 8 We toured the building and found that all the major issues in respect of the environment have been dealt with. Further re-decoration has taken place and the doors to bathrooms and toilets have been colour coded to help people identify them. The plastering that was required upstairs has been completed and the adjacent room is now occupied by one person which gives much more space for the occupant and for staff to work in. New pictures have been displayed in the corridors and a second piano has been provided in the dining room. A new boiler has been installed and has improved the heating system. We were told that new carpet has been ordered for the downstairs corridor and lounge. This requirement is met. Requirement 9 More Registered Mental Nurses (RMN) have been employed and there is one on duty at all times, which ensures someone skilled in the care of people with dementia or a mental illness is available for people using the service and for staff to consult. The manager said she has an assessment tool for assessing the competency of nurses to be in charge of the home should the need arise. This requirement is now Care Homes for Older People Page 4 of 9 met. Requirement 10 The electronic training records were supplied to us and show that a great deal of training has been provided since the last inspection and that statutory training is now up to date. This requirement is now met. Recommendation 1 Individual life histories have been compiled for all residents. The manager said that she hopes to improve on the present format. Recommendation 2 There is one person that has insulin injections, her medicine chart was seen and the injection sites are now recorded. Recommendation 3 The door at the bottom of the stairs remains locked, the manager stated this is for the safety of the residents and she feels it is appropriate at this time. A risk assessment has not yet been undertaken to see if an alternative method could be used to allow better access to staff and visitors. Recommendation 4 There has been no sluice provided upstairs Recommendation 5 The manager said she intends to review the homes policies in due course. She has only been employed since May 2008. The expert by experience found that the staff working in the home are very kind and caring and that they were very attentive to peoples needs without being patronising. They knocked on doors and respected residents privacy while she was there. She found that there were no offensive smells and that the home was clean, warm and comfortable. She spoke to staff including the cook who has worked there for many years. She says the menu is set but special needs are catered for. The dinner being served looked well cooked and nutritious. I would think no one goes hungry. She was told that singing plays a big part in residents lives and that someone comes in to encourage them to do various types of movement in the afternoons. People go outside when the weather is fine and have planted some pots of flowers. Knitting and other crafts are done by those who are able.
