This document summarises the inspection findings of the CSCI in respect of Coxhill Manor Nursing And Residential Home. The inspection findings relate to the National Minimum Standards (NMS) for Care Homes for Older People published by the Secretary of State under the Care Standards Act 2000. The Regulations applicable to the inspected service are secondary legislation, with which a service provider must comply. Service providers are expected to comply fully with the National Minimum Standards. The National Minimum Standards will form the basis for judgements by the CSCI regarding registration, the imposition and variation of registration conditions and any enforcement action. INSPECTION VISITS Inspections are undertaken in line with the agreed regulatory framework with additional visits as required. This is in accordance with the provisions of the Care Standards Act 2000. The report is based on the findings of the specified inspection dates.Coxhill Manor Nursing And Residential HomePage 4 BRIEF DESCRIPTION OF THE SERVICES PROVIDED. Coxhill Manor Nursing and Residential Home is an extended Elizabethan Manor House, situated on the outskirts of Chobham. The Service is privately owned by Coxhill Manor Limited and is run and operated by the Principals, Dr & Mrs Swift-Hook. Both Principals are closely involved with the day-to-day operation of the home. A Matron/Manager is employed to oversee the general running of the home. Care is offered for up to seventy-four older people in single and shared rooms, all with en suite facilities. There are extensive gardens and communal areas, all of which are actively used by the service users. Car parking is available to the front of the building.Coxhill Manor Nursing And Residential HomePage 5 PART A SUMMARY OF INSPECTION FINDINGSINSPECTORS SUMMARY (This is an overview of the inspectors findings, which includes good practice, quality issues, areas to be addressed or developed and any other concerns.)Coxhill Manor Nursing And Residential HomePage 6 This was the third inspection to be undertaken in the Commission for Social Care Inspection year April 2004 to March 2005. For details of how each standard was met please refer to the main body of the report. It will be necessary to review all three inspection reports for 2004-05 to obtain a full understanding of the extent to which the home meets The National Minimum Standards for Older People. The inspector would like to thank the manager, staff and service users for their time, assistance and hospitality during the inspection. 1. Choice of Home (Standards 1-6) All of the 3 standards assessed were met The home now has a comprehensive statement of purpose and service users guide. Together these documents provide the information required for prospective service users to make an informed choice about where to live. The revised terms and conditions document now meets the standard and the new method of documentation evidences that service users needs are being met. 2. Health and Personal Care (Standards 7-11) 2 of the 3 standards assessed was met Since the previous inspection the home have reviewed and revised their care planning and documentation system. The previous requirement regarding improving the care planning and documentation system has been met with the exception of including individual plans for each service users social care needs. This was discussed at this inspection and a new requirement made. Medication administration record (MAR) sheets, medication storage and controlled drug storage and records were checked at this inspection. These were found to be in keeping with the homes policies, procedures and current legislation. 3. Daily Life and Social Activities (Standards 12-15) Both of the 2 standards assessed were met The home employs one full time activities coordinator. Activities provided at the home are varied and include: daily crossword puzzles; gentle exercise sessions; art sessions; current affairs sessions; film shows; plus entertainment and involvement from local musical groups, clergy people and schools. There are also seasonal activities/entertainment provided at Christmas and a summer garden party. There are no restrictions to visiting times at the home. Service users are able to invite their friends and family for a meal if they wish to do so. 4. Complaints and Protection (Standards 16-18) Both of the 2 standards assessed were met All required policies and procedures are in place to ensure that service users are safeguarded from harm or abuse. The home has a clearly set out complaints procedure that includes stages and timescales as required. Coxhill Manor Nursing And Residential Home Page 7 5. Environment (Standards 19-26) These standards were fully assessed at the inspection carried out in June of this year and not assessed at this inspection. 6. Staffing (Standards 27-30) 1 of the 2 standards assessed was met All interactions observed between the staff and service users were caring, respectful and inclusive. Since the previous inspection the home has reviewed their staffing levels on The Gables unit and increased the staffing to ensure that the health and safety needs of the service users on that unit can be met. The home operates a recruitment procedure that is based on equal opportunities and is designed to ensure the protection of service users. It has been recommended that the home review their recruitment procedures in line with the new Schedule 2 of The Care Homes Regulations 2001. 