This inspection was carried out on 30th August 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Service users feel at home and relaxed at the Clerkenwell Project because staff have a clear understanding of the needs of the service users. Service users themselves said that they liked living at the home and were satisfied with the quality of the care they receive. One service user said " I find the staff very helpful and caring. Any assistance I need staff will help". Both the General Practitioners and the Care Managers recorded that they were satisfied with the overall care provided by the home. Service users felt able to approach the manager if they had any concerns or worries. The home is being managed effectively by a well-trained manager. The line manager keeps in close touch with the staff and service users. As a result the service is being developed to meet the needs of the service users.

What has improved since the last inspection?

The manager is now registered and has completed the Registered Managers Award. A major refurbishment of the property is planned and this will include upgrading fixtures and fittings as well as bringing the Manager`s office within the confines of the service. However as this is a conservation area then compromises are likely. Attention has been paid to the smoking policy to ensure that service users and staff are not put at risk.

What the care home could do better:

An area where the home could do better was discussed with the manager. Staff need to update their medication training.

The service is operated by Camden and Islington Mentak Health and Social Care Trust. The project is run along a communal living framework with service users sharing the homes day-to-day activities. The staff team work within a multi agency team to provide individual support for each service user to live as independently as their abilities will allow. The home links in with day centres, colleges, clubs and other community resources so that service users have the opportunity to become involved in the local community. The project is situated within a quiet residential area that is also a conservation area. There is easy access to transport links, shops and other local amenities. Accommodation for service users is provided over three floors. This unannounced inspection took place over one day in August 2005 and lasted about 4 hours. The manager assisted with the inspection. The inspector was able to talk to staff and service users as they came and went from the home. A variety of records were looked at and in particular care plans. The Commission for Social Care Inspection sent out comment cards as part of the inspection process. There was a positive response and three General Practitioners, three Care Managers and four service users responded. What the service does well: What has improved since the last inspection? JUDGEMENT – we looked at outcomes for standard(s) 2 Staff work out with service users what they want and need so that the home is sure it can meet the service users wishes and needs. EVIDENCE: Two of the service users said that they liked living in the home. Overall the comment cards from service users echoed this although some only liked living in the home “some of the time”. Three care files were inspected and each one contained an appropriate assessment. Service users are involved in assessments and reviews. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. JUDGEMENT – we looked at outcomes for standard(s) 6 & 9 There is a clear care planning system in place to ensure that service users needs are being met. EVIDENCE: The records for three service users were looked at and indicated that for each one there is a current plan of care which sets out the needs of the service users and how they are to be met by the home. There is clear evidence that these were being reviewed regularly. In addition to the care plans there are minutes of regular meetings between the key worker and their key service user. One record showed that the minutes of the meetings are read and agreed at the start of each new session. Each file contained an assessment of any areas where there was considered to be any risks to the service user and how these risks were to be dealt with. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 & 17 The arrangements for group and individual activities are well organised and attempt to meet individual needs. The meals in the home are good involving service users to ensure that choice and variety are available. EVIDENCE: Service users meet regularly as a group and individually with their key workers in order to discuss all areas of care and support including social activities. ON the day of the inspection an outing had been organised. An inspection of the care records and discussions with the service users identified that a range of social events are planned; these include boat rides, pub outings, video nights and day trips. The minutes of the key working meetings show that integration in the local community is discussed and encouraged. An example is of confirmation that a service user has a place at a local centre. Staff support service users to obtain higher benefits through an advocacy link. The weekly menus are discussed at the house meetings. The minutes of the meeting show that service users have been giving positive feedback about the quality and variety of the food provided. Overall the comments cards completed by service users confirmed that they liked the food. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 & 20 The support workers have a good understanding of the health and welfare needs of the service users. The systems of administration have been subject to review to ensure the safety of service users. EVIDENCE: All service users are registered with a General Practitioner and have access to other health care professionals. Generally the GP’s who responded by sending back questionnaires felt that the staff had a good understanding of the service user’s needs and communicated and work in partnership with the practice. An inspection of the care records showed that service users attended out patients’ appointments at local hospitals. The service manager has looked into a possible drug administration error in the home and has taken steps in order to rectify the situation. Discussions with the manager highlighted that staff should attend courses in the administration of medication to update their knowledge. The GP’s generally felt that the staff in the home manage medication appropriately. The medication administration records were clear and up-to-date. A risk management strategy can be in place for service users who are assessed as competent to manage their own medication. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. JUDGEMENT – we looked at outcomes for standard(s) 22 There is a clear complaint’s procedure and service users are able to make their views known. EVIDENCE: The home has policies and procedures in place in relation to reporting and investigating complaints. Service users said that they knew who to approach if they had any concerns about the care they were receiving. There is a complaint’s procedure made known to the service users. They said they could raise concerns as part of their key working meetings or as part of the house meetings. Those service users spoken to felt confident to approach the manager if they had concerns. The Care Manager’s and the GP’s who responded by returning comment cards recorded that they were satisfied with the standard of care in the home and had not received any complaints. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. JUDGEMENT – we looked at outcomes for standard(s) 24 & 30 The home provides a safe and comfortable environment for the service users. EVIDENCE: The home is due for major refurbishment, which will provide improved facilities for both service users and the staff. As the home is in a conservation area it does pose some difficulties with making changes to the exterior as well as the interior of the property. The home is in keeping with the neighbourhood and it does not stand out as a care home. Service users have access to public transport links and community facilities. An integrated fire alarm system is installed and this is tested on a regular basis. A partial tour of the home found it to be clean and tidy. EVIDENCE: The manager has successfully completed the fit person process for registration and has also completed the Registered Manager’s Award and NVQ 4 in Care. A wide range of records were looked at including fire safety and accident reporting. These records were detailed, up-to-date and accurate and confirmed that the home is being run responsibly with essential checks being made and acted upon. Fire procedures are discussed in the house meetings. A fire risk assessment is in place however records show that there is no map of the building, which the fire brigade would require in the event of a fire.
