What the care home does well

Care plans provide appropriate information with regards to residents individual needs and how these are to be met. The health care needs of residents are monitored and met. Activities provision within the home is varied and engaging and co-ordinated by a motivated and enthusiastic manager. The staff team is committed to providing a high standard of personalised care. The Registered Manager provides good leadership and ensures an open and inclusive atmosphere within the home.

What has improved since the last inspection?

The Registered Manager has successfully completed her Registered Manager Award. The bathing facilities in one bathroom have been upgraded.

The home is set in landscaped grounds which back onto woodland. Accommodation is provided over two floors which are serviced by a passenger lift. All Service User rooms are single occupancy, the majority of these rooms have ensuite facilities. In addition there are two lounge areas and a mezzanine seating area, separate dining room, television room and assisted bathing and shower facilities. There is also a large activities room which accommodates a daily programme of varied activities which Service Users can access as they wish. The Registered Providers of the home are BUPA Nursing Homes Limited. This was an Unannounced Inspection which took place over a total of 8 hours. During the inspection the Inspector reviewed care plans and staffing and other related records. The Inspector also spoke with a number of residents, their relatives and other visitors to the home as well as the Registered Manager and staff on duty. It was clear from the above, that the Registered Manager is committed and experienced and that both she and the competent and motivated staff team provide consistently high standards of care at the care home. The home provides a welcoming and homely atmosphere and residents spoke very highly of the care provided by the staff team, comments included ‘the staff are very good,’ and ‘I am very happy here’. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 4, 5 All residents have a pre admission assessment of need undertaken prior to moving into the home. The home is able to meet the assessed needs of the current residents. Prospective residents and/or their representatives are able to visit the home prior to moving in on a permanent basis. EVIDENCE: Pre assessment records were seen for five residents who have moved into the home since the last inspection. This assessment is undertaken by the Registered Manager and covers all aspects of personal, social and health care needs. A number of residents spoken with confirmed that they had been visited and their needs discussed prior to moving into the home. It was clear through records seen and discussions with residents and staff that the home is able to meet the needs of the current residents. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 All residents have a care plan in place which provides details of their health, personal and social care needs. Residents health care needs are monitored and met. EVIDENCE: The care plans of five residents were reviewed during the inspection. Where a need has been identified either through the pre admission assessment or further review a care plan is in place outlining the aims and goals of any staff intervention and giving clear guidelines for staff as to how the needs are to be addressed. The care plans covered all aspects of health, social and personal care needs as required. Within the care plans a daily support chart was in place which staff complete to confirm the care given. Staff spoken with demonstrated a good understanding and awareness of the individual needs of the residents and how these were to be met. Residents spoken with were satisfied that their care needs were met by the staff team. Relatives visiting the home were also generally satisfied that the care needs of their relatives were being met by the staff team. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 15 With the exception of main meal times daily routines can be flexible to meet residents preferences. Residents are able to maintain contact with family and friends and access the local community A variety of nutritious meals are available for residents which are served in the dining room or in individual rooms dependent on personal preference. EVIDENCE: It was clear through care plans and discussions with residents and staff that daily living routines and activities can be flexible and take into account personal preferences. A range of leisure and recreational activities is available to residents. The weekly programme of events is co-ordinated by the activities manager, and details of the activities on offer are distributed to all residents each week. During the inspection a harvest festival service was held, in liaison with the local church, from which a number of volunteers had come to participate. The volunteers from the church confirmed that they visited the home four times during the year to hold a service; at Easter, summer, Harvest festival and Christmas. In addition to these services a weekly service is held at the home. All of the residents spoken with confirmed that they attended the activities that they wished to do so and enjoyed them very much. In addition to group activities the activities manager also arranges to spend time individually with those residents who do not enjoy group activities or who are unable to attend. All residents spoke very highly of the activities manager. In addition to activities within the home, outings are also arranged and a number of residents spoke of a trip earlier in the week to a garden centre. Some relatives had also taken part in the service and those spoken with confirmed that they are able to visit the home at any time and are always welcomed by staff. A daily menu is displayed outside the dining room. Residents advised that they have a choice of main meals and were generally satisfied with the meals served. It was clear through discussions with the Registered Manager and the staff team that every effort is made to cater to individual likes and dislikes. EVIDENCE: There are clear policies and procedures in place within the home in respect of adult protection. Staff have received training in this area and this was confirmed by the care staff spoken with. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21. The residents live within in a safe and well maintained environment A range of both indoor and outdoor communal facilities are available to the residents. There are sufficient and suitable toilet and bathing facilities provided within the home. EVIDENCE: The home is accessible, safe and well maintained. Internally the home is kept clean and tidy with routine maintenance undertaken as required. Externally the large garden and grounds are well kept and provide an accessible and attractive area for residents and their visitors to enjoy. Accommodation is provided over two floors with a lift in place allowing residents access to both. All of the rooms are appropriately and adequately furnished and well maintained. The majority of individual rooms are for sole occupancy however there are six double rooms. Residents spoken with were very happy with their rooms. They are able to bring in their own furniture if they wish to do so. Furniture can also be provided. Twenty nine of the rooms provide ensuite facilities. In addition to this there are further appropriate bathing facilities, one of which has recently been upgraded. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29, 30 The needs of the current residents are met by the numbers and competencies of the current staff team. The home is committed to providing National Vocational Training for its staff team. Residents are protected by the homes recruitment procedure which is thorough and robust. There is a staff training and development programme in place. EVIDENCE: Current rotas for the home were seen and these showed that staffing levels are provided consistently and in sufficient numbers to meet the needs of the current residents. Agency staff are used to cover annual leave and sickness, but when this is necessary the same agency staff are used whenever possible. Five of the current care staff team have an NVQ at level 2 or above with a further four registered to start the training. In addition a range of training is available to staff. Staff spoken with advised that they had received training in a variety of areas including moving and handling, food hygiene, health and safety. In addition the Registered Manager keeps a training action plan which provides an annual overview of training given/planned for the forthcoming year. The records of two staff employed since the last inspection were seen and these demonstrated that the home operates a thorough recruitment process with all required information and checks obtained prior to the commencement of employment. Staff confirmed that they received induction training once they had started work at the home. Induction training packs were available. Staff considered that communication throughout the staff team was very good and effective. Staff also considered that they worked well together as a team with individual members advising that the staff had been very welcoming and supportive when they had first started to work at the home. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33. The Registered Manager is qualified, competent and experienced to run the home and meet its stated purpose. The management approach within the home creates an open and positive atmosphere The home is run in the best interests of the residents. She has now successfully completed her Registered Manager Award. It was clear in talking with the Registered Manager that she participates within the staff training programme enabling her to update her knowledge and skills base. The Registered Manager demonstrated a good knowledge, awareness and understanding of the current and changing needs of the residents and how these could be met. It was evident through discussions with residents, staff and visitors that the Registered Manager encourages an open and inclusive atmosphere within the home. The Registered Manager advised that she speaks with residents on an individual basis each day and this was confirmed with residents spoken with. Both residents and staff spoke highly of the Registered Manager with comments including ‘the manager is excellent’, ‘she is very supportive and will work alongside us’.
