The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home was responsive to peoples needs with a well trained and committed staff team. People were cared for safely.

What has improved since the last inspection?

The home had checked electrical wiring to ensure it was safe for people. People had it confirmed in writing the home was able to meet their needs following assessment to enable them to make an informed decision about moving into the home. Recording in medication administration had improved in the main home. Staff were deployed in sufficient numbers to meet the needs of the people in the home.

What the care home could do better:

The home needed to ensure when staff in intermediate care wrote out medication sheets by hand these were double checked and counter signed to ensure they were accurate and maintain peoples safety. The laundry floor required effective repair to ensure a waterproof seal for infection control purposes. Care records would benefit from revision to make the care planning and recording process a seamless working record. The registered manager should consult with the local community pharmacist to ensure PRN medication policies are best practice and well managed. The registered manager should consult with the fire officer to ensure fire safety training in the home is acceptable.

That 1 place be maintained for a current service user in the category of DE(E) until that service user no longer requires this accommodation. 19th October 2005 Date of last inspection Brief Description of the Service: Feryemount residential care home has been long established in the town of Ferryhill. The home is owned by Durham County Council and is registered for 29 Older Persons (OP) of which 9 are in the category of Physical Disability aged 55yrs or over. The home provides an intermediate care service for up to 9 persons. Service users are accommodated on the ground floor and first floor in single occupancy rooms. Throughout the home there is a range of separate lounges including a separate visitors lounge. Meals are provided in the main kitchen. A vertical/shaft lift is provided to the first floor for those less ambulant. Gardens to the rear of the home are well cultivated and provide easy access for service users. The home provides 3-day care places for service users in the community. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 People were assured the home could meet their needs prior to making a decision to move in. EVIDENCE: The home now has a letter confirming it is able to meet people’s needs following assessment. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 9. Though people could be confident their health care needs would be met and recorded in a plan of care improvements to the record keeping process would enhance the continuity of care for people. EVIDENCE: The home had failed to review it’s PRN medication policy or its record keeping practice so care plans and daily records were kept separately and daily records were completed without reference to care plans. This does not encourage consistency of care or the use of care planning as a working process staff actively comply with. Many people are prescribed pain relief up to four times per day whether they need it or not on the off chance that one day they may need it and though staff are recording when medication is not given this is effectively a PRN medication system where staff are making the decision about whether people need the medication or not. It is recommended this policy be revised in conjunction with the community pharmacist to provide clear guidance for staff on best practice to manage this issue. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 People lived in a safe and well maintained environment which would benefit from repair to the laundry floor to improve infection control. EVIDENCE: The electric wiring had been tested to ensure safety. The laundry flooring had not been adequately repaired following the requirement from the last inspection and needed repair to create an effective waterproof seal. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 and 29 Staff were deployed in sufficient numbers and skill mix to meet peoples needs. EVIDENCE: Staff recruitment and staffing levels were found to be acceptable though the fluctuating needs of the intermediate care unit did mean that agency staff were regularly used due to long term sickness in the permanent staffing team. It is recommended these sickness issues are addressed to ensure consistency of the workers for people living at the home. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 31 and 38 The home was being managed safely in the interests of service users. EVIDENCE: Electrical hard wiring had been checked and fire safety training had been implemented though still with the use of a video which is not specific to the home’s needs. It is recommended the registered person consult with the fire officer on the appropriateness of fire safety training in the home. Yes STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard OP9 Regulation 13 (2) Requirement Timescale for action 21/02/06 5. OP19 13 (4(c)) In the intermediate care unit staff were handwriting medication administration sheets without having them counter checked and countersigned to ensure an accurate reflection of the prescribing instruction. This practice must stop and staff must have any handwritten entry counterchecked and countersigned. The laundry floor requires repair 30/06/06 to ensure a waterproof seal. (Previous timescale of 31/01/06 not Met) RECOMMENDATIONS These recommendations relate to National Minimum Standards and are seen as good practice for the Registered Provider/s to consider carrying out. It is recommended long term staff sickness issues are addressed to ensure consistency of the workers for people living at the home. It is recommended the registered person consult with the fire officer on the appropriateness of fire safety training in the home.
