Conditions of registration: Date of last inspection 30th June 2006 Brief Description of the Service: Field House is a large detached house situated in the Seaside town of Minehead in Somerset. The home is situated in a quiet residential area, which is only a few minutes walk from the town centre and benefits from views over the surrounding countryside. The registered provider/manager is Dawn Paxton. Field House is registered with the Commission for Social Care Inspection to provide personal care for up to 5 service users between the ages of 18 and 65years, who have a learning disability. The home is not registered to provide nursing care. All service users are accommodated in single bedrooms that are situated on the first and second floors. The home has large landscaped sloping gardens and access to the home is via a number of steps. Field House would therefore not be suitable for service users with mobility difficulties. This is an overview of what the inspector found during the inspection. The focus of this inspection visit was to inspect relevant key standards under the Commission’s ‘Inspecting for Better Lives 2’ framework. This focuses on outcomes for service users and measures the quality of the service under four general headings. These are: - excellent, good, adequate and poor. The quality rating for this service is 2 star. This means the people who use this service experience good quality outcomes.
This unannounced inspection visit was undertaken by B.Ludlow for the Commission for Social Care Inspection (CSCI). The visit was made at teatime when all people living at the home would have returned from their daily outings, activities and occupation. The registered provider/manager Dawn Paxton was present throughout the inspection visit. Two staff, who are family were on duty with the manager. On arrival people were having their evening meal together at the dining table. There was opportunity for people to be seen either in the privacy of their rooms or in private in the communal areas. There were four people in residence and all were seen and spoken with. A tour of the premises was made and with the permission of the people living here bedroom accommodation was sampled. All communal areas were seen. Records were sampled for staff recruitment processes, general day-to-day management of the service and of health and safety management. Care plans were sampled with the permission of the people in residence. The Annual Quality Assurance Audit (AQAA) was completed and was returned to CSCI prior to the completion of the inspection period. Comment cards were received from all the people living at the home and two members of staff. Feedback from visiting health care professionals was very positive. No requirements or recommendations have been made as a result of this inspection. The inspector would like to thank the management team and the people living at the home for their time and cooperation with the inspection process. The following is a summary of the inspection findings and should be read in conjunction with the whole of the report.

The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. Pre admission assessment is made using all available information to ensure the person needs and aspirations can be met at Field House. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The home has produced a statement of purpose and service user guide. These documents provide information to support an informed choice being made by prospective people and their families or representatives enquiring about the home. The inspector was not advised of any changes to these documents. There has been one new admission since the last inspection. This person was seen and appeared to have settled in well. They had a detailed care plan with input and review from their social worker. The trial period from admission is twelve weeks to allow a new person to settle in and for all parties to ensure this is the right choice of home for them. The AQAA indicated that potential clients are offered the opportunity to visit the home and stay overnight.

Fees are based on the assessment made pre admission and range from £1172 per week. The AQAA indicated in response to Value for money? That the fees have not been increased since 2004. At the last inspection it was reported that, ‘the home provides transport for organised trips and activities at no extra charge to the service user. Service users retain any Disability Living Allowance money awarded to them.’ This was also confirmed as current practice in the AQAA. The comment cards from people living at the home indicated that they were asked if they wanted to move into this home and that they had made a positive choice. Some indicated they had meetings about their move and had received information about the home, another described a visit when they decided they “wanted to stay”, and one person was said they were given forms. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9 Quality in this outcome area is good Care planning is well informed and is person centred. People are involved in the planning of their day-to-day care. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Two care plans were seen with the permission of the people for whom they provide a record of care. The professional input into assessment was clear and informed the risk management for day to day living of the people concerned. Input from community health care professionals was in evidence demonstrating that advice and health care was appropriate for the well being individual people living at the service. The improvements over the past twelve months reflected that people are more confident and active. A commitment to source further activities and expand opportunities for people over the next twelve months was stated in the AQAA. Detailed daily records are kept and all records are safely managed to promote the privacy and dignity of the people living at the home. At the last inspection it was reported that, ‘all service users have their own personal bank accounts. The provider manager offers support to service users in managing small amounts of ‘pocket monies’ where appropriate or requested’. The AQAA indicated that this is unchanged. The comment cards from people living at the home indicated people can make their own decisions and can do what they want to do each day, each evening and at the weekend. Staff commented that they always given up to date information about the people they care for and the way that information passes between staff works well. