Conditions of registration: Date of last inspection 10/01/05 Brief Description of the Service: The Ferns is a private care home, registered to provide care and accommodation for up to 36 elderly people. The home has operated since 1992 under the ownership of Mr and Mrs Arrowsmith, who live on site in a separate detached house. Mrs Arrowsmith is responsible for the day-to-day management of the home. The Ferns is conveniently located approximately two miles from the centre of Shrewsbury and enjoys easy access to all the towns main services and amenities. The building is set back from the Longden Road in attractively maintained grounds with ample parking space. It is a substantially extended large family house and offers single bedroom accommodation for service users, with en-suite facilities, in addition to ample communal space in the three lounge areas and a dining room. The Organisational Structure of the home is clearly defined. Mrs Arrowsmith has a Deputy, and also employs 20 care assistants, cooks, and cleaners. This is an overview of what the inspector found during the inspection. The inspection was unannounced and started at 10.25am and lasted 3 hours and fifty minutes. The inspection included discussion with service users and staff. Observation of daily routines, examination of service user and staff records, policies and procedures and a tour of the home all formed part of the inspection process. What the service does well: What has improved since the last inspection?
The hall corridor has been redecorated and new lighting installed. All bathrooms have been re-decorated and one room has been converted into a shower room allowing more flexibility and choice for service users. The upstairs bathroom now has a hoist in place. Mrs Arrowsmith and three other staff have completed the moving and handling instructors course. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 & 5 All service users are assessed by an appropriately qualified person before entering the home. Service users and their relatives are provided with the opportunity to visit the home to assess its quality, facilities and suitability to an individual’s needs before an admission. EVIDENCE: Evidence of service user assessment was seen on individual files examined. The latest admission to the home was happy to share with the inspector that they viewed the home prior to making a decision about moving into the Ferns. The individual was able to view the vacant room and meet with other service users and staff. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 &10 There is a good clear, consistent care planning process in place. The staff are sensitive to the individual needs of each service user and meet them in a professional manner. EVIDENCE: The care plans of the three newest admissions to the home were looked at. Service users health, personal and social care needs were set out in all care plans. The preferred terms of address were also recorded on each individual file. Care plans are reviewed on a regular basis and a record of changes/updates were well recorded. Service user care plans contained information that confirmed health care needs are met. District nurse, Doctors, chiropodists and opticians visits were recorded well. These visits were all confirmed by service users.

Service users were seen to receive their visitors in the privacy of their own rooms. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 & 15 Service users are supported to undertake activities of their own choice. Visitors are welcomed into the home and service users are able to maintain contact with their family and friends. Service users are offered a varied, well balanced diet, which takes into account individual likes and dislikes along with seasonal changes. EVIDENCE: Service users who were happy to talk to the inspector stated that they are provided with a variety of activities on a daily basis. A programme of which was seen on the hallway notice board. One service user told the inspector that they had enjoyed the visiting singer who provided entertainment to those who wished to participate the day before the inspectors visit. Visitors to the home were seen to come and go freely throughout the visit. One was seen to visit their loved one in the privacy of the individual’s bedroom.

The individual was impressed with the homes approach to the fact that this is considered the individuals ‘home’ and this is an activity that would be undertaken as a matter of course if they were still living at home. Service users spoken to were very complimentary about the quality of food. The home has exceeded in this standard on many occasions. The quality of food, variety of choice and presentation is always of an excellent standard. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 There is a clear complaints process in place. EVIDENCE: The home has a clear complaints procedure, which is explained in the homes statement of purpose, which is given to all prospective service users or their family/representative before moving into the home. Service user meetings are held regularly and minutes are kept of each meeting, this gives service users the opportunity to discuss any concerns. The manager takes all concerns complaints seriously and does her best to resolve these as soon as possible. It was stated by the manager that there have been no complaints received since the last inspection. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 & 26 The standard of the environment is good providing service users with a safe well-maintained environment to live in. The standard of cleanliness reflects the on-going cleaning schedule, which maintains this high standard throughout the home. EVIDENCE: The home is well maintained throughout, this was seen during a tour of the home. Service users spoken to were complimentary of the environment and cleanliness throughout the home, including their private space. The grounds are accessible to service users and seen in full use by many service users during the hot day when the inspection took place. Excellent standards of cleanliness were seen throughout the home.

There are polices and procedures in place for the management of infection control. Service users gave positive comments with regard to the laundry service, and told the inspector clothing is well cared for. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29 & 30 The homes recruitment processes are robust and all necessary recruitment checks are carried out before staff commence working at the home. The home continues to support staff to undertake training related to their job role. EVIDENCE: The files of the three newest recruits to the home were examined and found to contain all the relevant pre employment checks. One staff member confirmed that pre employment checks had been completed prior to employment. Two out of the three staff were spoken to by the inspector. The staff were happy to discuss their experience of induction training and gave a thorough account of the programme they had undertaken. Records of induction were seen to be maintained. One staff member told the inspector they were now undertaking their NVQ training and that the home supported them in this. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 & 35 The proprietor/manager of the home is involved in the day- to -day management of the home providing good leadership through her knowledge and experience of the service user group. Service users finances are handled appropriately in accordance with policies and procedures. EVIDENCE: Mrs Arrowsmith has been the proprietor/manager at the home since 1992. She has continued her own professional development throughout that time. She has recently completed the moving and handling trainers course. There is a clear management structure within the home and staff spoken to were aware of the lines of accountability within the home.
