This inspection was carried out on 16th January 2006.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Staff spoke about the improvement in the general atmosphere in the home since the new manager has taken over. A relative stated that he could not be happier with the home and that all staff treats him relative very well.

What has improved since the last inspection?

The home has sought the advice of the Environmental Health Officer and has met this requirement from the last inspection. The home is working towards meeting the requirement on the fitting of self-closing devices on bedroom doors and the maintenance man was working on this during the inspection

What the care home could do better:

Temperature control valves are currently only in place on bath taps. This was discussed with the registered manager and it was recommended that these be fitted to all taps to prevent unnecessary risks to the health and safety of residents.

A maximum of four (4) persons over the age of 50 years in the category physical disability PD may be accommodated. 19th October 2005 Date of last inspection Brief Description of the Service: Horncastle House is a care establishment providing nursing, registered to accommodate up to 43 service users in the category OP (persons over 65 years), four of which can be in the PD category (physical disability under 65 years). A maximum of four persons over 50 years in the category PD may be accommodated. Horncastle House is a large detached property located in the village of Sharpethorne, near East Grinstead. The accommodation is arranged over two floors, which are served by a passenger lift. There is one shared bedroom. Horncastle House is part of the Sussex Health Care Group and is privately owned by Dr Sachedina and Mr Boghani. The newly registered manager is Mrs Linda Mountford. This unannounced inspection was carried out on the morning and afternoon of 16/01/06 and was facilitated by the registered manager. The home was inspected against the National Minimum Standards, residents, relatives and staff were spoken to. The requirements and recommendation of the last inspection have almost been met. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8, Residents health care needs are fully met. 10, Resident feels they are treated with respect and their right to privacy is upheld EVIDENCE: Resident’s Health care needs are set out in a plan of care. New care plans are to be implemented and documentation was seen to support this. The registered manager stated that this would improve information available to staff to enable them to meet resident’s needs. Residents stated that they are happy with the care they receive. Residents and relatives spoken to stated that the staff are at all times respectful and that privacy is respected. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15, Residents receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. EVIDENCE: The kitchen and food stores were inspected. The environmental health officer has been invited in to inspect these e areas and the home is working to meet those requirements within the timescales stated. Bins have been purchased so that foodstuffs are not any longer stored on the floor. The chef also spoke about he intends to improve the documentation in the kitchen such as temperature charts, cleaning schedules and to include hazard analysis. The meal seen being prepared and served was nutritious and appropriately presented. Residents stated that they enjoy the food. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,Residents and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon 17,Resident’s legal rights are protected 18, Residents are protected from abuse. EVIDENCE: The complaints book was available for inspection and showed that complaints were recorded and that they were followed up. There is a clear complaints procedure in place and this is included in the service user guide, therefore residents can be confident that complaints will be followed up. The home does not hold Power of Attorney for any Resident and relatives are encouraged to help in this area. Residents are given the opportunity to vote. The home has its own organisational policies on abuse. It also has the West Sussex Adult Protection Procedures available. Staff training is in place thus residents are protected from abuse. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, Residents live in an entirely safe and well-maintained environment 25, Residents live in safe, comfortable surroundings 26, he home is clean, pleasant and hygienic EVIDENCE: It is evident that work has been ongoing to improve the environment of the home. The requirement on fitting door closures is ongoing and is expected to be complete in the near future. The timescale stated on the last inspection has been extended. The home is safe and comfortable with the exception of water from taps in sinks, which do not have safety valves in place, which baths do. This was discussed with the manager and it is a recommendation of this report that these are fitted to prevent unnecessary risks to the health and safety of the residents. The home is clean, free from offensive odours and has pleasant communal areas. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29,Residents are supported and protected by the home’s recruitment policy and practices. EVIDENCE: The registered manager stated that the home is supported in recruitment by policies and procedures. These were available for inspection. Local induction is completed in the first 4-6 weeks and a new Care Training Consortium induction is being commenced within 3 months and this links in to The National Vocational Qualifications. This protects and supports Residents. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33, The home is run in the best interests of service users 35, Residents’ financial interests are safeguarded. 38, The health, safety and welfare of service users and staff are promoted and protected EVIDENCE: Residents spoken to stated that they are happy with the way they are treated in the home and that staff are caring. Staff stated that they feel well supported and that there has been an improvement in the running of the home since the new manager took over. Monies for Residents are not held in the home. Policies, procedures and training are in place in the home to protect and safeguard Residents. The home is working towards meeting the requirements of all statutory bodies. Yes STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1 Standard OP19 Regulation 23 (4) Requirement The building complies with the requiremants of the local fire service. This was a requirement of the previous inspection and a new timescale has been agreed. Timescale for action 28/02/06 RECOMMENDATIONS These recommendations relate to National Minimum Standards and are seen as good practice for the Registered Provider/s to consider carrying out. No. 1 Refer to Standard OP25 Good Practice Recommendations Water to be stored at a temperature of 60˚ and distributed at a temperature of 50˚ and to prevent risks from scalding, pre-set valves to be fitted locally to provide water close to 43˚.
