This inspection was carried out on 17th May 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Eastfield provides a welcoming, homely and well furnished environment. Individual bedrooms are very comfortable and residents bring furniture from their previous homes if they choose to. Good staff morale is evident, at the time of the inspection there were no agency staff employed in the home. Residents and relatives expressed a high level of satisfaction with the care provided. Choice, privacy and dignity are respected. Meals are of a high standard and freshly cooked, emphasis is given to using fresh ingredients. The health needs of residents are appropriately met and care given by competent staff. A varied programme of activities is on offer.

What has improved since the last inspection?

Since the last inspection on 9th March 2005, the ramp from the lounge to the garden has been extended and is painted with a non slip surface. The laundry room has been improved and new washing machines installed. New furniture has been provided in the lounge and dining room. A new Head of Care who has extensive experience of working in the home has been appointed following the previous Head of Cares` retirement. The Service Users` Guide and Statement of Purpose have been revised and the complaints procedure recording method improved upon. Staff supervision and supervision recording have been improved.

What the care home could do better:

The home must ensure that all staff recruited have new CRB checks even if they had a recent check with a previous employer. The recording of applicants` responses at interview could be improved upon to ensure an accurate record of the interview in the best interests of the home and applicant. The information in care plan folders should be readily accessible in the correct sections. Written confirmation that the home can meet a prospective residents` needs when a place is offered should be given.

