Conditions of registration: Date of last inspection 23rd February 2006 Brief Description of the Service: Goodwins Care Home is a dormer bungalow registered to provide the facilities and resources for the care of two older people. There are two large single bedrooms at the front of the dormer bungalow that provide accommodation for the service users and also one bedroom occupied by the registered manager/owner. There are two communal lounges and a dining room/kitchen. The bungalow is situated close to Wakefield City centre and all the local amenities. The weekly charges for the service in February 2007 are £359. This is an overview of what the inspector found during the inspection. This was a key visit, which formed part of the overall inspection of the home and was completed on the 8th February 2007 extending over 3.5 hours. Information in respect of the service was obtained through talks with the service users, checking information provided on comment cards received by the Commission from service users, relatives, a GP and a social worker, talking with the manager who is also the registered person and her husband. Some records were checked and a walk round the areas in the home accessed by service users completed. What the service does well: What has improved since the last inspection? The banking arrangements for a service user’s personal funds were not satisfactory and a requirement has been made in respect of this. It has also been recommended that individual receipts for purchases are obtained for future purchases made. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3,6 Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. The assessment and admission arrangements are satisfactory. Intermediate care is not provided at the home. EVIDENCE: Records seen showed that a social care assessment had been completed prior to the service user being admitted to the home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users and their relatives are satisfied with the overall care provided at the home. The records seen and observations made during the inspection confirm service users needs are identified and action is taken to meet them. Goodwins Residential Care Home DS0000006256.V323620.R01.S.doc Version 5.2 Page 10 EVIDENCE: Care plans seen included service users’ identified needs, which had been reviewed regularly and changes in need recorded. The comment cards completed by the service users and their relatives showed that they are satisfied with the care provided. Records of daily events completed by the manager showed that the GP and other health professionals had been contacted when needed. Care management reviews had been held after a new service user’s admission to the home. Nutritional assessments had been completed and service users’ weights entered in the daily events diary. The medication records were seen, no signature omissions had occurred. It was the last day of the medication monthly cycle; one tablet was in stock which balanced with the records. Medications were stored securely and records of medication reviews were recorded. No incidences were seen that could have compromised service users’ privacy and dignity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. Service users have maintained links with relatives and friends. They are included in family events and are encouraged to exercise choice in all activities of daily living. EVIDENCE: The record of daily events seen showed that service users are included in family outings and events. The manger said service users had been taken to a restaurant for lunch at Christmas and some relatives had also been in the same restaurant. Service users said they go with the manager on short walks, to visit the supermarket and have lunch at the pub on occasions. A record seen showed a visitor had joined their relative for lunch. Relatives had made positive comments on the comment cards received by the Commission. Service users said they enjoy watching TV and a wide screen TV had been provided for them.

The service users said they have visitors who can visit them in their bedrooms, they go out with the manager and are included outings with the manager’s family. Service users said they can have visitors and privacy is given to their visits if requested. A record of meals provided was seen, a service user’s diet was discussed with manager who said the diet and service user’s weight would be monitored. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users know how the make a complaint if needed. The safeguarding training currently being provided and the homes recruitment process will ensure service users are provided. EVIDENCE: The homes complaints book was seen and no complaints were recorded. Service users spoken with and information provided on two comment cards confirmed that they had not made any complaints. The complaints procedure is included in the copy of the copy of the service user guide available in their bedrooms. The manager said they take the service users to vote at the polling stations if they want to vote at local and general elections. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19,26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home provides a very pleasant homely environment for service users to live in. EVIDENCE: The communal lounges used by the service users were clean, comfortable and decorated and furnished to a high standard. Service users bedrooms had been redecorated to a high standard and were personalised. No incidences of cross infection were observed at the visit. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The arrangements for ensuring the home is appropriately staffed are satisfactory. The recruitment procedure ensures service users are protected. EVIDENCE: A record of the homes staffing arrangements to the time of the visit was seen and was satisfactory. Three part time staff are employed to cover the times the manager is off duty. The manager is on call at night. The manager has an NVQ qualification. Records seen showed that some staff training updates are needed. This was discussed with the providers who said they would arrange this. A new member of staff had been recruited since the last visit and according to the records, a CRB check and two satisfactory references had been obtained prior to them commencing work. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31 35,38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The current banking arrangements for money to be used for service users’ personal purchases are not satisfactory. EVIDENCE: The manager who is also the registered person has the NVQ level 2 care qualification. Discussions with service users and the daily events records showed the manager consults with them on a daily basis about all aspects of their care and the events occurring in the home.
Goodwins Residential Care Home DS0000006256.V323620.R01.S.doc Version 5.2 Page 18 A service user’s records were checked, the records showed a reducing balance and receipts for purchases made by the manager on their behalf were available and those checked could be cross referenced to the entries made in the records. No discrepancies were observed. However, individual receipts had not been obtained and the current banking arrangements do not meet regulatory requirements. A co owner said he is a trained Fire Marshall at his works he trains staff and arranges fire drills at the home. Records of smoke detector checks, standing and running hot water temperature checks were seen. The report of a food hygiene inspection was seen and no risks had been identified. STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard OP35 Regulation 20(1)(a) Requirement The registered person shall not pay money belonging to any service user into a bank account unless the account is in the name of the service user, or any of the service users, to which the money belongs.
