The inspector found no outstanding requirements from the previous inspection report, but made 2 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The home is nicely decorated and each of the Service Users rooms are personalised, one Service User having a picture board as they do not have verbal communication. The home continues to maintain it`s high standard of decoration and during the inspection a builder was present, modernising the kitchen. All the Service Users are out on a Wednesday, attending various day centres where they meet up with their peer group. On other days the Service Users have varying activities, which include sailing and shopping. Service Users have developed new skills, one Service User would not go near a train, and now the Resident goes regularly with the support of staff on trains and buses.

What has improved since the last inspection?

The home has now produced a good Service User guide that is in a format that is suitable for the Service Users in the home. It has a number of pictures that are suitable for the Service Users and is very detailed. The home continues to offer the Service Users a good quality of care, with documentation that details all their needs.

What the care home could do better:

The home does have a current issue around the interaction between management and staff. This does need to be resolved, as it will impact on the Service Users. The home does have good documentation, but some of the information has to be reached via a ladder and some information is in various places in the office. The home does need to make sure that all information is easily accessible for staff working in the home.

There is easy access to public transport, shops and leisure activities. Ground floor accommodation comprises a lounge, kitchen/dining area, laundry area, toilet, shower and two bedrooms. There are three bedrooms, a bathroom with toilet, a separate toilet and a small office/sleep in room. Service Users’ bedrooms are individually decorated and furnished and reflect their interests. There is also a small well-maintained garden with a large summerhouse. This is heated, comfortably furnished and has a TV and stereo. Five adults with learning difficulties live at the home and they are needing a lot of assistance and support from the staff team. Service Users have access to day centres and other leisure pursuits. The unannounced inspection took place in accordance with the annual inspection programme for this home. The previous inspection took place in January 2005 and was an unannounced inspection. A tour of the home was undertaken and a number of documents and files were seen. A number of the Service Users were seen and communicated with when the inspector returned in the afternoon. One member of staff was spoken to in the morning and two others in the afternoon. The Manager and the new Deputy Manager were present added valuable information about the home. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 Service Users moving into the home have the information that they need to make an informed choice. EVIDENCE: The home has now developed a new Service Users Guide, which at the last inspection was a requirement, as it was not user friendly. The new Guide is now done in a format that is user friendly, with pictures that would allow any potential Service User to have information that they would need to make an informed choice about where they live. It was not possible to assess the standard around assessments prior to moving to the home. The newest Service User was in 1998 and four others came together and have been at the home for a long time. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 8 and 9 Service Users care plans reflect their needs and choices in a format they understand. Service Users make decisions about their lives and participate in all aspects of life in the home. Service Users are supported to take risks and be as independent as possible. EVIDENCE: During the inspection a number of documents were looked at, which included two of the Service Users individual plans. One of the Residents had changed one of their daytime activities from the day centre to going out in the community. The care plan was done for staff to sign to the Service User that they were going out and where to. The outcome was for staff to arrange transport to the chosen venue (bus or train) and to ensure that the Resident takes their own money. There was a clear note on the Care plan that this Service User will not use the escalators. There was a section called the tenant’s daily log, this covered choices and control, competences, relationships, skills, physical and emotional as well as personal care. These are completed every day, responding to the changing needs of the Service User. Kingswood Road (47) DS0000025907.V257577.R01.S.doc Version 5.0 Page 10 Service Users choose where they wish to go, one Service User went sailing, the care plan clearly documented that they went sailing if they wanted to. Other Service User involvement documented was choosing where they went and whether they went on a bus or train. The minutes of the Residents meeting was seen and although these meetings are not held every month, the one looked at for July 05 involved a discussion with the Service Users about their holiday at Butlins. The home does support Service Users to take risks as part of an independent lifestyle; risk assessments were seen on the Service Users file. One risk assessment outlined issues about using the bath, assisting the Service User to access this independently but safely. One Service User is now accessing the community by travelling on the train with the support of staff would not have been able to do this two years ago. