We give a quality rating following a full review of the service. Cowbridge Nursing Home is registered to provide accommodation and care to 30 residents who may experience mental health problems or dementia. Cowbridge Nursing Home is a detached property situated on the outskirts of Lostwithiel. The original house has a modern day extension. The grounds are extensive with views over the surrounding countryside. There is an area of garden and a patio that are enclosed and accessible to people using the service. Accommodation is provided on two floors with a very small passenger lift for access. There are rooms for both single and double accommodation - only one room has en-suite facilities. What did we do for this annual service review? We looked at all the information we have received, or requested, since the last key inspection. This included the annual quality assurance assessment (AQAA) that was sent to us by this service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gives us some numerical information about the service. We looked at surveys returned to us by staff, healthcare professionals and people using the service. We have considered things that the manager has told us about what has happened within the service, these are called notifications and are a legal requirement. We have also looked at any other information from external organisations we may have received. The previous key inspection report was taken into consideration as well. What has this told us about the service? This service sent us their annual quality assurance assessment (AQAA) when we requested it. It was clear and concise. We looked at the information in the AQAA and our judgement is that the care home is still providing a good service and that they are making improvements to maintain this. The AQAA told us that the home provides a supportive loving environment in which personalised care of a high standard is given. It provides a welcoming environment to all visitors who are encouraged to be involved in the life of the home. The homes reputation with external agencies has improved over the past year. It told us that thorough assessments of residents mental state are carried out using the skills of the multi-disciplinary team. Person centred care is provided and privacy and dignity are respected at all times. The manager feels there could be more emphasis on cultural and religious beliefs. There is a weekday activities programme and one to one activities take place at the weekends. It is hoped that a more structured programme will be devised to improve the weekend activities and include trips out. There have been no complaints or safeguarding issues reported to the home or the Commission in the past year. There is an improvement programme in place for the refurbishment of the home and improvements to the grounds and a great deal of work has been undertaken. Redecoration is ongoing, new carpets have been provided in some bedrooms and corridors and the lighting in the communal areas has been improved. The kitchen has been redecorated and a substantial amount of new equipment has been provided. A safe outside facility has been provided for the use of residents and their families. The nursing team has been consolidated and their knowledge and commitment has been improved to deliver a high standard of care. 50 of care staff are qualified to at least NVQ level 2 in care and the manager has stated that the other care staff are undertaking NVQ courses. Annual Service Review Page 4 of 7 The induction of new staff has improved by the utilisation of the common induction standards. Dementia training has taken place and further training is planned including end of life care training. It is hoped that a training liaison officer will be employed across the group of homes. Administration systems have improved over the past year and the manager hopes to utilise the Companys administrators more in the future. The manager of the home is not yet registered with the Care Quality Commission but has told us that she has sent her application to us. We have received six surveys from people using the service which were completed with the assistance of relatives or staff and these were positive about the home and the services provided. Three people said the food is always to their preference and three said usually. Three people said there are always activities they can join in with, two said usually and one not applicable. When asked what the home does well the comments included You are made to feel wanted and special and all types of dementia are cared for, Very good home made food, Good interaction between staff, residents and family, Everything is always done extremely well, nothing is too much trouble for the staff, Always welcomes you and acts hospitably, Tries to give you a quieter place within the home so that you can spend time with your relative, where possible and feasible, The staff make Cowbridge and I am grateful to them and Always notifies you of any change in the residents wellbeing and informs you of the doctors diagnosis and medication prescribed. When asked what they could do better comments included Possibly vary the sandwich fillings a little more at tea time, At the present moment in time they have a high number of residents with very little ability to communicate and complex needs so staff are very busy. Maybe they could look at increasing the level of all staff for interaction with individual residents, and All staff use residents names but residents dont use staff names, would it be worth staff wearing name badges with first names? Other comments included The staff are nothing short of miraculous, working long hours, struggling to get in in all weathers, meeting heavy demands of dementia patients in a loving way, doing the best they can. It makes a terrible situation for sufferers and loved ones more bearable, My mother is extremely well looked after and I am happy that she is fortunate that if she has to be in a home that she is at Cowbridge. Well done to all the staff and I have great trust in their professional competence and feel mother is totally safe and cared for and has best quality of care available. We received six surveys from staff which were positive and we spoke to one person at their request. All six surveys told us there are usually enough staff to meet the needs of residents. When asked if the way information about clients is shared works well, two said always and four usually. When asked if they have enough support and knowledge to meet the different needs of residents, two said always three usually and one did not respond. When asked what the home does well the comments included Cares for our residents, A good team, Treats the residents as individuals, Staff ensure the place has a homely atmosphere for the residents, Communication between staff is usually
Annual Service Review Page 5 of 7 very good and helps maintain individualised care for each resident and Staff take time to build up a rapport with service users and their families which helps staff to deliver a person centred approach to care. They also said Putting the needs of residents first, all the staff work hard to achieve this and Each day is full of happy people, we and the residents are very proud of our home. When asked what the home could do better comments included The home is let down by environmental factors, for instance the lift is too small, I think we could improve or do things better by the interior decoration, it lets us down a bit, The home itself needs some tlc decor wise but that is in hand at the moment and It is not easy transporting some residents between floors as the lift is small when wheelchairs are needed and special measures need to be taken to facilitate this. Also, Activities could be better with more 1:1 but there are not enough staff to do this. Other comments included In recent weeks staff have shown their true metal, adverse weather conditions and an isolated position made Cowbridge almost inaccessible but they pulled together and made sure residents did not lose out in any way with their care. Cowbridge has flourished in the last few years and I am proud to work there, and There is good teamwork at Cowbridge and generally all the staff are extremely supportive of each other, this is demonstrated if sickness/absence/emergency cover is needed, trained staff and carers help each other out in situations like this when they arise, often at short notice. We received four surveys from external healthcare professionals and we spoke to one person at their request, the comments were generally positive about the home. When asked what the home does well comments included The physical care of clients, Prevent unnecessary admissions to acute hospital, Person centred care, End of life/palliative care and Uses the GP service frequently and appropriately. When asked what they could do better comments included More client/staff interaction apart from the daily physical care. This for the most part is not possible due to staff numbers, Improve the environment (old property). However I recognise that efforts /progress has been made and work is ongoing and I believe the care could be more socially inclusive and stimulating. Other comments included The staff genuinely seem to care about the clients there just isnt enough of them to spend quality time with everyone, Cowbridge Nursing Home works with other organisations to improve the care of people with dementia. They are open to suggestions, advice and new research to improve care and outcomes and There have been variable standards over the years but staff are good at present. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a Key inspection by 25 March 2011.
