Conditions of registration: 1. That service user accommodation shall be restricted to the ground floor and first floor of the Gwenlyn House only. Date of last inspection 17/01/05 Brief Description of the Service: Gwenlyn House is a Care Home for Younger Adults and is registered to accommodate 3 persons of either sex, whom have mental health problems. The home is situated in the North Shore area of Blackpool, within easy reach of local amenities. The residents each have a single bedroom and there are sufficient bathing and toilet facilties to meet their needs. The communal areas comprise of a large lounge and dining room in which residents can undertaking the daily activites of their choice. The home does not have any special equiptment or aids as the residents are independent and require no assistance. This is an overview of what the inspector found during the inspection. This was an announced inspection that took place over 1 ½ hours. The Inspector spoke to the homeowner and the 3 residents. The Inspector also spoke to the homeowner’s daughter who was present at that time and offers her mother support when needed. Conversation with the residents was very limited due to the fact that they consider the inspection to be in invasion of their privacy. The residents presently accommodated have lived together in the home, which operates as an extended family unit, for approximately 21years. The home is very much domestic in nature and the residents are encouraged to be as independent as their abilities allow. In the event of any of the existing residents no longer residing in the home, the homeowner has no intention of offering accommodation to any other persons in the future. The Inspector examined the records kept for each resident and looked around the home. What the service does well:
The residents in this home are well cared for, protected and encouraged to be independent within their capabilities and wishes. The home is run as a small family unit and the residents clearly see themselves as part of this family. Members of the homeowner’s family give assistance and support when required and the holidays that residents take are with these family members. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 5 Each resident has a copy of the terms and conditions of residence, which confirms the service provided by the home to meet their needs. EVIDENCE: The residents chose to live in the home over 21 years ago, which was before there was any requirement for the home to be registered and therefore there was no assessment and admission procedures carried out at that time. The Inspector noted that residents had a signed copy of the home’s terms and conditions on their file wich is important as this ensures they are then aware of their rights. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 8, 9 and 10. There is a clear and consistent care planning system in place, which ensures that the resident’s needs, aspirations and goals are met. EVIDENCE: The residents are fully involved in every aspect that affects their lives. The Inspector examined the care plans that had been developed for each of them and was able to evidence their involvement and that they had signed to confirm that they agreed and understood the contents. The information covers all aspects of their individual needs, risk assessments, physical, mental, emotional, spiritual and social needs. The information is reviewed by the homeowner as necessary and at least every 6 months. Information that is recorded on each individual resident is kept secure, and shared only on a need to know basis with appropriate persons. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15, 16 and 17. The support provided to residents enables them to maintain their choice of lifestyles in and outside the home. The meals provided offer a balanced and varied diet to meet individual tastes and choices. EVIDENCE: The residents have a range of activities available to them, which they enjoy. These include a variety of board games and at the time of the inspection they were playing dominoes. They each have a television in their room in addition to the television in the lounge. Information that they have written in their daily activities record confirms that they enjoy music, reading, the cinema and swimming. The Inspector discussed these records with the residents and found that one of the residents was assisted by the others to record their activities. On arrival at the home the Inspector was met by one of the residents that had been out for a newspaper. Although the residents may go out alone to the local shops, in the main they choice to go out as a group and support each other. The residents do not have any family links or external friendships and are very dependent on the homeowner. The involvement of a social worker in the reassessment of the residents needs has introduced another person into their lives, however the residents suspect any involvement by outside agencies and will take time to build up their trust, if at all. The Inspector is well aware of their feelings and conducts the Inspection as unintrusive as possible. Conversation with the residents is dependent on the degree that they wish to communicate and on what subjects. Through comments made and observations during the inspection there was no evidence to indicate any cause for concern. The residents and homeowner have their meals together and over the years have become accustomed to the types of meals preferred. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19, 20 and 21 Promotion of resident’s health, personal and social care is taken seriously and closely monitored to ensure they are met. EVIDENCE: The residents in this home do not require any assistance with their personal care needs. They are very independent and require only gentle reminders in relation to personal hygiene. On examination of the residents’ individual files the Inspector found that a record was kept of every time the resident was seen by their GP and all appointments with other health care professions. Two of the residents are on medication and self-administer this. The homeowner maintains a record of the medication prescribed and any changes made. The Inspector noted that the residents had clearly indicated on their individual file, their wishes in relation to ageing, illness and death. The homeowner had obtained this information in a sensitive and respectful manner.

Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25and 30 The home is redecorated, refurbished and well maintained to provide the residents with a safe, comfortable and homely environment. EVIDENCE: Observations made during the inspection confirmed that the home was meeting the needs of the residents. The home is very much domestic in character and in keeping with the local community. The Inspector found the home to be warm, clean and free from any offensive odours and the homeowner ensures that standards remain high by undertaking a continual programme of upgrading and refurbishment, to which the residents are involved. A recent inspection by the Fire Department had identified changes that were required to the fire alarm system. The Inspector was informed that this work had been carried out and that the Fire Department had been informed. JUDGEMENT – we looked at outcomes for standard(s) 37, 39 and 42 The home is well managed and run in the best interest of the residents. EVIDENCE: The provider is a competent person who has provided the residents with a home and the support that they require for the past 21 years. The Inspector was able to evidence through discussions, examination of records and observations made, that the residents are able to make their views known and are involved in all decisions that affect them. As there are no staff employed in the home the requirement of a management qualification is not seen to be appropriate, however the provider was reminded of the need to undertake relevant training to ensure that the needs of the residents continue to be met. This includes obtaining a Level 4 NVQ and a qualification in First Aid.
