The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Staff spoken to felt they were valued and listened to and all of the residents spoken to felt cared for and respected.

What has improved since the last inspection?

The area which was not being fully utilised is now clear and ready for use by residents. Application has been made for funds to replace existing flooring in toilets. A new emergency call system is in place. Quotes have been sought for providing more radiator covers. Although there is no evidence that residents` assessed needs are not being met, the manager has presented proposals regarding staffing levels to the registered providers. A new fridge has been acquired, as has a new thermometer.

What the care home could do better:

The health, safety and welfare of both residents and staff members could be safeguarded by carrying out urgent work to the electrical installation as identified in the periodic electrical installation inspection of October 2003.

Date of last inspection 03/12/04 Brief Description of the Service: Double Day Lodge is owned and run by Kent County Council. It is an Old Persons Direct Service Unit (OPDSU). It is located on the edge of a large residential estate to the east of Sittingbourne and offers accommodation on two floors for up to 36 older people, all in single rooms. It is close to a bus route and there are small shops within walking distance. There is ample parking to the front and the side. There are two garden areas to the rear of the building. The home has 36 bedrooms, 9 of which are on the ground floor and 27 on the first floor.13 of these have en-suite facilities. This was a routine unannounced inspection. The inspector was reviewing some of the outstanding recommendations/requirements from previous inspections. The inspector was in the home for four and a half hours. A tour of the building was made and the inspector observed interactions between staff and residents, spoke to staff and also to residents and viewed some paperwork. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 Residents are protected from abuse. EVIDENCE: In-house training is provided by Kent County Council regarding adult protection. This training is carried out off site. There is also a regular programme of updates. Appropriate procedures are in place including the new protocols. Information is included within the induction training and there are leaflets available and on display for information about adult protection, including whistle blowing procedures. The manager is fully aware of the Protection of Vulnerable Adults (POVA) register and her responsibilities under the Care Standards Act. There are policies and procedures in place regarding physical and/or verbal aggression and the use of appropriate restraint and these are currently being reviewed. There are systems in place for ensuring that residents’ valuables and finances are protected including Personal Property Accounts and the use of a Financial Affairs Officer. Staff spoken to were very clear about their responsibilities and residents said that they felt safe and cared for. One person said ‘…they make sure you’re all right here. They look after you…’. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 24 and 26 Although generally the home appears reasonably well maintained, the problem identified with regard to the safety of the electrical installation means that residents may not be living in a safe environment. Residents can access outdoor areas and comfortable communal areas and their own rooms are comfortable and contain personal possessions. The home is clean, pleasant and hygienic. EVIDENCE: The home is purpose built and is within a residential part of Sittingbourne, close to the town centre with all of its amenities. Not all areas are up to date with regard to re-decoration but the manager monitors this. It would be helpful for a programme to be recorded indicating planned routine redecoration. This could also include requirements and recommendations from the inspection process relating to the environment. The inspector was informed that urgent repairs are carried out as required. Please see Standard 38, which identifies an area of concern regarding electrical safety. One of the rear garden areas has recently been redesigned with support from the local Round Table and staff members. The area is very attractive and two residents in particular spoke of how much pleasure they had from this. It includes raised beds and seating. Another garden area to the rear is also an attractive area and this is maintained voluntarily by members of staff. The communal space within the home has recently been improved by clearing an area not previously fully utilised. The use of this area will need to be carefully monitored with regard to the need for extra staff at some times. (Please see Standard 27). The furnishings and lighting in communal areas are domestic in style and offer a variety of different atmospheres. There is a room designated the Library which although small is comfortable and well used. There is a Visitors’ Room which is comfortable and welcoming and residents who smoke have a comfortable lounge for this. There is a large dining area and two lounge areas. One resident said he was ‘…very pleased…’ with his room and other people indicated that they were satisfied with their room. One resident who has lived in the home for several years said she thought her room was ‘…lovely…’. The tour of the building identified that residents can and do have a variety of personal possessions and some residents choose to have their doors locked. The home was free from any offensive odours. Everything appeared in order in the laundry including hand-washing facilities. Appropriate policies and procedures are in place and equipment and systems are up to date. A member of staff confirmed that she had received relevant training ie Control of Substances Hazardous to Health (COSHH) Regulations, manual handling and infection control. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 Residents’ current needs are met by the numbers and skill mix of staff. EVIDENCE: The rota seen indicates that there are sufficient care and other staff on duty. The record of training shows that staff are provided with various courses. The plan to use new areas within the home for communal activities will necessitate a review of staff levels. The manager informed the inspector that work has already been undertaken about this and proposals have been presented to the Registered Provider. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38 The health, safety and welfare of both residents and staff is compromised. EVIDENCE: To achieve compliance with the legal requirements of the Electricity at Work Regulations 1989 (EAWR), proof must be provided that an electrical system is safe. This includes the proper inspection and testing of a system by competent people. The inspection and testing of the electrical installation at Doubleday Lodge took place in October 2003. This identified many things that need attention, 11 of which were designated as requiring immediate action. This work has yet to take place. The manager has tried to progress this but has been unsuccessful because of procedures which depend on external agents. She is hopeful that work will be carried out in the very near future.