7. Management and Administration (Standards 31-38) 4 of the 5 standards assessed were met The registered manager at the home is a registered nurse with over twenty years experience. She is an NVQ assessor and has completed an English National Board certificated course on the Care of the Elderly. She has three years experience as a nursing homes manager and has been the manager at Coxhill Manor for the past sixteen months. She has completed her Registered Managers Award and is awaiting her final verification results. All interactions observed between the manager, staff and service users evidenced an open and positive atmosphere. Service users make decisions about their lives with assistance as needed. Service users, where appropriate, are responsible for their own medication, and are protected by the homes policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect.Standard 7 (7.1 7.6) A service user plan of care generated from a comprehensive assessment (see Standard 3) is drawn up with each service user and provides the basis for the care to be delivered. 2 Key findings/Evidence Standard met? Since the previous inspection the home have reviewed and revised their care planning and documentation system. Twelve service user care plans were sampled at this inspection. These care plans were seen to include individual risk assessments, including prevention of falls and risk of pressure sore development. The needs and goals are clearly set out with actions required of staff to meet the goals. Daily report writing now relates more specifically to the staff actions taken to meet goals and there is evidence of monthly review of the care plans. In addition the home has introduced a personal care document that is kept in each service users room, where the staff carrying out the care document their actions on a daily basis. The inspector was advised that the new system is being monitored in this early stage of implementation with staff suggestions and ideas being sought for improvement. The previous requirement regarding improving the care planning and documentation system has been met with the exception of including individual plans for each service users social care needs. This was discussed at this inspection and a new requirement made. The home must consult service users about their social interests and about the programme of activities arranged by the home. Individual care plans to include details of social care needs and how these will be met. (see Requirement Number 1) The home is to be commended for their pro-active action in this area in the five weeks since the previous inspection.Coxhill Manor Nursing And Residential HomePage 17 Standard 8 (8.1 8.13) The registered person promotes and maintains service users health and ensures access to health care services to meet assessed needs. Intake is recorded at each meal with supplements offered and recorded as appropriate. All service users are now weighed on admission and then weekly or monthly as indicated by the service users risk assessment. The inspector was advised that a GP referral is now made where any weight loss of concern is identified. Actions taken to meet service users oral hygiene needs are now recorded by the care workers on the service users personal care document. Standard 9 (9.1 9.11) The registered person ensures that there is a policy and staff adhere to the procedures for the receipt, recording, storage, handling administration and disposal of medicines, and service users are able to take responsibility for their own medication if they wish, within a risk management framework. 3 Key findings/Evidence Standard Met? The home encourages links with the local community. These links include local churches, schools and entertainers. The village of Chobham is nearby and provides shops, restaurants and a pub, which the service users are able to visit with friends and family members. The home operates a recruitment procedure that is based on equal opportunities and is designed to ensure the protection of service users. A selection of staff files was inspected. In line with the requirement made at the inspection carried out in June 04, the home has reviewed the staff files to ensure they contain the information required of Schedules 2 and 4 of The Care Homes Regulations 2001. However, since that inspection the regulations have been amended with a new replacement Schedule 2 coming into effect on 26th July 2004. Amongst other changes this schedule now requires a full employment history with a satisfactory written explanation of any gaps in employment, this information was not present in the file of the one staff member employed after 26th July. It was stated that this information will be obtained from the staff member. The Care Standards Act 2000 (Establishments and Agencies) (Miscellaneous Amendments) Regulations 2004 was discussed with the manager. The inspector was advised that the home will review their application form and recruitment procedure in line with the new Schedule 2 requirements. It is recommended that the home review their recruitment procedures in line with the new Schedule 2 of The Care Homes Regulations 2001. She has completed her Registered Managers Award and is awaiting her final verification results. The home has a quality assurance and quality monitoring system, based on seeking the views of service users. This includes informal information gathering and formal surveys. There is a residents committee at the home that holds three monthly meetings which are chaired by members of the committee. The inspector was advised that the home is preparing to carry out a survey of visitors and stakeholders in the community. Visits in compliance with regulation 26 of The Care Homes Regulations 2001 are carried out by the Responsible Individual on a monthly basis with copies sent to the CSCI, Eashing Office.
