Conditions of registration: Date of last inspection 7th June 2005 Brief Description of the Service: Hawthawn Court is a purpose built nursing home for 80 older persons managed by Hampshire County Council Social Services Department. It was registered 24 February 2005 and commenced admitting residents on 5 March 2005. The home is located within the former grounds of Coldeast hospital Fareham. There are ghood good parking facilities and a large enclosed garden. Accommodation is provided on two floors serviced by two passenger lifts. There are 8 units each comprising of a lounge/dining room, kitchenette and ten single bedrooms with en-suite toilet facilites. Other facilities include a visitors lounge, activities lounge, hairdressing salon, smoking lounge and medical rooms. This is an overview of what the inspector found during the inspection. This report summarises the assessment of the extent to which the National Minimum Standards for Care Homes for Older People were being met at the time of the inspection. This visit was the home’s second insepction subsequent to registration February 2005. The inspection took place over one day, a total of four and a quarter hours between 10-15 a.m. and 2-30 p.m. Opportunity was taken to look around the home, view records and talk with, the registered manager 7 residents 6 care staff and two administration staff. Mrs Biggs, the Registered Manager informed the inspector that staffing level requirements have been revised to enable admissions to increase to sixty by the end of November. It is planned to reach full capacity early part of 2006. At the time of this inspection, forty-eight residents were accommodated. Standards not inspected on this occasion will be assessed during future visits. Four of the five requirements raised resulting from the previous inspection of 7 June 2005 have been met. The outstanding requirement regarding the ventilation in staff working stations is also raised in the main body of this report. One new requirement was raised resulting from this inspection. What the service does well:
Residents spoken with stated staff were so kind and helpful before and after they had moved in. They and their family had opportunities to visit the home before making a decision. Positive working relationships were evident between residents and staff on duty. Residents spoken with had nothing but positive remarks regarding how well staff look after them. Identifying training needs through one to one supervision and providing it accordingly. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1. Prospective residents have the information they need to make an informed choice with regards to moving into Hawthorn Court. EVIDENCE: An informative and well-presented statement of purpose was seen. Residents spoken with stated staff were so kind and helpful before and after they had moved in. They and their family had opportunities to visit the home before making a decision. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 11. Residents’ are assured their health, personal and social care needs will be met. Residents are assured that at the time of their death they and their family will be treated with care, sensitivity and respect. EVIDENCE: Two requirements were raised resulting from the previous inspection regarding residents care plans. Information within the care plans required to be more detailed and include information where regular checks need to be made regarding frail residents unable to use the call bell system. It was assessed that they have been met. The home has an appropriate policy and procedure to assure residents at the time of their death, that they and their family will be treated with care, sensitivity and respect. It was noted that residents’ cultural/requirements/ wishes upon death are in the process of being recorded in care plans. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. JUDGEMENT – we looked at outcomes for the following standard(s): 20, 21, 25 and 26. The location and layout of the home is suitable for its stated purpose. It is accessible, safe, clean and well maintained. It meets residents’ individual and collective needs in a comfortable and homely way. EVIDENCE: Hawthorn Court opened February 2005. Living areas are divided into eight separate units each accommodating ten residents. Each unit has a lounge/ dining room. In addition there are three quiet lounges, a smoking lounge and activitiy rooms. Furniture and soft furnishings are in good order. Every bedroom has en-suit toilet and wash hand facilities appropriate to meet residents needs. There are also sufficient communal bathing and toilet facilities to meet residents needs. Bedrooms are individually and naturally ventilated with windows conforming to recognised standards. Central heating is under floor with residents having the ability to control the temperature. At the time of the visit the premises were clean, hygienic and free from offensive odours throughout. Systems are in place to control the spread of infection. Laundry facilities are sited so that soiled articles, clothing and infected linen are not carried through areas where food is stored or prepared. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33, 34, 35, 36, and 38. Residents live in a home, which is well run and managed with an ethos of being open and honest. Residents are assured that their financial interests are safeguarded. Staff benefit from regular one to one supervision. Effective quality assurance systems based on seeking the views of residents and relatives are in place. Residents benefit from an effectively and efficiently run home. EVIDENCE: Staff spoken with readily informed the inspector that the working environment has become more positive as the team has settled. This was evident during the inspection. Positive working relationships were evident between residents and staff on duty.

For example, “They are so kind” and “They always take time to talk with you.” A relatives group and a league of friends have been set up as part of the home’s audit system. Hampshire County Council have recently developed a new quality control audit system that is to be put in place by April 2005. Hampshire County Council manages accounting and financial procedures centrally. The manager and unit’s administrator maintain records of all financial transactions within the home. Discussions indicated that relatives manage residents’ financial interests. The home was however in the process of setting up an account for one resident’s finances in line with Hampshire County Council’s procedures. Discussions indicated that one to one staff supervision has been implemented on a bi-monthly basis. This was a requirement raised resulting from the previous inspection of 7 June 2005. The home’s health and safety file was available. Staff are provided with a copy and advised of it’s content as part of the induction process. No unsafe practices were observed during the inspection. Certificates were available for required checks of systems and equipment. Risk assessments where necessary have been completed. Staff have received training in first aid and moving and handling. A requirement was raised resulting from the previous inspection to improve the ventilation in the nurse station areas. The inspector was informed that Hampshire County Council have assessed the temperature levels but no work has been carried out. The situation is compounded in that doors to the nursing stations are not fitted with automatic closing devices linked to the alarm system. These doors were found to be wedged open which compromises fire safety precautions. This practice must stop.
