This inspection was carried out on 13th December 2005.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The service provides a homely, comfortable and safe environment in three acres of beautifully landscaped gardens. It is positive in encouraging service users to maintain contact with family and friends and welcomes feedback from service users. Emphasis is placed on supporting service users in a positive way and maintaining their independence, privacy and dignity.

What has improved since the last inspection?

Since the previous inspection there has been an improvement in the quality of assessments completed for new service users. The new Manager is in the process of assessing the developments required in order to improve the service provided and is tackling some of the existing issues such as the staffing arrangements for the cottages and how this impacts on the service provided in the main house.

What the care home could do better:

The home still needs to fully implement a quality assurance system based on the views of service users and develop a final resolution to the issue of night staff having to support people who live in the cottages. There is also a need to improve practice in recording medication administered in the home and for staff to have further training in this area.

It has been modernised to meet the requirements of the National Minimum Standards but still retains much of the old character and charm of the original house. Caring Homes Limited who have recently acquired it owns the house. The previous Registered Manager has now left and a new manager has been recently appointed. The home is set in three acres of landscaped gardens on a site that also includes seven cottages and a number of apartments that also house older people (referred to as The Cottages). These dwellings do not form part of the homes registration and are not registered in their own right either. It has been common practice for staff from the home to support the people living in The Cottages with some tasks including taking them meals and assisting them at night if required. This has detracted from the support available to people living in the main house. Part of the new manager’s role is to address this problem. This was an unannounced inspection and was the second inspection of this home in the current financial year. The inspection concentrated on the key standards not assessed at the previous inspection, along with those standards where requirements or recommendations had been made last time. During the inspection the Inspector toured the premises, looked at relevant files and spoke with the Manager, four staff and five people living at the home. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Service users moving into the home are clear that their needs will be met. EVIDENCE: The Inspector looked at six service user files and each one contained an assessment that had been completed prior to them moving into the home. The assessments were completed by staff who had received training in assessing and service users were involved in their own assessments. The assessments covered a full range of need areas and were written clearly. Care plans had been written in response to the issues highlighted on the assessments. Service users spoken with were aware of their assessments. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 and10 Service users were respected by staff but were not fully protected by the home’s medication practices. EVIDENCE: Staff involved in administering medication had received only a one-hour training session from the Chemist in administering medication. The new Manager acknowledged that this was insufficient and this was confirmed by the fact that there were multiple errors on the recording sheets for medication. Some medication had not been signed for, some had been signed for where it had not been administered, some had been signed for for the following day and the Manager acknowledged that not all staff were recording in the same way. It was agreed that more in-depth training needs to be put in place for all staff involved in administering medication and a requirement has been made in respect of this. Staff spoken with and observed on the day of inspection demonstrated an ability to support service users in a respectful way and training records showed that this had formed part of their induction. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Service users benefited from support to control their own lives. EVIDENCE: Service users spoken with confirmed they were able to bring their own possessions with them to the home. Information on advocacy services was freely available within the home for service users and their families or representatives. Service users were encouraged to be independent in managing their own financial affairs and, where they were no longer able to, relatives took on this role. Service users spoken with said that they were able to control and make choices about their own lives as much as they wanted to and felt there were no unnecessary restrictions placed upon them in this respect. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 and 29 Service users were protected by the home’s recruitment practices and their needs were met by adequate staffing arrangements. EVIDENCE: The staff rota showed that sufficient staff were planned for each part of each day and matched with the number of staff on duty at the time of inspection. Staff numbers were calculated according to the needs of service users and were regularly reviewed. The number of domestic and catering staff ensured that care staff were able to concentrate on supporting service users. The staff are currently covering some care and other needs of people who live in The Cottages in the grounds of the home. This sometimes detracts from the support available for people living in the main house. The new Manager is aware of this situation and the need to ensure that staffing for the main house and The Cottages needs to be separate. This was a requirement from the previous inspection and the company has made this a high priority for the new Manager to address. The requirement remains in this report. Staff files demonstrated that the home’s recruitment policies were being adhered to. References and Criminal Records Bureau checks were received prior to staff beginning work in the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 and 33 Service users will benefit from having their views sought when the quality assurance process is fully implemented. EVIDENCE: The Manager had only been in post for three weeks at the time of inspection and so is not yet registered. She is appropriately qualified and has previous experience of managing similar services and has been a Registered Manager in the past. The Commission for Social Care Inspection is waiting for an application for this Manager to become registered. There are regular audits of each aspect of the service, but these are mainly management audits that rely on analysis of statistical information about the service. These are useful, but need to be complemented by information from service users and their families about the quality of the service. The company is keen to ensure that the home is run in the best interests of service users and is introducing systems to support this. They hope to use the information gained from these to identify areas for improvements and implement a plan to achieve those improvements. Staffing arrangements for the 31/03/06 main house must be kept from staffing support for The Cottages. This requirement was made at the previous inspection on 8/7/05.
