What the care home does well The residents are well cared for, protected and encouraged to be independent within their capabilities and wishes. They are involved in all aspects of their lifestyle, which ensures that their needs continue to be met. The residents have lived as part of the family for over 23 years and clearly see themselves as part of the family unit and the owner as a `mother figure`. One resident spoken to said, "This is my home and family". The owner said, 2They both gave me mothers day cards and presents we are part of one family". Holidays are taken altogether and regular visits to other members of the family who live in the south of England are often taken throughout the year. "We enjoy the family holidays", said one resident spoken to. What has improved since the last inspection? To ensure the residents are kept safe, the owner has had the home rewired, a new fire system installed, electrical and gas appliances checked. We looked at the certificates to evidence these safety checks have been completed. The owner said, "We have had everything checked as recommended". The home is situated in the North Shore area of Blackpool, within easy reach of local amenities. The residents each have a single bedroom and there are sufficient bathing and toilet facilities to meet their needs. There is a large lounge and dining room in which residents can undertake daily activities of their choice. The home does not have any special equipment or aids as the residents are independent and currently require no assistance with personal care. The home has a Statement of Purpose, which is kept in the lounge for the residents to read if they wish and they each have a copy of the Service User Guide. The residents moved into the home before there was any requirement for the home to be registered and any written information to be given to them, however the documentation they now have confirms their rights, the service provided and the level of care that they can expect to receive if required. Information received at the time of the visit from the homeowner showed that the fees were £284.00 per week. The residents receive their full personal allowance to spend as they, whilst the homeowner pays for all their requirements including, holidays, their choice of clothing and toiletries etc. Gwenlyn Guest House DS0000010016.V374815.R01.S.doc Version 5.2 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. The quality rating for this service is 2 stars. This means the people who use this service experience good quality outcomes.
This unannounced visit was part of the key inspection process and took place on the 7th April 2009 and lasted approximately 3.0 hours. We spoke to the two residents who live here and the owner. There are no staff employed. Resident’s care, maintenance and financial records were looked at during the visit and we had a walk around the premises with the owner. As part of the inspection process we talked to the residents, had a look around the rooms examined care records and daily notes this is called case tracking. Every year the person in charge or manager is asked to provide us with written information about the quality of the service they provide, and to make an assessment of the quality of their service. This document is called ‘Annual Quality Assurance Assessment’ (AQAA). We use this information, in part, to focus our inspection activity. The residents have lived at the home for over 20 years. There is an assessment procedure in place should the owner wish to accommodate future residents. EVIDENCE: The residents chose to live in the home over 20 years ago, since the home has been registered social work assessments were carried out to ensure there needs could be met. We spoke to the owner who is aware of the assessment procedure if another person chooses to live at Gwenlyn House. We looked at assessment information on one of the residents, which had details for the owner to develop a care plan. We spoke to a resident who said, “I have been here for over 20 years, and was glad Social Services said I could stay here”. This is what people staying in this care home experience: JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9. People using the service experience good quality outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. Residents are helped to make decisions, which support them in their day-today lives, whilst taking risks into account. EVIDENCE: The two residents are part of the family unit and are cared for by the owner and her family. A care plan has been developed for each resident, which they have been involved in and signed to indicate that they agree with the contents. The information covers all parts of their individual needs, risk assessments, physical, mental, emotional, spiritual and social needs. The information is reviewed by the owner and signed by the resident as necessary and each year their care plan is reviewed by their social worker to ensure that the home continues to meet their needs. Records clearly described the level of support and assistance provided to one resident with his decision-making to ensure his choices are his own. The owner tells us in the AQAA how she sometimes provides guidance and support when needed. One resident said, “She is always there to help me”. Another said, “I go out when I feel like it”. Records examined confirmed risk assessments are completed, reviewed and updated to ensure resident’s independence and living skills are developed. JUDGEMENT – we looked at outcomes for the following standard(s): This is what people staying in this care home experience: 12,13,15,16 and 17. People using the service experience good quality outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. Opportunities for development and community participation are good to ensure residents have opportunities for personal development. EVIDENCE: Residents have a range of activities available to them, which included television, music, videos and board games. At the time of the visit a friend had called to see them. The residents continue to live as part of the family unit and call the owner affectionately ‘mum’. They go away on family outings and holidays and later this year will be going to Turkey, one resident spoken to said, “Yes I am looking forward to going on holiday”. Another said, “I have never been on a plane before”.

DS0000010016.V374815.R01.S.doc Version 5.2 Page 12 Both residents said they were provided with the choice of spending time on their own or in the lounge areas and the owner respected their privacy. The owner said, “We go out for meals together and shopping but they please themselves”. The resident’s respect each others individual space and do not go into each other’s room unless invited. This was confirmed through observations and their permission was asked before we had a walk around the home. As of previous inspections the residents do not have any family links or external friendships and are very much dependent on the homeowner and her family. The annual involvement of a social worker in the review of their individual needs has introduced another person into their lives, providing an overview of their care and current needs. The residents and the owner have their meals together and although none of the residents has any specific dietary needs, their preferences, which have changed over the years, are taken into consideration when meals are being prepared we noticed fresh fruit and vegetables in the kitchen area. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. Residents health care is taken seriously and needs are closely monitored ensuring health issues are met. EVIDENCE: The residents in this home do not currently require any assistance with their personal care needs. They are independent and require only gentle reminders in relation to their personal hygiene. There is evidence in records kept they have good access to specialist healthcare services for the benefit of residents, so that their healthcare needs are met. Looking at the residents’ individual files showed that a record was kept of every time a doctor and other health care professionals saw a resident. The residents take daily medication and they collect their own prescriptions from the doctors and chemist and take responsibility for taking their medication. The owner ensures that the residents are aware of their rights and have a complaints procedure. EVIDENCE: The home has a detailed complaints procedure, which has been made available to the residents. One residents spoken to said, “I have been here twenty years and never had to complain”. There have been no complaints since the previous inspection. There is a procedure in place for dealing with safeguarding adults so that people know what they should do should an allegation or suspicion of abuse be noted. Through discussion with the owner it was evident every effort is made to protect the residents from abuse, neglect and self-harm. The residents live in a comfortable, homely and safe environment. EVIDENCE: The home is kept clean and tidy by the owner and family and well maintained to ensure the residents live in comfortable surroundings. The building is very much furnished and decorated as a large family home and is domestic in character. One resident spoken to said, “It’s a lovely home”. We had a walk around the home and found they have had a new bathroom installed and are finishing off the decoration. The owner said, “The new bathroom is now fitted”. There is a continual programme of upgrading and refurbishment carried out to ensure that standards remain high and the residents confirmed that they are

The home is run in the best interests of the residents, EVIDENCE: The owner has provided the residents with a home and the support they require for over 20 years. The owner’s is relaxed so that there is no formality in the day-to-day running of the home. Residents are encouraged to follow their individual daily routines as part of the family and helped by the owner, which we observed throughout the day and was seen to meet the individual needs of the residents.
