What the care home does well

Holywell Park is run efficiently and in the interests of the residents. Residents live in comfortable and pleasant surrounding that are well maintained, refurbished as required and provide all the resources they need. There is a calm and pleasant atmosphere and residents are happy and contented. They are well cared for, like living there, are treated well and are safe. The management continually strive to build on and improve the service provided to residents.

What has improved since the last inspection?

A bound book is now used to record controlled drugs as recommended at the last inspection. In addition to this recommendation, the home has replaced its kitchen, further improved its call system and procedure and fitted a new roof to the annexe. Recruitment has been successful and has further enhanced the existing, stable staff team.

It has forty-eight bedrooms and two apartments accessed by three lifts. Some of the bedrooms have en-suite facilities. It is a large spacious house set in extensive, very well maintained grounds and is a listed building. There is ample car parking to the front of the home. There is a shop/post office approximately two miles away. A bus runs from Hodsoll Street to nearby towns and there are train stations in nearby Meopham and Borough Green. This was an unannounced inspection carried out by Wendy Jones, Regulatory Inspector between 10:00am and 12:15pm. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 Residents’ health, personal and social care needs are fully met and they are treated with respect. Medication procedures and storage arrangements protect the residents. EVIDENCE: The introduction of a new Standex care planning system has been completed. Care plans seen contained detailed information about residents’ personal and health care needs and evidenced doctors and district nurses are asked to visit when needed. Daily recording is now in these files. Detailed assessments have been carried out and added to the care plans. Medication was stored securely and records accurately reflected what residents had taken and confirmed that medication was being administered safely. Controlled drugs records are now kept in a bound book. Keys to the controlled drugs cabinet are kept in a combination safe in the medication room. For added security only senior staff have the combination of this safe. Residents said that all their needs are being met and they are very happy with the help and support they get. Residents clearly trusted and had a good relationship with them. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 25, 26 Residents live in a clean, pleasant, safe, comfortable and well-maintained environment. EVIDENCE: The home is situated in a rural area with extensive views of the countryside around. It has a large and airy entrance hall with a staircase leading to bedrooms on the first and second floors. Residents’ can also access their rooms by two lifts and there are grab rails and other aids that have been provided for them around the home. There are further bedrooms in a twostory annexe to the side of the main house, which has a lift to the bedrooms on the first floor. There are also two apartments in this annexe that have a living room and separate bedroom. Both have en suite bathrooms. The roof of this annexe has been replaced since the last inspection. Although there are a number of rooms that can be shared, all were being used as singles at this time. Most rooms have en suite facilities but there are communal toilets very close to those rooms that do not. There are four bathrooms with Parker baths and a shower room. A sixth bathroom is due to Holywell Park Residential Home DS0000023960.V266067.R01.S.doc Version 5.0 Page 13 be completed in the near future. Bathrooms have also been fitted with ceiling hoists where it has been identified that residents need them. There are a number of comfortably furnished and attractively decorated living rooms and a main dining room. A smaller dining room can be used for the dignity and comfort of residents who need more support than others when eating their meals. The manager advised that the communal areas are to be redecorated and new carpet fitted on the ground floor and stairs. Two samples had been chosen and the manager is waiting for quotes. There is a large car parking area to the front of the house and an extremely well landscaped and attractive garden area to the side. It was a very cold day but the home was warm and residents said they were comfortable. One resident said that they had been cold first thing but was now beginning to warm up. The manager advised that 6 new ‘bleepers’ had been bought and an extra ‘mimic board’ had been fitted on the first floor landing to ensure a more prompt response when residents use the call bells. She had also reintroduced the paper printout for the call system so that she could assess more easily how promptly staff respond to calls. Everywhere was extremely clean and there were no offensive odours. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27. 28, 29 Residents are protected buy a robust recruitment process and an appropriate number of skilled and competent staff are on duty at all times. EVIDENCE: There were 48 residents living in the home at this time. Staff were able to carry out their duties unhurriedly and efficiently and had time to speak with residents. In addition to senior and care staff a cook, laundry, domestic and maintenance staff were also working at this time. In addition the manager, was on duty. Although two staff had left recently to work in the local hospital, there were no vacancies at this time and the home was, in fact, over establishment. The manager advised that there had been a good response to the recruitment campaign and some good people had been employed, four of which had already started their NVQs. She added that she prefers to employ all applicants who would be a good addition to the team and be over established, rather than struggle and have to keep advertising. The head of care will be leaving in the New Year. But the manager was very pleased to have recruited an ex-community nurse to this role and was looking forward to them joining the team. At this time the head of care post will be renamed manager and a senior post will be made assistant manager. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 37 and 38 Residents benefit from living in a home which is well managed, safeguards their best interests and promotes and protects their health, safety and welfare. EVIDENCE: The registered manager is very experienced and runs the home competently and in the best interests of the residents. She has achieved the Registered Managers Award and is currently completing the NVQ4 in Care. The manager explained plans to further improve the service currently provided to residents. The atmosphere in the home was calm and pleasant and residents spoken with were happy and contented. The registration certificate and a current insurance certificate were displayed. The manager advised that the fire safety officer had visited the previous day and carried out a fire inspection. No major concerns had been raised and work to rectify minor issues that had been identified had already started. The kitchen and dry store were clean and hygienic. The kitchen has now been completely refurbished and everything has been replaced. A cook who had recently joined the team said they thought the new kitchen was very nice and they were enjoying using the new equipment.
