This inspection was carried out on 27th February 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Fair Glen, Gate House and Maurice House are three separate self-contained units, which are homely and comfortable. Maurice house is a new building and has been designed to offer modern attractive accommodation for 5 people. The philosophy of the home promotes self-help skills, independence and decision-making. Activities and leisure interests have been encouraged. All the residents were going on holiday in May 06.

What has improved since the last inspection?

The Registered Manager and the deputy have both attended the Adult Protection training on 25th Jan 2006 as recommended at the previous inspection. The information will be cascaded to staff in supervision and in a staff meeting to ensure they are aware of the prevention of abuse.

Age 18 - 65 The home may accommodate two service users named elsewhere who have mental health needs 6th October 2005 Date of last inspection Brief Description of the Service: Fair Glen/The Gate House is a care home providing personal care and accommodation for seventeen people aged 18 - 65, with learning disabilities. Mr and Mrs Purgaus privately own the home. This home is located in the residential area of Whitleigh, close to shops, pubs, the post office and other amenities. The home is at the end of an un-adopted road, close to a housing estate, and overlooking the local playing fields. The home was opened in 1987 and is comprised of two detached two-storey houses. Fair Glen has twelve bedrooms and The Gate House has five bedrooms. All the homes bedrooms are single and none of them have en suite facilities. Both of the houses have separate lounge and dining rooms and Fair Glen has a conservatory off the dining room. The home has large grounds; a patio and all areas are accessible to the Residents. The owners have now extended the property and included another house next to the existing properties and have called this Maurice House. At present this property has one resident. The home is staffed 24 hours a day with sleep in staff in each house at night. This was an unannounced inspection, which took place on Monday 27th Feb 2006. There were 17 residents living in the home and 8 were at home during the inspection. The inspector toured the houses, spoke to residents and staff and discussed the management of the home with Mr Purgaus, the owner. Records were sampled. The focus of this inspection was to talk to residents and hear their views about the quality of the services being provided. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Prospective residents can be confident that the home will be able to meet their assessed needs. EVIDENCE: Fair Glen is located near shops and amenities in Whitleigh. An important part of enabling residents’ independence has been to encourage them to use the local facilities either with support or independently. Led by the Registered Manager, staff have the experience and attitudes to work sensitively with residents according to their assessed care needs. Evidence of this was seen during the inspection. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 Residents can be confident that the information about them that is held by the home is handled and stored in a confidential secure system. EVIDENCE: Residents’ personal files are held securely in the office. Residents have access to their own files if they wish. Privacy is respected and anyone who wishes to speak to a member of staff in confidence does so behind a closed door. Evidence of this was seen during the inspection. Meetings and reviews, which take place in the home, are held in the dining room with the door closed. Residents said they were treated with respect and that they felt happy about talking to staff about personal and private matters. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11, 13, 14, 15 Residents can be confident that they will have access to a variety of activities and the encouragement of staff to develop an independent lifestyle. EVIDENCE: Residents were seen relaxing at home, listening to music, using a laptop, reading, art work and drawing, making drinks for themselves, doing their washing and going out and about to the local shops. Residents said they were very happy and comfortable in the home and felt well supported by the staff team. Mr Purgaus has encouraged residents to be independent and to take some responsibility for keeping their rooms tidy and doing their own laundry. Evidence of this was seen during the inspection. Residents were quite happy to be contributing to the domestic arrangements in the home. Fair Glen is located close to the shops and the bus route to town. Residents go out for coffee and shopping. Some residents have lived in the home for many years. Many of them have formed close friendships and personal relationships. Some spoke of their longterm aspirations and their hopes for the future. Residents’ family and friends are welcome to visit and residents said they go out to see them occasionally. Residents contribute to transport costs and have their own spending money. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19, 21 Residents are supported in a way they prefer to ensure their physical and emotional needs are met. EVIDENCE: Residents said they were satisfied with the level of support and care they receive from staff. Some residents have few physical care needs as recorded in their care plans but are encouraged and prompted by staff to care for themselves including personal hygiene and grooming. Residents said they were able to talk to the staff if they had a worry or felt upset about anything. Evidence of this was seen during the inspection when staff listened patiently to a resident’s concerns. There have been no deaths in the home since Mr Purgaus bought the home three years ago. However, residents have suffered family bereavements and Mr Purgaus has provided appropriate grief counselling and support as and when it has been needed. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22, 23 Residents can be confident that their concerns and complaints will be listened to, taken seriously and acted upon. EVIDENCE: None of the residents seen during the inspection had any complaints about the home or the services being provided. No complaints have been received by the CSCI since the previous inspection. Mr Purgaus and the senior carer both attended Adult Protection training on January 25th 2006 to ensure they fully understand the issues of abuse and the Protection of Vulnerable Adults. They will be passing on the information and learning from the course to the staff working in the home. The home also uses the TOPPS Videos for staff training. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 25, 27, 28, 30 Residents live in a well maintained home that is warm, safe and comfortable. EVIDENCE: Fair Glen is located close to all amenities in the Whitleigh Green shopping area. It is close to the bus route into Plymouth City Centre. The home is laid out in three houses all self contained and with their own facilities for independence. The bedrooms and sitting rooms provide spacious attractive living areas that were warm and comfortable. The handyman employed by the home is on hand to make minor repairs as needed. Residents have their own keys to their rooms. All the rooms are single and there were personal possessions, pictures and ornaments on display. The latest extension to Fair Glen and Gate House is a modern detached building named Maurice House, which provides a high standard of contemporary accommodation for five people, designed for ease of access, comfort and convenience. The bedrooms in Maurice House are en suite. At the time of this inspection one resident was living in the new house. The occupied bedroom had been personalised and was furnished with the resident’s own and the home’s new furniture. Bathrooms in Fair Glen are to be refurbished and shower cubicles added to the facilities. Mr Purgaus is planning to upgrade the conservatory attached to the dining room, which will add to the spacious accommodation already provided. Each house has a large sitting room, a dining room and a kitchen. The spacious Fair Glen & Gate House DS0000036285.V270387.R01.S.doc Version 5.1 Page 14 hallway in Fair Glen acts as a reception area where visitors are invited to sign the visitors’ book on arrival and departure. Throughout all three houses, there were high standards of cleanliness. The home was free from offensive odours and the bathrooms and toilets were clean and hygienic. JUDGEMENT – we looked at outcomes for the following standard(s): 34, 36 Staff are well supported and supervised to provide services to the residents with confidence. EVIDENCE: One staff member had joined the team since the previous inspection. The personal file showed that the home’s recruitment processes have been well used. There were two references, an application form, a CRB disclosure, proof of identity and a health questionnaire seen on file. Mr Purgaus provides staff supervision for all eleven staff at least six times each year. These have been recorded and include training needs assessments and appraisals. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 38, Residents living in Fair Glen benefit by a well organised home that is managed with their best interests at heart. EVIDENCE: The Registered Manager is Mr Purgaus, the owner. He holds the Registered Manager’s Award and NVQ4 in Care Management. He manages the home supported by a senior carer. The records and offices are well organised and confidential information is securely stored. Residents spoke very positively about the Manager and were confident about stating their views and commenting on the lifestyle in the home. The management approach to running Fair Glen is a relaxed style that consults with staff and residents in meetings about changes and plans. The atmosphere in the home was lively and positive. Residents pleased themselves about what they wanted to do and some joined in with the inspection process offering comments and opinions.
