Patients detained under Sections of the Mental Health Act are not to be admitted to the home. 23rd November 2005 Date of last inspection Brief Description of the Service: Hazelwood is a home providing nursing care and accommodation for up to fifty older people, some of who may also have dementia. Smartmove Homes Limited owns the home. Hazelwood is purpose built and located in the village of Longfield, which is accessible by public transport. The house is a large detached property with good parking facilities to the front, and a garden to the rear. The home is divided into two units, ‘Hazel’ and ‘Wood’. On the ground floor twenty people requiring general nursing care can be accommodated, whilst on the first floor, there are facilities for thirty people who require nursing and dementia care. All bedrooms are connected to the call bell system. Hazelwood employs nursing and care staff, working a roster, which gives 24hour cover. Ancillary staff cover all housekeeping, laundry, catering, administration and maintenance duties. The cost of the service ranges from £475.00 to £769.00 per week. This is an overview of what the inspector found during the inspection. This report of the Key inspection of Hazelwood Nursing Home has been compiled using information gained during a site visit that lasted a little over seven hours, discussion with the registered manager, residents, relatives and staff, information supplied on survey forms and comment cards and the pre inspection questionnaire completed by the manager. During the site visit a tour of the property took place, records and documentation were inspected, and general observation took place of staff interaction with residents and the routines of the day. Due to the nature of the service some judgements about quality of life and choices have been made from discussion with the manager and documentation. Comments made by residents during the site visit included, “ The home is very nice, food is very good, the nurses are all very nice” “ I am very happy” “ My room is very comfortable” Comments made by relatives and visitors during the site visit and on comment cards included, “ Cannot fault the home, I have never had any concerns” “I looked at several other homes and liked it straight away, would recommend it to anybody” “First rate care and attention, excellent all round” “ We are very happy with the standard of care and kindness at this home” “My (relative) always looks clean and nicely dressed when I visit” “ There are obvious difficulties in trying to engage many patients in activities and many spend a lot of time on their own” Comments made by health and social care professionals included, “ This home provides an excellent service” Hazelwood Nursing Home DS0000026177.V309842.R01.S.doc Version 5.2 Page 6 What the service does well: What has improved since the last inspection? Staff assisting residents with their meals need to be reminded that they must not feed residents whilst standing up. The recording of staff supervision needs to be more formalised, and supervision notes recorded stored in the staff files so that all the documentation is easily accessible. It is a recommendation that team meetings be held more frequently than twice a year, and that meetings be relevant to respective units and all staff at appropriate intervals. Monthly Regulation 26 visits undertaken by the provider must be recorded and be available for inspection. Please contact the provider for advice of actions taken in response to this inspection. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,3,4 and 5 The quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. Prospective residents and their relatives are able to access information about the home and visit before making a decision to take up a place. Needs are fully assessed prior to admission. EVIDENCE: The home has a statement of purpose and service user’s guide and prospective residents and their relatives are able to visit before a decision is made to move in. A resident spoken with said that their relatives had viewed the home on their behalf and had chosen it after comparison with several other services, relatives spoken with stated that they had also chosen the home in preference to others visited, and that their expectations as regards the service had been met. The manager or a senior member of the nursing staff undertake initial needs assessments and visit together if necessary. Evidence of the assessments was

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 and 11 The quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. The needs of residents are set out in their care plans and needs are regularly reviewed. Residents are treated with respect and dignity by staff. EVIDENCE: A sample of the care plans of residents on each unit were inspected. The information was up to date and included sections on personal care, in some cases a personal history, information and charts relating to health care needs, risk assessments, daily record sheets and evidence that regular review of needs takes place. The manager audits the files monthly to ensure they are correctly completed. The nature of the service is that residents can have a wide range of medical conditions and as such the home maintains good liaison with health care professionals, such as the specialist dementia nurse, tissue viability nurse and dietician. During the inspection a resident was accompanied to an out patients