It is near to public transport services and a junction of the M20. Shops and other services and facilities are located in the town centre. The property provides accomodation for 43 residents on the ground and first floor, there is a shaft lift.Of the 36 bedrooms, 7 offer double accomodation. There is a pleasant ,safe and secluded garden and patio with access for residents. This announced inspection took place over six hours, the Manager of the home, Head of Care and Resources Manager provided information and documentation. Five residents, two members of the care staff, the chef and a relative were individually spoken with. Much of the inspection focussed on discussion with senior staff and reading of documentation. The lunchtime meal was sampled and lunchtime observed. During the inspection the daily routines could be observed. A full tour of the premises took place. Comment cards had been received from one GP, four Health and Social care professionals, three care managers, thirteen relatives and visitors and twenty two residents. Several cards from relatives and residents were very complimentary, two residents recorded “I cannot imagine that I would enjoy living more in any home” and “It is a super place really” What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5, and 6. Prospective residents are provided with information the help them make an informed choice before moving to the home. EVIDENCE: Prospective residents and their relatives have access to a written Statement of Purpose and Service Users’ Guide. Following referral a full assessment of needs takes place before confirming that a place at the home is to be offered. This should routinely be followed up in writing. Prospective residents and their relatives are able to visit the home before admission and spend time there to be sure they are making the right decision. Residents confirmed that dependant upon their health at the time of referral to the home, they were able to visit, or relatives did so on their behalf. A relative visiting stated that several homes were visited before choosing Eastfield, which they had found to be the most welcoming. A resident very recently admitted to the home was assisted to settle in and commented on ‘the lovely bath and breakfast’. It was evident that residents accommodated for respite care are encouraged to be independent and return home if possible. A comment card received from a resident receiving respite care recorded that the home was ‘helping them to get fit’. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10 and 11. Residents have individual care plans. Health, personal and social needs are well met. Residents are treated with dignity and have their privacy respected. EVIDENCE: A number of care plans were inspected. All contained comprehensive information regarding individual health, personal and social care needs. The home has good relationships with local GP’s and District nurses, residents are able to stay with their known GP if possible. District Nurses visit regularly and one resident spoke of being visited at the home by a physiotherapist. Medication procedures are in place with records completed correctly. A small number of residents self medicate, this is risk assessed, reviewed and closely monitored. A medication round took place appropriately. Two members of care staff were present and during the time the medication trolley was in view it was always attended. Residents felt that they were treated with dignity and their privacy was respected. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 and 15. Residents are able to maintain social contacts and exercise choice over their lives. EVIDENCE: Residents are encouraged to maintain contact with friends and family. During the inspection a number of visitors were present at the home. They clearly felt comfortable to visit freely and take their friend or relative out. Residents considered the home offered activities of interest and that the facilities were good. The home employs an activities coordinator. The range of activities includes musical sessions, bingo, films and musical movement. Rooms are furnished to individual tastes. If residents wish, furniture can be brought from their previous homes. The lunchtime meal was very well cooked and presented. Residents confirmed they enjoyed their meals and were offered choice. The cook said that all meals are freshly cooked with fresh fruit and vegetables delivered daily. Responses from residents included references to the good quality of food. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18. Residents can be confident that complaints would be listened to and acted on. EVIDENCE: The home has a complaints procedure displayed on the notice board near the entrance and included in the Statement of Purpose and Service Users’ Guide. There had been no complaints recorded. Since the last inspection the format for storing any possible complaints has been improved. Residents were aware that there was a complaints procedure and felt confident that their complaint would be taken seriously if they had cause to make one. The home has an Adult Protection procedure. New care staff receive Adult Protection training as part of their induction. Ongoing training is provided by the home. POVA training will be sourced, at the time of the inspection there was some difficulty in accessing training due to demand. Staff are only appointed if they have been CRB checked, some staff who had been appointed with checks from previous employment need to have their CRB’s reapplied for as a priority. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25 and26. The standard of the environment in the home is good providing residents with a safe, attractive and comfortable home to live in. EVIDENCE: The home is well decorated with plenty of space and light. Two lounge areas are available with views to the well maintained garden. Some furniture in the lounges has recently been replaced to make the surroundings more homely. There is a patio area with chairs, tables and umbrellas. Since the last inspection on 9th March 2005, the ramp allowing access to the garden has been extended. Bedrooms offer single and double accommodation. Residents can choose to bring their own furniture if they wish. Residents liked their rooms, those who had moved in more recently were planning for more furniture and other items to arrive from their previous accommodation. Sufficient bathrooms and lavatories are available for residents without en suite facilities. The home was cleaned to a high standard throughout and there were no unpleasant odours. Specialist equipment was provided according to needs. It is an ongoing recommendation that an Occupational Therapy assessment of the premises take place. Records seen evidenced that servicing and maintenance was up to date. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29 and 30. The needs of residents are met by a well trained and competent staff group whose morale is good. EVIDENCE: At the time of the inspection there was one part time care staff vacancy, agency staff were not employed at the home. Sufficient care staff were on duty to cater for residents needs. The recruitment process is well established but should include CRB checks for new staff even if they have had a recent check by another employer. All staff have induction training which includes health and safety, first aid, fire procedures and adult protection. Eight care staff had gained the NVQ 2 in care and a further four were currently on the course. The home is hoping to reach the target of 50 NVQ qualified staff in 2005. All residents spoken with were complimentary about the care staff, a comment card from a relative stated “the care and attention are second to none. The staff are to be commended on their unfailing courtesy and good humour at all times”. Staff spoken with stated that they enjoyed working at Eastfield and felt well supported. During the inspection staff treated residents and visitors with respect and were attentive. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,35,37,and 38. The home is well managed and run in the best interests of residents. Staff benefit from good managerial support. EVIDENCE: The registered manager of the home is experienced in the provision of care and is qualified to manage a care home. The atmosphere in the home is friendly and welcoming. The responses of care and other staff and personal comments from residents and relatives in person and in writing show that the home is run in the residents’ best interests. During the inspection staff the attitude of senior and other staff members was open and transparent and staff spoken with were enthusiastic. The home is not involved in residents’ financial affairs. Staff confirmed that regular supervision is provided with records made. Regular staff meetings are held and recorded. The home operates an internal quality assurance policy seeking comments from residents and others. Residents would benefit from receiving feedback to comments raised through residents meetings.
