This inspection was carried out on 22nd August 2005.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home is light, clean, homely and well maintained. It was evident that a good rapport exists between the residents and staff and residents spoke warmly of the staff and the care they receive.

What has improved since the last inspection?

The home continues with a redecoration and refurbishment programme, with bedrooms being decorated as they fall vacant. The shower room is due to be refurbished next year and other equipment is replaced when necessary.

What the care home could do better:

There is a continuing requirement for the recruitment policy to be followed for every applicant. The home does regular fire training but the plan for evacuation of the home in the event of a fire requires review and updating, advice should be sought from a fire safety officer. Open, unsealed, packets offood were found in the kitchen. Food out of the original packaging must be kept in suitable sealable containers that are labelled and dated.

Up to 4 service users may be accommodated in the category PD(E) 2. Up to one named service user under the age of 65 years (date of birth 27/10/1943) with dementia may be accommodated. Date of last inspection 10.02.05 Brief Description of the Service: Forest Brow is a service that provides accommodation for up to 30 service users, or residents as they prefer to be called, within the category of old age. The home also provides a service for older people with dementia. The home is a large house that has been extended to provide accomodation over three floors, which are accessed by passenger lift. There are twentyfour bedrooms, single and double, and some have ensuite facilities. There are two sitting rooms which are light and airy. The home has a one and a half acre garden which is accessbile to the residents. The home is located in a semi rural area in the small village of Liss, between Petersfield and Liphook, and is within access of local shops and larger towns in the area. This was the first of two unannounced inspections for the inspection year 2005/06. The focus of the inspection was to talk to residents about their life in the home and how their needs are met. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Standard 6 does not apply to this service. Arrangements for assessments are in place to ensure that the home can meet the needs of potential residents. EVIDENCE: Four assessments were looked at and were seen to include relevant information, for example, health issues, social interests, religious preferences and details of the persons mental state. The registered manager undertakes the assessments. The inspector and the manager discussed the need to remove funding and payment details from the care plans, this type of information should be regarded as confidential and stored separately. The home does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 8 and 10 Arrangements in the home ensure that the residents’ healthcare needs are met. Arrangements for health and personal care promote the well-being of residents and upholds their dignity. EVIDENCE: The residents General Practitioner (GP) contact details were seen, as were the details of other medical practitioners such as the optician, chiropodist and district nurse. Residents spoken to said they felt confident that the staff would recognise their health needs. They also said that their dignity and privacy was always maintained and staff were always respectful to them, this was also apparent when observing the manner of the staff with the residents, they spoke gently and gained the residents attention before talking to them or touching them. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 15 The range of activities offered by the home promotes the residents mental and physical well-being. The arrangements for meals ensure residents have a nourishing and balanced diet and are able to choose where to eat. EVIDENCE: The home has a range of social activities, which includes music and movement, arts and crafts and reminiscence sessions, which several residents said they enjoyed. Some residents go out to church with their families and a lay preacher visits the home. The manager and the cook have undertaken a ‘Healthy eating’ course. The daily menu is displayed in the hall and choices are available. Residents can choose where to eat their meals, such as breakfast in their bedrooms. Menus are varied and nutritious and the cook is aware of the likes and dislikes of the residents. Residents said that they were happy with their meals and generally liked the food that was served. JUDGEMENT – we looked at outcomes for standard(s) 18 Arrangements for the training of staff in adult protection protect the service users. EVIDENCE: The induction training for all new staff contains training in adult protection. The manager plans to undertake a ‘train the trainers’ course in adult protection so that the staff training can be expanded. The home has policies in place that provide guidance on dealing with aggression and violence. Risk assessments are undertaken and included in care plans if required. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 Arrangements for cleaning, decorating and maintaining the home ensure residents live in a safe and comfortable environment. EVIDENCE: The home was clean, bright and homely on the day of inspection. There is an on-going decoration programme, which ensures the standard is maintained. Each bedroom is redecorated and refurbished when they become vacant. The home employs domestic staff and gloves and aprons are worn when appropriate. Staff have recently completed a training course in infection control. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29 and 30 Arrangements for staff recruitment do not always protect the residents. Arrangements for staff induction and training enables staff to meet the needs of the residents. EVIDENCE: The home has a recruitment policy, but do not always use it properly. Several staff files were looked at and it was noted that application forms were not always completed, references had been taken from friends and several Criminal Records Bureau (CRB) disclosures were outstanding. This was discussed with the manager, as it has been a requirement for the last three inspections that the registered manager must obtain all the information required in Schedule 2 of the Care Homes Regulations in relation to staff employed at the home. All staff have induction and foundation training and hold their own training records. The home has a training plan and the next three sessions will cover health and safety, fire safety and first aid. Training in infection control has recently been completed. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38 Safety checks are completed on a regular basis to promote the safety of residents and staff. EVIDENCE: An environmental health officer visited the home in February 2005; no requirements were made following this visit. Regular testing of fire equipment is carried out and fire-fighting equipment was recently serviced. Staff have received fire training. The plan for evacuation of the home in the event of a fire requires review and updating. Current good practice suggests horizontal and vertical evacuation within the home to places of safety, rather than a general evacuation of the home in the event of a fire; advice should be sought from a fire safety officer.