Hazelwood Nursing Home DS0000026177.V309842.R01.S.doc Version 5.2 Page 12 appointment by a staff member and a GP arrived for a regular weekly visit that had been arranged with the local surgery, an arranged visit by a GP takes place to each unit weekly so that any health concerns may be raised. The home has two medication storage areas and policies and procedures are in place for the storage and administration of medication. MAR sheets had been filled in correctly and the list of staff qualified to administer medication was up to date. There were a small number of gaps in the recording of the room and drugs fridge temperatures in one of the medication rooms. Residents spoken with said that care and nursing staff treated them in a respectful and dignified manner, comments made by relatives on comment cards and in person echoed this view. Staff observed during the inspection were respectful towards service users and maintained their privacy. Wishes in the event of terminal illness and death were recorded on care plans, and specialist nurses are involved when necessary. Comments on correspondence sent to the home by relatives were positive regarding the care of residents who had now passed away. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15 The quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. Residents are able to exercise choice over their daily lives and contact with relatives and friends is welcomed. The full range of activities normally available is to be reinstated with the appointment of a new activities coordinator. Meals are well cooked and nutritious, with choice available EVIDENCE: Comments made by residents and relatives spoken with during the inspection and on comment cards presented the view that the home met their expectations. Some comment cards referred to a lack of activities, the manager said that the activities co-ordinator had left several weeks previously and evidenced that a new co-ordinator was imminently to be appointed, it is hoped that a second co-ordinator can be recruited at some stage. In the meantime staff were providing some activities. A resident said they especially liked bingo. A hairdresser visits weekly, there are monthly visits from a vicar and live music is provided monthly. Contact with relatives and friends is supported and relatives spoken with said that are always made welcome, can visit at any time and are contacted in between visits if there are any concerns. A service user spoke of a relative bringing in their dog to visit, and said that staff respect privacy when visitors are present. Residents are able to exercise choice over their daily lives, for example in terms of time to get up, area to access in the home and meals. Two daily main meal choices are offered; one being a vegetarian option and special diets are catered for. The menu is varied and residents and relatives commented that the food was good. Each floor has a snack room where relatives can prepare drinks and snacks. Lunchtime on the unit for residents with dementia was partially observed, the dining room provides a pleasant environment, and residents were served and ate their meal in an unhurried fashion. The meal was well presented with appropriate sized portions. Residents are given the choice to eat in the dining room or in their rooms, dependant upon their health and mobility needs. Residents who required help with their meal were assisted appropriately, although one staff member observed was feeding standing up. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 The quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. A complaints procedure is in place and the complaints process is adhered to. Procedures are in place to protect residents from abuse and staff are vetted prior to commencing employment at the home. EVIDENCE: The home has a complaints procedure; a copy is displayed in the entrance area. Staff and relatives spoken with were aware of the procedure, residents and relatives spoken with said they had not needed to complain about anything, and felt confident in addressing any concerns to the manager. Three formal complaints had been recorded since the last inspection, two had been appropriately responded to, one had been referred to CSCI and a written response had very recently been submitted by the manager to CSCI. The home has an adult protection procedure; staff spoken with were aware of it and had received adult protection training. Recruitment practices ensure that all staff recruited are CRB and POVA checked. There had been one adult protection alert since the last inspection that had been investigated and closed under adult protection procedures. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,20,21,22,23,24,25 and 26 The quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. The home is provides a safe, clean and comfortable environment for residents to live in. Individual bedrooms are personalised and equipment is available to maintain and aid independence. EVIDENCE: The home is well maintained, well decorated, clean and comfortable. Accommodation is provided on two floors with access via stairs or a shaft lift that has a security keypad. The residential unit is on the ground floor, both units comprise of bedrooms, bath and shower rooms, a dining room and lounge areas, one lounge on the ground floor is a quiet room with no television and one in the dementia unit has been decorated as a reminiscence room, recognising that residents may relate to different decades eg, 40’s, 50’s or