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 14 and 17 Opportunities for social and leisure pursuits are actively promoted and supported by staff. Service Users receive and enjoy meals that are healthy; all residents are offered a choice. EVIDENCE: Service Users do take part in a number and varied activities, at the time of the inspection all the Residents were out at daytime activities. Some of the Service Users attend a day centre where they meet their peer group and people they have known for many years. Service Users each have activities on their care plans with choices and documented what they do not like and what they like to do. Service Users were spoken to in the afternoon, when they had returned from their daytime activities, all said or indicated that they liked the food and one Service User said they really looked forward to their meals. Staff spoken to said the food was good and varied, the fridge looked at was well stocked with a range of foods. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 and 19 The Service Users’ physical and emotional health needs are closely monitored and this ensures that Residents’ needs are recognised and met. EVIDENCE: Service Users all require some level of support. In a Service User’s room a picture board was seen. This had a number of pictures around personal care and clothes that allowed the Service User to readily inform the staff what they wanted. The Service User’s file had Health needs documented with dentist, consultant and GP visits and appointments. All Service Users are registered with a local GP and these details are documented on their files. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23 Service Users feel their views are listened to and acted on and are protected from abuse, neglect and self-harm. EVIDENCE: During the inspection it was not possible to communicate with the Service Users individually regarding whether they felt their complaints were listened to. Staff spoken to were clear that any issues that the Residents had would be dealt with very quickly. There is a pictorial complaints procedure to assist Service Users; there were no records of any complaints for several years. Staff spoken to were fully aware of the procedure for reporting any abuse and were also very clear that any abuse, neglect or self-harm would be acted on very quickly. There was very clear information on the office wall that would give Staff immediate information as to where to phone or contact in the event of an incident. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 25 and 30 Service Users live in a homely, comfortable and safe environment, with bedrooms that suit their needs and the home is clean and hygienic. EVIDENCE: During the inspection a tour of the home was done and the home was very homely, comfortable and safe. The lounge was very nicely decorated with very comfortable looking chairs and settee. Although Service Users were out during the first part of the inspection, they had left their bedroom doors open and the rooms were seen through the open door. Each room was personalized, with pictures of the Residents choice and decorated how the Service Users preferred; this was done to a high standard. The inspection was unannounced and on arrival at the home, the member of staff and the new Deputy were busy cleaning. Despite there being a builder in the home fitting a new sink unit, there was no dust or building material to be seen in the home, the staff work very hard to maintain this standard. Service users benefit from clarity of staff roles and responsibilities. Service users are supported by competent and qualified staff. Service users are supported by an effective staff team. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32 and 34 Staffing levels are satisfactory and there is sufficient trained staff on duty to meet the needs of the residents. EVIDENCE: Staff files seen during the inspection had Criminal Records Bureau checks (CRBs) and references; the new Deputy file had a CRB but only one reference. The new deputy had worked for Mencap before they took a break in their career; the Manager felt that it was not necessary for two. The Regulations require that 2 written references are taken up. However, after a discussion the Manager said that he would request one from the other Mencap home. On the staff files were training records, the staff keep their own certificates, one member of staff had done Moving and Handling, first aid, fire safety, health & safety, COSHH and RIDDOR. Other staff had done: infection control, and food hygiene. Service users benefit from a well run home. Service users benefit from the ethos, leadership and management approach of the home. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38 and 41 The Service Users are currently not benefiting from a workforce that is working as a team. The record keeping is not helping the staff to safeguard the Service Users best interests. These factors could put service users at some risk, if not addressed. EVIDENCE: There is an unresolved conflict within the management and staff team, which, whilst not yet impacting on the welfare of service users, had the capacity to do so, if it is not settled fairly soon. The Manager is very keen to get this sorted out and said that he will contact the registered provider, to resolve this as quickly as possible. The records kept in the home are in-depth, however some of the information was in different places in the office. The Care Plan was easily accessible for staff, but personal details needed to be accessed via a ladder and another part of the information was in the desk drawer.
