Conditions of registration: 1. That the home can accommodate 25 service users who require care for reasons of old age plus 1 named service user who needs care for reasons of mental health problems (over 65 years), 25 in Hollywood main unit and 1 in Evelyn Court Unit for reasons of old age with a mental disorder. That the Evelyn Court unit is staffed separately by a minimum of 1 person when the service user is in the accommodation. 2. Date of last inspection Brief Description of the Service: Hollywood rest home is situated on the corner of Chester Road and Orphanage Road in a mainly residential area of Erdington. It is close to local shops, public houses and other community facilities. The home is within easy access of public transport and on the main bus route for convenient access to Erdington. Formerly a family home with an attached coach house, the property has been converted and extended to provide accommodation for 26 elderly people. Bedrooms are provided over three floors consisting of seventeen single and four double bedrooms several of which have en-suite facilities. The registration for the home also includes a one bedroom flat next door to the main home. The flat is self-contained comprising of a lounge, kitchen, one bedroom, toilet and bathroom. All the rooms are very large, well decorated and the furnishings and fittings are of a very high standard. The occupant of the flat spends the majority of the daytime during the week socialising and taking meals with the homes residents returning to the flat at nigh time. Separate staff are allocated to provide support to the flat when the service user returns. Communal facilities are all on the ground floor and comprise of two comfortable lounges, two dining rooms, one of these having another lounge area leading off it. Service users also have access to another room known as the library, which provides a quiet sitting area when needed with toilet off. Bathrooms and toilets are located throughout the home and there is a shaft lift to the first floor ensuring service users with mobility difficulties have easy access. There is parking to the side of the home to accommodate up to six cars. Further parking facilities are available adjacent to the home offering a further nine spaces. At the rear is a large well-maintained garden with garden furniture, patio, barbecue and an aviary. Hollywood Rest Home DS0000065660.V278284.R01.S.doc Version 5.1 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. The purpose of the visit was to conduct an unannounced inspection. The outcome was determined by various means. In depth discussions were held with the deputy manager who also participated with the inspection by providing information, documents and a tour of the premises. Individual discussions were held with three residents, a senior carer and the cook. A group discussion was held at a dining table. Relevant documentation was reviewed, three care plans and staff training was checked. At the conclusion verbal feedback was given to the deputy manager. Historically this home has a good reputation and positive inspections therefore both inspections for the year have been unannounced covering all of the key standards and others deemed to be necessary. It is suggested that in order for a full insight into the home to be obtained that this report should be read in conjunction with the report from 9th August 2005. What the service does well:
The home has a deputy manager and senior carers. The home owner/registered manager and his wife are regularly present in the home and actively contribute to the service delivery. There is an effective management structure and sound levels of communication. The deputy manager is a registered nurse who takes an active role in ensuring that all healthcare needs are identified and met. The home provides a high standard of accommodation throughout, the decorations, layout and furniture is age appropriate for the client group. There is a high ratio of staff with extra being rostered when the home experiences busy periods. The workforce are well motivated and work together for the good of the residents. The maintenance programme is regularly reviewed, repairs and redecoration are ongoing. Emphasis is placed on resident’s safety and they are encouraged to view the home as their own. They are encouraged to participate with the extensive activities programme and are invited to stay for Sunday lunch. Meals are of a high standard and individual requests are catered for. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4 and 5 Documentation provides prospective residents with sufficient information to make an informed choice about the home. Pre-admission assessments and trial periods of stay assists the home in determining its ability to meet all identified needs. EVIDENCE: The statement of purpose remains the same and adequately reflects the registration category of the home for external professionals to make decisions about the home. A copy of the service user guide is supplied to each resident. A contract of terms and conditions of residency is supplied to all residents irrespective of the funding arrangements. The deputy manager commences an enquiry form; all contacts are documented prior to carrying out a pre-admission assessment at a convenient venue for the prospective resident. Residents who are admitted to hospital from the home are also re-assessed prior to discharge back to the home. This