There is also a sensory room with a nautical theme. Picture charts and information regarding the season, weather, and other items of interest are placed on some first floor walls to aid recognition and prompt discussion. Individual bedrooms on both floors are personalised to varying degrees, and to the taste of the resident, some included furniture brought from home and plenty of pictures and photographs. First floor bedroom doors each have pictures on of a topic of interest to the resident, for example dogs, books or flowers. Residents spoken with said that they liked their rooms and they could choose to spend time in them or in communal areas. There is a well kept garden with a patio area for use in good weather. Each floor has a snack kitchen and staff station. Aids and equipment for individual or communal use are available throughout the home. All areas were cleaned to a high standard, water temperatures are thermostatically controlled and the laundry is located away from food preparation areas. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30. The quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to the service. The needs of residents are met by a staff group who are well trained and competent in their areas of work. Recruitment procedures are thorough and serve to protect residents. EVIDENCE: The home is staff by qualified nurses, nurses qualified in other countries gaining experience at the home whilst undertaking a qualification in this country, care staff, ancillary staff, an administrator and the registered manager. There were sufficient care and nursing staff on duty to meet the needs of residents, any gaps on the rota are covered by existing staff and agency cover is not used. Two waking staff are on duty on the ground floor, and three waking staff are on duty on the upper floor at night. Staff observed presented as friendly, competent and confident in their roles, residents and relatives who commented upon staff spoke highly of them. Staff spoken with said that they had received a thorough induction and had shadowed existing staff until they felt competent to work alone. Staff are provided with training on mandatory topics and a range of topics specific to

Training is tracked on a database that highlights the need for refresher courses. The home has exceeded the ratio of 50 care staff with an NVQ in care at level 2 or above. Members of the catering and housekeeping staff have gained NVQ qualifications with the support of the home. The manager is committed to providing staff with plenty of training opportunities through courses and individual or informal sessions. Recruitment policies and procedures are thorough, a sample of staff files read included the required documentation, staff do not start work before a satisfactory CRB disclosure has been received. Recruitment procedures had been improved upon following the last inspection. Diversity within the staff group is valued and acknowledged, and awareness is promoted by information being provided in the home on religious and cultural dates and festivals throughout the year. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,35,36,37 and 38 The quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. The home is well run in the best interests of residents and staff. Increased opportunities for staff to meet together as a group would allow their views to be sought more frequently, and for more information sharing. EVIDENCE: The registered manager has worked at the home for a number of years and has previous experience of nursing and managing services. The manager is undertaking the registered manager’s award and is on schedule to complete it shortly. The home is efficiently run, whilst maintaining a friendly and welcoming atmosphere, there is good rapport between staff and residents, and the manager and nursing and care staff.
Hazelwood Nursing Home DS0000026177.V309842.R01.S.doc Version 5.2 Page 22 The manager supervises the senior nursing staff and senior staff provide supervision to nursing staff and carers. Staff spoken with said that they received regular supervision and felt well supported by the manager and that they could always approach her for advice and support. Supervision is also provided in the course of day-to-day direct work with residents, and the manager sets and assesses assignments for nursing staff undergoing conversion training. Supervision recording is held on a supervision file, work is underway to file the records on individual staff files and further formalise the recording of supervision so that it is more easily accessible. The home seeks the views of residents and relatives via annual questionnaires, this year’s form was to be distributed in November, the manager expressed intent to extend it to include health and social care professionals as recommended at the last inspection. Team meetings are held six monthly, it is recommended that meetings be held more frequently so that the views of staff can be presented, sought and recorded as to the running f the home on a more formalised basis. Views are also sought by the provision of a comment box in the entrance area. The home does not manage the finances of residents, but holds small amounts of money securely if required. If a resident is unable to manage their own finances a relative or other advocate does so on their behalf. The assistant director of the service audits a random sample of the funds on a monthly basis. Records are held safely and securely and safe working practices are maintained with the regular checking and maintenance of equipment. Evidence was seen that fire equipment had been regularly tested and fire drills had taken place. Accident recording was in place and any trends monitored.
