What the care home does well

Although most of the residents experience communication difficulties, they were seen to be relaxed, cheerful and comfortable with staff. The staff showed a high level of awareness of good practice and of individual residents` needs. They also commented on the high level of support provided by the manager and senior staff. The general day to day management of the home and completion of records are of a high standard.

What has improved since the last inspection?

The on site recreational facilities have improved, with the on-going provision of a range of new equipment to provide stimulation and enjoyment for residents.

What the care home could do better:

Although there is a satisfactory level of training in core areas, staff might benefit from training in behaviour management techniques which are appropriate to residents behavioural needs.

Date of last inspection 15.3.05 Brief Description of the Service: Dovedale Court provides personal care and support to 9 residents who require a high level of assistance due to their learnng disabilities. The home comprises a number of self contained units situated on a housing estate within walking distance of the local town centre. Residents have their own bedrooms, bathrooms, kitchens and living space.The units, which are supervised by staff on a 24 hour basis, are part of a small complex which also includes office facilities, an activities room and a safe and secure garden. Nearby facilities include shops, health centres, churches, a cinema, and swimming pool. All healthcare and social needs are accessed within the community. The inspector was in the home between 10.30 and 14.45 and looked at the premises and some of the records. EVIDENCE: The care plan files included detailed pre-admission assessments, residents’ histories, risk assessments and guidelines. Information is also gained from other health and social care professionals and residents’ families. JUDGEMENT – we looked at outcomes for standard(s) 6,7,9 There is a clear care planning system in place and residents are consulted as much as possible according to their individual abilities. EVIDENCE: Some of the residents behaviours are monitored and recorded, with the information gained used to compile clear and useful reference charts. The staff had a good understanding of residents’ needs and referred to their input and use of the care plans as a working tool. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15,16,17 The relationships between residents and staff are good and create an environment where residents are encouraged to participate in a range of leisure activities. Staff are supportive and caring and residents’ rights are respected. Nutritional needs of residents are well catered for and residents are able to exercise as much choice as possible over their diet. EVIDENCE: Residents were seen to be relaxed and comfortable with staff and able to communicate with them. A range of methods, such as picture boards, are used to enable staff and residents to communicate and for residents to make their needs known. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19,20 Residents healthcare and social and emotional needs are met. The medication management systems are generally sound but storage may need reviewing. EVIDENCE: Care plans referred to monitoring residents’ health care needs and these were discussed in detail during the staff handover period. Staff have received appropriate training in the administering or medication and the records were clear and current. The systems are generally sound and storage is secure, but the current system of storing medication in domestic style kitchen cupboards should be checked to ensure it complies with current guidelines. Staff interviewed referred to the importance of offering personal support in such a way as to preserve residents’ rights and maintain their dignity. JUDGEMENT – we looked at outcomes for standard(s) 22,23 The home has a satisfactory complaints process and residents are protected from harm or neglect. The home makes every effort to seek residents’ views. EVIDENCE: Staff said that every effort is made to ensure that residents can communicate their feelings if they are not happy with something. The home has adult abuse procedures in place and staff have received training on adult protection awareness. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,30 The standard of the environment is generally good with the exception of some minor maintenance needs.. EVIDENCE: The bedrooms and living areas were furnished and decorated to a good standard, and contained the type of furniture and equipment necessary to provide a homely environment. Kitchens and bathrooms were clean and hygienic. There are some areas, eg one residents’ bathroom, which require attention. JUDGEMENT – we looked at outcomes for standard(s) 34,35 Although residents are supported by sufficient, competent and appropriately trained staff, staff would benefit from further training. Recruitment practices are generally sound but would benefit from improved checking systems. EVIDENCE: At the time of the inspection there were 5 staff on duty. There is an intercom system throughout the individual units to enable staff to respond to residents needs. Staff files showed that CRB checks and references are obtained, but references are not currently being verified by phoning the referees. Staffing is stable, with staff saying that they are well trained and supported. However staff would benefit from specific training in how to manage difficult situations, but this training should be appropriate to the needs of the particular residents. The manager has a good understanding of the areas where the home could improve. EVIDENCE: Records, polices and procedures and maintained to a high standard and regularly reviewed as part of the organisations quality assurance system. The manager showed a high level of awareness of the need to monitor and review the methods and systems associated with providing good care.