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16,17 Quality in this outcome area is good People living at Field House can access local community activities and facilities. Individual rights are respected. Support is given to maintain community contact with family and friends. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The ages of the people in residence varied but this did not appear to detract from harmonious living seen at this visit. People have their own rooms and spend their time as they choose. People access and use the communal areas integrating into the family style of daily living with the providers and staff. The AQAA indicated that a positive choice has been made by people living at the home to eat together in the evening. This was reported to be proving a success and providing an enjoyable time. The people living at the home are actively involved in the day-to-day running of the home, helping choose the menu each week and assisting with the shopping to support this. The inspector heard how one person chooses to help with the washing up once a week. The input by people living at the home with routine chores was varied. The manager explained how the people at the home take advantage of the local community and access the shops and facilities as they wish. Support is given to those who require help with community access in line with their individual care planned needs assessment. Transport is available. The home reports success with supporting and encouraging family contact. Plans for improvement over the next twelve months include sourcing work placement or preparation for work placement, for those for whom this would be a positive choice. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 Quality in this outcome area is good. Personal support is given to build confidence and promote independence. Emotional and health care needs are met. Medication management is safely supported. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Care plans indicate that personal support is tailored to meet the individual’s needs in an enabling and nurturing way. An example was heard where reducing support with personal care is managed for one person as their confidence grows. Care plans detailed contact made with health care professionals where chronic health conditions are monitored. Health promotional checks were recorded for example eyesight screening, breast care screening, body mapping and

The AQAA indicated that the home could do better on the health care and personal support if well-woman and well-man clinics could be sourced and then attendance would be supported. Staff have received training in the safe management, storage and administration of medication. This was a recommendation at a previous inspection. Medications systems at the home have been inspected by the local pharmacist. A monitored dosage system is used (MDS) with pre printed medication administration records. Risk assessments were seen in the care plans for medication refusal or medication misuse. The AQAA reported that residents remain on minimal medication. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good There is complaints policy and procedure. People confirmed they would be able to raise any concerns with staff at the home. The homes staff are aware of their responsibilities towards safeguarding vulnerable people in their care. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The home has a complaints procedure. This is available in the homes statement of purpose and service user guide. No complaints have been reported to the home or to CSCI. Staff awareness and good recruitment practice demonstrated the homes commitment to safeguarding vulnerable people from the risk of harm. One member of staff’s file was examined and an enhanced CRB check had been made before the person commenced working at the home. The people who were spoken with were asked if they would be able to complain or raise a concern. One person responded that they “would speak to Dawn”(provider manager) if unhappy with anything. All indicated that they felt that carers would listen to them and act on what they say. Comment cards from staff indicated that they would know what to do if concerns were raised about the home. Staff said they are given relevant training and training which helps them to understand individual needs (relating to gender, faith, age, race and ethnicity, sexual orientation and disability.) The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24,30 Quality in this outcome area is good. The home is clean, hygienic and well maintained. The homes ethos is of homely family style living. People living at Field House have a spacious, good quality comfortable environment to live in. Decoration and improvements are made to benefit the people living at the home. