Conditions of registration: Date of last inspection 22nd September 2005 Brief Description of the Service: Ferndale House is set in a residential area close to local amenities on the outskirts of the town of Gosport. It provides residential care for up to 20 elderly residents, some of whom have mild dementia. The home is on ground and first floors and there is a stairlift between these. There are a variety of aids and adaptations to allow residents to move about more independently. Sixteen of the bedrooms are single and two are doubles. All of the bedrooms have en suite toilets, and the two double bedrooms are provided with screening for privacy. There is a communal bathroom and toilet on the ground floor and a further communal bathroom and toilet on the first floor. This is an overview of what the inspector found during the inspection. Three and a half hours were spent visiting the home, during which time the opportunity was taken to look around the home, view records and policies and to talk to two of the owners. The inspector also spoke privately with two members of staff. Most of the residents were seen to be using the communal areas and a number were spoken with briefly. Two residents who had moved into the home more recently were spoken with at greater length in private. The inspector was not able to speak with any visitors to the home on this occasion. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 and 3. The admission process is well managed with an assessment of needs being done to ensure these can be met. Residents are given clear information regarding the service to enable them to make an informed choice about where to live. EVIDENCE: There is a clear understanding by the owners of the type of resident suitable for the home, the inspector being informed of an instance where a potential resident whose needs could not be met was not admitted. Two assessments relating to the two most recently admitted residents were examined and contained a good level of detail. These assessments contained clear information regarding care needs including those relating to health. Where appropriate both a care management and primary care trust assessment had also been obtained. Both residents confirmed the admission process, which had included the opportunity to visit the home beforehand. One
Ferndale House DS0000064121.V279334.R01.S.doc Version 5.1 Page 9 resident, for example, said she had ‘stayed for a day’ and had liked it, so decided not to go back to her flat. The home provides a copy of its Statement of Purpose for prospective residents and their families with clear details of its services. One resident also confirmed that she was given clear information on the cost of the service before moving in. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15. Residents enjoy varied lifestyles and undertake activities of their choice. The provision of a range of entertainment and activities is appreciated by residents, who also enjoy a varied and nutritious diet. EVIDENCE: During the inspection several residents were relaxing in the communal areas around the building and in their bedrooms. The inspector was informed that there was entertainment provided on a regular basis and that a ‘keep fit’ session was held every other week. A number of residents said they enjoyed reading and the home is visited by a mobile library on an occasional basis when large print books were exchanged. Residents confirmed the extent to which there were choices over everyday activities, one saying, for example, that she could ‘preferred to stay in her bedroom’ at times. Another resident said that she was particularly looking forward to summer so that she could enjoy looking at the garden from the conservatory. Ferndale House DS0000064121.V279334.R01.S.doc Version 5.1 Page 12 Several residents attended outside clubs, and although it was reported that noone currently wished to attend church, a priest visited one resident privately on a regular basis. Visitors are welcomed at any time but asked to avoid mealtimes if possible. The inspector was informed of the particular special arrangements that had been made recently for family members of a resident with a terminal illness. The home’s menus were examined and were very varied, the inspector being informed that individual likes and dislikes were catered for. Residents commented positively about the food at the home, one saying that she had just had ‘ a wonderful lunch’. The provision of a new cooker should enable a wider range of meals to be offered. Currently all residents choose to take breakfast and other meals communally although a resident confirmed to the inspector that meals could be taken in private if this was wanted. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 23, and 26. A comfortable clean and safe standard of accommodation with suitable bedrooms to meet residents’ needs is provided. EVIDENCE: The home is well maintained and suited to residents’ needs. It is decorated and furnished to a standard that creates a homely ambience with some areas beginning to be in need of a little redecoration. There is a lounge with an adjoining conservatory on the ground floor, together with a separate dining room. Four residents’ bedrooms were inspected and were adequately furnished and looked homely, as residents had been able to bring items of their own furniture and possessions with them. Two residents were spoken with privately in their bedrooms, one saying that she liked to spend time on her own occasionally rather than in the communal areas. One resident also said how nice the view from her window was.

The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29. Staff at the home are recruited through a sound and robust process and the new owners have instigated appropriate CRB checks for staff. EVIDENCE: Two staff records examined indicated a recruitment process, which included the obtaining of written references. Since the change of ownership some six months ago the new owners have undertaken CRB checks on all staff with nine checks having been received and the results of a further five still being awaited. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 and 38. The home is well managed and residents’ welfare is promoted through sound policies and procedures. Financial interests are safeguarded, although residents need to have a clearer, more formal, means of influencing policy and practice. EVIDENCE: The manager has a considerable number of years experience in a senior capacity, and supports less experienced family members in the day–to-day running and management of the home. Staff spoke positively about the change of ownership saying that the new owners were ‘very caring’ and that they will ‘get anything you need’. The inspector saw residents being consulted by staff informally on day to day care issues but was informed by the manager that the home did not consult
Ferndale House DS0000064121.V279334.R01.S.doc Version 5.1 Page 18 residents or their relatives formally by way of, for example, a questionnaire or something similar. This is currently being developed and it was hoped would be put in place within a period of some two months. The owners confirmed that they provided security for monies for one resident but that there was no other involvement in any resident’s personal financial matters. Two residents confirmed to the inspector that they were happy for family members to assist them with their finances. The inspector observed no immediate obvious hazards to health and safety during the inspection and staff were clearly aware of issues relating to these areas. Staff said that they would report anything dangerous and that they had access to things like protective clothing and gloves. A sample of policies and records required by regulation was inspected and all were in order and up to date. This included the home’s accident book and its Health and Safety policy. The home’s laundry is fit for purpose and contains a dryer and washing machine with disinfection capability. There is a clinical waste policy and a procedure for dealing with soiled items.