Prospective residents and relatives are encouraged to visit the home as often as they wish and to socialise with residents and staff. They are invited to have a meal. A trial period of stay of 28 days is supplied and subsequent review carried out before a placement is confirmed. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 Resident’s health and care needs are clearly documented and files include evidence of a proactive approach to the services of external professionals. The system for the administration of medications is satisfactory and protects residents from harm. Arrangements are in place ensure that residents are treated with dignity and respect. EVIDENCE: Care plans are laid out in logical sequence and include assessments and care planning for physical and mental health conditions. Risk assessments are collated as well as assessments of skin integrity, nutritional status and medical details. Care plans are reviewed monthly including the risk assessments; the resident’s weight and body mass index is also determined. Files include the rationale and outcome of visits made by a wide range of external professionals. Observations revealed that staff possess an in depth knowledge of individuals needs. All medications are audited and any discrepancies found are documented and raised with the respective agent. Comments received from residents and observation of staff practices revealed that the dignity and privacy of residents is respected. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 Meals are of a high standard offering both choice and variety and special diets are catered for. EVIDENCE: Standards 12, 13 and 14 were assessed at the last inspection and were fully met with standard 12 achieving a score of 4. Residents spoken with said, “I go out to the shops”, and “ “I went to my daughters in the Cotswolds for Christmas”. A resident wandered into the library, she was still demonstrating exercises to music that had taken place in the main lounge. Lunch was observed being served. Kitchen staff were seen instructing carers regarding the likes and dislikes of individual residents. The home has two dining rooms and the smaller of the two is utilised for those who require prompting or assistance and these residents are invited to commence their meals 15 minutes prior to others to ensure that they are given ample time to complete the meal. The small dining room can seat up to six persons and two members of staff are allocated to the room to ensure that residents receive adequate levels of support. Lunch is the main meal of the day and includes three courses. Soup is followed by a choice of main course and sweets. Individual requests are catered for. The meals served were very well presented and appealing. Staff provided discreet assistance whilst

A senior carer used her knowledge of the resident and her skills in a successful result; the resident eventually enjoyed a full two course meal. Breakfast requests are catered for including a cooked meal. The evening meal consists of a light cooked meal, sandwiches or a salad. One resident commented, “ I really enjoyed that”. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 24 and 26 The standard of accommodation is excellent providing attractive, comfortable and a homely environment. Communal rooms provide residents with choices. The levels of hygiene are high in all areas of the home. EVIDENCE: The building has many interesting features, is furnished and maintained to a high standard. It is evident that people’s comfort, safety and choices are an important aspect of the services. There is a main lounge, small lounge, a library and two linked dining rooms. The extensive rear garden is attractively laid out and residents make full use of it during clement weather. Communal toilets and bathrooms including assisted facilities are strategically located throughout the home. Fourteen of the single rooms and two shared rooms include en-suite facilities. Bedrooms vary in size and layout. Rooms are personalised to the extent preferred by the occupant. There was evidence of personal possessions and

All bedroom doors have been fitted with alarms to alert staff of movement. The requirement for the fitting of suited door locks remains outstanding from numerous inspections. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 and 30 Well in excess of 50 of staff possess NVQ level 2 training and some have higher qualifications. The rolling programme of training ensures that staff have sufficient knowledge and skills to meet all care needs. EVIDENCE: Of the 20 staff employed 16 possess NVQ level 2, 8 have level 3 and 4 more are undertaking the course. All mandatory training, refreshers and other courses to meet the needs of the current client group are supplied on a rolling basis. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 35 and 38 There is an effective management team in place; staff have clear lines of accountability. Health and Safety procedures are robust in protecting residents and staff. EVIDENCE: The home owner/registered manager who has a wealth of experience was not available for the inspection. The deputy manager has achieved the registered managers award and NVQ level 4 in management. He has sufficient knowledge and skills to manage the home on a day to day basis. He was observed to have good relationships with staff and gave instructions to staff in a professional manner. The deputy manager advised that he is currently compiling a quality assurance programme which when completed will be comprehensive.

The storage and arrangements in place for financial transactions was found to be satisfactory. All aspects of health and safety issues including checks and servicing of equipment are valid. Fire alarms and emergency lighting are tested regularly and the results recorded. Fire safety training and fire drills are carried regularly. Storage of COSHH items were found to be appropriate. Safety is considered to be a paramount matter. Standard 36 was assessed at the last inspection and was fully met.