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Field House, Minehead DS0000060745.V366925.R01.S.doc Version 5.2 Page 20 At the last inspection it was reported that ‘Field House is a large three storey detached house situated in a quiet residential area, which is not far from the centre of Minehead. The home is in an elevated position and enjoys far reaching views to the surrounding countryside. All areas of the home have been decorated and furnished to a very high standard and the home promotes a ‘homely’ feel. Communal areas are located on the ground floor, with bedrooms and bathrooms located on the first and second floor, which are accessed by stairs. There is a toilet on the ground floor. The home has large landscaped gardens to the front and rear. As the gardens are sloping, they would not be accessible to those with mobility difficulties. The home is fitted with appropriate fire detection systems and fire fighting equipment. A call bell system has been installed throughout the home. All up stairs windows have been restricted to ensure the safety of service users. All service users are accommodated in single bedrooms, which are fitted with the provision of a wash hand basin. One bedroom is fitted with en-suite shower facilities. Service users are encouraged to personalise their rooms. Privacy is respected and service users have a key to their rooms. Three bedrooms are situated on the first floor and two on the second floor. Stairs give access to first and second floor accommodation. Service users have access to spacious and comfortable communal areas, which are situated on the ground floor. The home has two very large lounge areas, which are decorated and furnished to a very high standard. A dining area is available in one of the lounges. One lounge has a television and the other can be utilised for those wishing to enjoy some quiet time. This lounge also has the provision of a payphone. Cleaning is carried out by the provider/manager. Service users are supported to clean their own bedrooms as appropriate. Liquid soap and paper hand towels were seen to be appropriately sited throughout the home.’ This was also confirmed at this inspection. Decoration and improvements had been made to the kitchen and breakfast rooms. One bathroom has been refurbished to make the bath more accessible. New fire doors have been fitted to six rooms on the first floor. The AQAA stated that there are plans to improve one other bathroom to create a walk in shower in line with requests from residents. The comment cards from people living at the home indicated that all people living at Field House found the home to be always clean and fresh. Recruitment procedures are thorough and training is undertaken. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Field House, Minehead DS0000060745.V366925.R01.S.doc Version 5.2 Page 23 The provider and her partner and her son were on duty at the inspection. The provider and her partner provide 24 hour cover for the service. The home has four permanent care staff. Of these four staff one has a National Vocational Qualification and two are working towards an NVQ in care. The AQAA indicated that the manager wants to build the staff team to enable a core team of bank staff to be deployed when needed to cover the home for holidays. The recruitment records for one member of staff were examined. These records were complete and demonstrated that references had been taken up and these were satisfactory. A Criminal Record Bureau (CRB) check had been made before the person came to work at the home. Induction had been given and there was evidence of medication and food hygiene training. One recent recruit is awaiting a CRB check before commencing work at the home. At the last inspection the provider/manager informed the inspector that she had commenced staff supervision sessions. The home has a staff supervision policy and supervision is informal due to the team being small. Records were sampled and these demonstrated that staff supervision is regularly undertaken and is recorded. The comment cards from staff indicated that they receive support and training. It was commented that they work well as a team and would discuss and work through any difficulties. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39,42 Quality in this outcome area is good The home is well managed and the people living at the home benefit from the staffs experience and support with daily living. The management of health and safety of the premises and risk management is of a good standard. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Field House, Minehead DS0000060745.V366925.R01.S.doc Version 5.2 Page 25 The registered provider/manager is Dawn Paxton. It was previously reported that Dawn has worked with service users with a learning disability since 1994. Dawn has previously been registered with the NCSC in Surrey as a manager of a care home for younger adults with a learning disability. Dawn has a City & Guilds certificate in ‘Advanced management for care’ and certificates in ‘epilepsy and the administration of rectal diazepam’ and ‘Nonphysical & physical intervention’. A tour of the premises was made and no health and safety matters were identified for attention. The home was very clean, tidy and well maintained. The maintenance records were sampled. These demonstrated that regular checks are made on the fire protection systems at the home and the system is serviced every six months. The fire safety officer made a visit in May 2007. Servicing records were seen for the whole system, fire extinguishers, and emergency lighting. A fire warden has been determined and this person carries out the routine checks on the premises. Evacuation is also practised to ensure the people living at the home are aware of what to do in the event of the alarm sounding. Gas safety had been checked in June 2007 and the electrical checks have been carried out, Minor works in August 2007 and the installation in May 2004. Hot water checks are made on a regular weekly basis to ensure that the water is at a safe delivery temperature and the risk of accidental scalding is minimised at bath and shower outlets. The inspector was informed that the storage of hot water is regulated at the recommended temperature to reduce the risk of Legionella. All records seen at this inspection were up to date, well maintained and stored in accordance with the Data Protection Act 1998.
