This inspection was carried out on 10th October 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Residents spoken to stated that the staff are kind and attend to their needs as promptly as possible. Three comment cards received from relatives also commented on the kindness, helpfulness and friendliness of the staff "always greeted with a smile".

What has improved since the last inspection?

The premises have improved since the last inspection with the complete refurbishment of the kitchen; which makes it fully compliant with the requirements of the last environmental health report, the installation of a new lift and the conversion of a bathroom to a shower room, which the residents are said to enjoy.

What the care home could do better:

Topical medicines are not currently being signed for and the manager stated that she will put this practice in place.

Date of last inspection 16 05 05 Brief Description of the Service: Elton Lodge is a care home that is registered to accommodate twenty-one service users, over 65 years of age (registration category OP), who may require nursing care. Six of the twenty-one service users may be over 50 years of age with a physical disability (registration category PD). The registered provider Elton Lodge Ltd and the responsible individual on behalf of the company is Miss R Saimbi. The registered manager is Mrs S Lamb . The home is situated in Worthing close to the seafront and town centre. There are twenty-one single rooms arranged over two floors. There is a passenger lift in place. There is a large lounge/dining room plus a separate smaller lounge. There is a garden with a large patio area that is accessed by a ramp. The inspection took place on 10/10/05. The premises were inspected, residents, and staff were spoken to and all expressed that a high standard of care was offered. The premises remain in good decorative order and a new passenger lift, the conversion of a bathroom to a shower room and a full kitchen refurbishment. The manager was available throughout the inspection and she made all documents required available. The overall standard of care observed was good and there was evidence that the needs and opinions of residents are taken into consideration. Residents were very happy with the care offered and felt that they could complain about any issue and it would be addressed. A pre inspection questionnaire has been completed by the manage rand three comment cards have been received at the Commission. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1-3 Prospective residents have the information they need to make an informed choice about where to live. Each resident has a written contract/ statement of terms and conditions with the home. No resident moves into the home without having had his/her needs assessed and been assured that these will be met. EVIDENCE: A Statement of Purpose is in place and was available for inspection today. A Service User Guide is given to all Residents and contains information relating to the home and terms and conditions of their stay. Care plans inspected show pre assessment of residents prior to admission to the home. A recently admitted Resident stated that she had been helped to settle in very quickly. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7-9,11 The resident’s health, personal and social care needs are set out in an individual plan of care. Residents make decisions about their lives with assistance as needed. Residents, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Resident’s are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. EVIDENCE: Care plans inspected show that Resident’s are recorded and care is given according to those needs. There is evidence of monthly update but not in social needs as it was noted that the social needs of one resident were now quite different from when he first moved in. The manager stated that she would rectify this. Medicines are appropriately stored and there are policies and procedures in place to ensure the health, safety and wellbeing of residents. Three residents self medicate and this is recorded, they also have a lockable space for medication. Discussion was held with the manager as to when is the appropriate time for discussion with the Resident on their wishes following death, as this was not recorded in the care plans inspected. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) These standards were not inspected on this occasion as they were fully inspected fully and met at the last inspection, It was however noted that the kitchen has been fully renovated and a new chef has been employed who will be reviewing the menu over the coming weeks. JUDGEMENT – we looked at outcomes for standard(s) 16-18 Residents and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Resident’s legal rights are protected. Residents are protected from abuse EVIDENCE: Residents spoken to stated that their complaints would be listened to, taken seriously and acted upon, one resident spoken to gave an example of this and stated that she was very happy with the outcome. There is a complaints procedure in place and it is on display. Resident’s solicitors are consulted as necessary to maintain their legal rights, There are policies and procedures on abuse and these were available for inspection. Adult Protection training is in place and evidence of this was seen in staff files. Residents have sufficient and suitable lavatories and washing facilities. Residents have the specialist equipment they require to maximise their independence. A bathroom has been converted to a shower room, which is said to be preferred by most residents. The home is well redecorated creating a warm atmosphere in the communal rooms. The majority of the bedrooms had been personalised with pictures, photographs and personal memorabilia according to individual tastes. The requirements of recent environmental health and fire inspections have been met. Bedrooms were clean, tidy and well presented; therefore residents do live in a safe and well-maintained environment. EVIDENCE: Staff files inspected had two references and Criminal Records Bureau clearance and a check against the Protection of Vulnerable Adults list. Policies and procedures are in place to ensure the recruitment process is rigorous. JUDGEMENT – we looked at outcomes for standard(s) 31,32,35,36,38 `Residents live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Residents benefit from the ethos, leadership and management approach of the home. Residents’ financial interests are safeguarded. Staff are appropriately supervised. The health, safety and welfare of service users and staff are promoted and protected EVIDENCE: The manager is a Registered Nurse and has almost completed the Registered Managers Award. Staff and residents spoken to stated that the manager is very approachable and acts on comments or suggestions. Comment cards received stated that the home is a happy and welcoming place. Staff supervision files were available and staff confirmed that they receive supervision. The manager has policies and procedures, staff training, health and safety checks and risk assessments in place to ensure the health, wellbeing and safety of residents and staff.
