This inspection was carried out on 16th February 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home offers a relaxed and friendly atmosphere. Service users with a range of care needs are well supported and encouraged to be independent with appropriate paperwork being completed. The home has a very organised system for all paperwork, which makes any records needed easily accessible. The home has good communication with professionals and visitors to the home.

What has improved since the last inspection?

The home tries to improve the range of social activities to meet service users needs. One carer now calls in at the weekend as well as weekdays to provide social activities. Temazepam is now being stored as a controlled medication.

What the care home could do better:

The home continues to work hard to maintain the standards of care and organisation in the home. Efforts need to continue to ensure this standard continues both in the care provided and for the physical environment.

The home is just less than four miles from the centre of Totton and close to the New Forest.The home is registered to accommodate twenty-two older people, including service users with an aged related mental health problem. Forest Edge has twenty-two single bedrooms, all provided with an en suite toilet. This unannounced inspection lasted three hours during which time staff, visitors and service users were spoken to. The manager of the home, Mrs parker assisted with the inspection. Records, which were easily available, were looked at during the inspection. The core standards not inspected during the last inspection were inspected on this visit, to ensure all core standards were inspected over the two visits. Eleven service user cards were received, all eleven felt they were well cared for and their privacy was respected. Ten service users had stated they liked living in the home one had stated sometimes they like living in the home. Nine service users had stated they liked the food and two had stated sometimes they liked the food. Seven relative/visitor comment cards were received, six gave very positive feedback. This included statements “ very satisfied with every aspect of care”, “liaised well”, “very well run and friendly home” and “completely satisfied”. One relative/comment card was concerned about ramps and handrails being clearly marked, risk assessments on the building and the procedures for dealing with emergencies. These were passed onto the manager who confirmed a risk assessment on the building has been completed, the home has a procedure for dealing with emergencies and steps and ramps are clearly identified. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 Medication procedures in the home are adequate to ensure the safety of service user. EVIDENCE: The home has a medication procedure, which details the receipt, recording, storage, handling, administration and disposal of medicines. All medication is checked and recorded when it enters the home. Medication is stored in a locked filing cabinet, in the office. Controlled medication is stored in a locked box, which is pad-locked inside the locked filing cabinet. Only members of staff who have received training and deemed competent are involved in the medication procedure of the home. These staff had recently received training from the primary health care trust. The medication and records of numerous service users were checked and it was found there was only one error. Medication for one service user had been administered in the morning but the record had not been signed. Medical notes are maintained for each service user; next to each service users medication is a photograph of the service user. It was agreed risk assessments would be completed where necessary. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14, 15 Choice is promoted in the home for service users to live as independently as possible. The meals in the home are of a good quality with a choice always being available. EVIDENCE: It was clear from observations and discussions with staff and service users the home tries hard to promote choice and independence for service users. The new activities co-ordinator offers a range of activities to meet service users choice and service users were pleased they were not forced to take part in activities they did not enjoy. Service users where appropriate are involved in assessments and care plans. Service users are involved in reviews and evidence was seen where a service user had detailed the concerns she had and an action plan had been agreed to meet and action these concerns. The home has a planned menu, which service users have an input into. A list of service users likes and dislikes is detailed in the kitchen. A choice is always available with a meat dish and a vegetarian dish available. Service users spoken to confirmed the food in the home is of good quality and a varied menu is available. Nine comment cards received stated service users liked the food, with two service users stating they liked the food sometimes. The kitchen was well stocked with a good selection of fresh fruit and vegetables available. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 The home has relevant documentation and staff have received training to ensure service users are protected from abuse EVIDENCE: Information relating to adult protection, Hampshire abuse procedure, the Department Of Health guidance “No Secrets” and a whistle blowing procedure are available in the home. Staff receive training on abuse during their induction period and the home has it’s own information on abuse, which staff receive training on. The file contained a lot of useful information, which was available to staff. The manager reported staff meetings are sometimes used as a venue to discuss the topic of abuse. The home has no aggressive service users at the present time, but a policy on dealing with aggressive behaviour is available. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29,30 A mix of well-trained and experienced staff offers service users with a consistent level of care. Training is promoted in the home ensuring staff have the necessary skills and knowledge to meet service users needs. EVIDENCE: The Manager explained the home has a very clear recruitment procedure, which involves all staff being interviewed, completing an application form, two written references being obtained and a criminal records bureau (CRB) check being received. The inspector looked at the records of two members of staff, which supported this, all documentation was available. All staff undergoes an induction period within the first month of employment, which includes the needs of the service user, principles of care and safe working practices. Both the carer and the manager sign the record when each stage of the induction has been completed. All care workers receive training on an annual basis on manual handling, food hygiene, health and safety, fire and first aid. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33,35,38 The home is well managed and run in the best interests of service users. The financial arrangements protect service users from abuse. The home takes adequate steps to ensure the home and the equipment in the home are safe for the protection of service users and staff members. EVIDENCE: The manager was unsure if the home had an annual development plan due to the new ownership of the home; however a list is made of all work needed and given to the proprietors. The home does consult service users, visitors and professionals to ask their views on the home, The home does not manage any service users finances, but does oversee the personal allowance of the majority of service users. These records are well Forest Edge Rest Home DS0000066132.V281968.R01.S.doc Version 5.1 Page 16 maintained with individual records, totals, and receipts maintained. All finances are kept secure. The home has a policy and procedures file in the home, which is accessible to all staff. A training record is maintained and training is routinely undertaken. The kitchen was clean and tidy with plenty of space for the preparation of food. Food was being stored appropriately. All cleaning materials in the home are kept locked away and cosh assessments have been completed. The fire logbook was seen, which demonstrated all the necessary tests were being carried out within the agreed timescales. Certificates were seen to demonstrate the fire alarm, emergency lighting, fire extinguishers, lift, gas, electricity and portable electrical appliances and all been inspected recently. The report on the electricity stated some circuit’s required urgent investigation, but there was no further information on the report. It was agreed the manager would seek further clarification and keep the Commission informed. The fridge and freezer temperatures are not currently being recorded and a probe thermometer is not being used to test the temperature of the meat. Mrs Parker had a new book, which has been provided by the New Forest District Council, which gives a daily print out to record all this information. Mrs Parker confirmed all staff are going to be given training and this will be introduced at the beginning of March.
