This inspection was carried out on 13th January 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The staff are supported by senior members of staff to provide a continuing high standard of care in the home. Residents spoken to by the inspector commented that were happy and well cared for and that they were kept informed about any changes or events that may be taking place. They said that staff are supportive and that they provided assistance when needed. Staff told the inspector that the home was a pleasant working environment and that they were supported by their colleagues to provide and maintain a high standard of care at all times. The staff also commented on the opportunities that were provided for training and that they had access to the equipment essential to do their job.

What has improved since the last inspection?

The deputy manager is undertaking regular audits of procedures and equipment used in the nursing wing of the home.

What the care home could do better:

Staff recruitment files did not provide evidence that essential and appropriate police checks had been undertaken. Written references were not available for some members of staff together with checks of professional identification. The home does not, at the time of the inspection, have the minimum of 50% of their care staff with NVQ level 2.

No more than 16 beds may be used at any one time for service users who are in need of nursing care. 9th September 2005 Date of last inspection Brief Description of the Service: Forest Oaks is a care home providing personal care and accommodation for thirty service users and sixteen service users in need of nursing care. The home is registered to accommodate a total of 46 service users in the old age category of over 65 years. Forest Oaks is one of two care homes that is administered by the Wilverley Association Ltd, which is a registered charity and as such all the members of the governing committee are volunteers. The home is purpose build some years ago and is housed on three floors and is surrounded by pleasant well-maintained gardens. The home is divided into two distinctive areas. All residential rooms have en-suite facilities and are accommodated on all three floors of one wing of the home. The sixteen service users in need of nursing care are accommodated on the ground floor in single rooms. Forest Oaks is situated in the New Forest Village of Brockenhurst and is close to the village amenities that are widely used by the residential service users. This was the second statutory unannounced inspection for 2005/2006. The inspection took place on 13th January 2006 between 09.15 and 12.30. The registered manager for the home was on annual leave. The deputy manager was present at the time of the inspection. Records for the residents and staff were examined. The inspector spoke to both residents and members of staff. A tour of the building was made. Two requirements have been at this inspection and are addressed in the main body of the report. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 An on-going programme of redecoration and refurbishment ensures that the residents in the home are living in safe, comfortable and well-maintained surroundings. EVIDENCE: On the day of the inspection all areas of the home were found to be clean and tidy. A programme of regular redecoration and repair ensures the home is maintained to a high standard. People living in the residential wing of the home are responsible for providing their own furnishings and decoration of their rooms. The rooms in the nursing wing of the home are furnished by the charity that owns the home. The inspector noted that many of the residents in this wing, who were previously living in the residential wing of the home, have brought many of their personal possessions with them. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 and 29 The staff undertakes training that enables them to be competent to do their jobs but ensuring that all care staff have additional training and knowledge will maintain the standards of care in the home. Evidence that appropriate checks have been undertaken to ensure the safety and well being of the service users were not available. EVIDENCE: It was reported to the inspector that at this time, only five care staff working in the home has achieved NVQ at level 2. This is less than 50 of the total number of staff. Three further members of staff are currently undertaking the training. It was reported to the inspector that the training co-ordinator is reviewing the NVQ training for staff and will be ensuring that additional members of staff are put forward for this training. The home has a varied training programme, which ensures that all staff receives regular training, including fire safety, safe manual handling, infection control and first aid. The inspector examined four staff recruitment files. These were all found to be incomplete. None of the files examined contained evidence that checks had been undertaken with the Criminal Records Bureau (CRB) or the Protection of Vulnerable Adults register (POVA). The deputy manager reported that she was Forest Oaks Home For The Elderly DS0000011426.V278118.R01.S.doc Version 5.1 Page 14 sure these checks had been done but evidence of this was not available to the inspector at the time of the inspection. Records for two trained members of staff did not contain evidence that their PIN with the Nursing and Midwifery Council was current. Neither of these files contained written references for the member of staff. Two files did not contain evidence of personal identification. Application forms had been completed for all the members of staff and a record of an interview with the staff was available in two of the files. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33,35 and 38 The home actively seeks the views of residents and makes decisions on information received. Procedures are in place to ensure that residents’ financial interests are safeguarded. All safety checks are completed on a regular basis to promote the safety of residents and staff. EVIDENCE: The deputy manager is regularly auditing many of the care systems used in the nursing wing of the home, such as: Care plans Medication records Nutritional assessments Pressure relieving equipment. The Commission has not been made aware of the outcome of these surveys. The inspector spoke to residents in both the nursing and residential wings of the home. Comments about the services provided in the home were generally positive. Some negative comments were made, these were generally about the meals provided in the home. The comments varied from the lack of choice, temperature of the food when served and the size of the portions, too large or too small. The home is responsible for safe keeping of monies for residents in the nursing wing of the home. All sums of money are stored securely and individually. Records of transaction are kept in individual notebooks together with receipts for any purchases made on behalf of the resident. People living in the residential wing of the home are responsible for their own financial transactions and safe keeping of their money. A full risk assessment for the building has recently been undertaken. All staff in the home receive regular training for fire safety and safe moving and handling. The catering staff have regular updates for food handling and hygiene. Records demonstrated that all equipment and systems used in the home are regularly maintained and repaired. Fire safety records confirmed that staff do attend regular training.
